It takes a very special person to care for those unable to care for themselves. Each and every one of the Moorpark Health Care Staff are that special person. We were so blessed to find Moorpark Health Care Center. To know that your loved one is being treated with dignity and care at a time when you are losing them is the greatest comfort one can have. From the moment we stepped into this lovely "home" to when we had to say goodbye, every member of the staff treated our family with genuine kindness. We marveled at how organized and skillful everyone performed their duties, and always with a smile. Please notice at the bottom of the Yelp page that there are 14 five star reviews and 4 four star reviews on the Yelp site that for some reason don't show up. I'm an RN and brought my dad to this place because I couldn't take care of him at his own home. If I had not been there myself to care for my dad I can't imagine how he would have been treated. The CNA was rude, and clearly burned out. She acted like doing her job was doing us a favor. She was rough with my dad and I ended up kicking her out of the room. The RN had to constantly be reminded to medicate my dad and I had to push her to get orders for stronger meds. My dad was on Hospice for pancreatic CA. Why should I have to educate their nurse on pain management?? The staff never even came in and said sorry to my family after my dad passed. It's a nice, clean facility, but that's it. If all you want to do is write a fat check then this is your place. If you're looking for quality, compassionate care, keep looking!

I urge you to carefully consider this before contracting with any hospice provider. Unfortunately, our experience with Faith & Hope made an already painful situation even harder. In April 2025, my mom suffered a massive stroke. She was placed under Continuous Care (CC), which meant she required critical, around-the-clock nursing support. We were told her case would be re-evaluated after three days. However, the care was stopped in less than 24 hours and we were given only 4 hours' notice that services would be discontinued. At that point, it was clear our needs no longer aligned with Faith & Hope's philosophy of care. We scrambled to hire private help on extremely short notice. That night, my mom was in significant pain & experienced agitation. She needed her medication adjusted and an increase of the dosage frequency to help manage her symptoms. The next day, we made final attempts to Faith & Hope to reassess mom's eligibility for CC, even though the on-call night nurse, when we asked, did acknowledge that we were free to seek another hospice provider. We waited all day for a response from Faith & Hope, but heard nothing. Faced with another evening without adequate care, we transferred my mom to a different hospice. They gave us the right care without hesitation. Sadly, our mom passed away only 4 days later. The only positive experience we had with Faith & Hope was with Nurse Theresa. She cared for my mom with compassion, dedication and a true commitment to patient advocacy. Theresa's thoughtfulness and support helped guide my brothers and me through an incredibly challenging time, and for that we are deeply grateful.

Communication with the doctor and home health didn't happen because I didn't know or convey side effects to the following: Do not use newfangled digital machines on everything! They are not tried and true and nothing like old fashioned methods. I talked, conveying concern to family members these were not good..turned, souring making injuries and a health trip to the hospital after home health had to remedy streaks from wounds! Tell your doctor they've done studies. Just say that was a bad move. Let's not try that again causing streaks! Next, I provided, asking to cover one of the wounds with a sticky band aid on hand! Bad mistake for home health because it went beyond blistering and caused panic! After getting The Wound Pros they said do it one way and I forgot and no one fully understood the potential of the wounds again! But it happened! Lastly, and no one could have anticipated and individuals don't need to know the details of repeated hospital admissions and rehab -- even a gentle alternative was too much for the wounds but through home health they got this patient moving again.

The last thing families need during these difficult stressful times is added stress from terrible cavalier, and lacking service. They refuse to accept their horrible mistake which is the TRUTH. I did NOT have a problem with mom getting discharge I am very thankful for it as I am thankful she transitioned from hospice to home health, I did have a major problem with their disgusting discharge procedures, their refusal to take responsibility for the wrong information Maria gave me would affect my elderly mom's well being. My mother's discharge was atrocious and very poorly handled, yes she was given a 2 week supplies and equipment after discharge however this does NOT excuse them for the terrible treatment the last day. I would even consider it inhumane. 2 days before the medical equipment was supposed to be picked up a nurse named Maria informed me that NO date was actually set for moms medical equipment to be picked up. (Wrong information) 48 hrs. later and about 1 hour before mom's bed time a receptionist by the name of Samantha informs me by telephone that law enforcement would be called out if I did not surrender the hospital bed, I was shocked and panicked and in utter disbelief. Yes I elevated my voice I was outraged for the handling of their mistake and the misinformation they gave me and to be told the police would be called out if mom did not surrender the hospital bed. I had to call public health to investigate their terrible discharge practices, ** the investigator advised me on 2/16/2024 to share my experience so others would be made aware of my experience. They did NOT care when I informed them that mom would be sleeping on the floor because of their terrible mistake, the case manager named Vera korkanovic declined to speak to me to defuse this matter, or to find a resolution. The owner Tali Fayfel refused to speak to me as well. She later calls screaming at me. The receptionist screamed at me talked over me and repeatedly told me moms hospital bed was going to be picked up that evening no matter what! In panic and in the evening hours I began taking out some medical equipment outside, I had no idea how mom's health would be affected sleeping on the floor that night. ****The only nurse that stood out from everyone was a nurse named DAMARIS she was consistently caring, professional, and patient. ** I did email hospice to let them know I would be sharing this horrible experience so other families would not have to go through this. Sorry if you view this as threatening. * Social worker did not return calls/ text messages for days. * Case manager refused to take my calls a number of times when I needed guidance for discharge. Resources for your family for unethical treatment/ or to file a complaint regarding care & treatment * CA Dept of public health 714 567-2906 * OC COUNCIL OF AGING 714 479-0107 Wow , you expect professionals to be professionals. Beyond disgusted. Please think twice before using their services.

    If I could give this organization a 10 I would…read more Placing a loved one in hospice is so difficult. You question whether you are doing the right thing. These hospice nurses however, are the kindest, most gentle, and loving people I have ever met. Mark, Jhewel, and Carmelita were ALWAYS there when I needed them. Whenever I called, a real person ALWAYS answered-even on the weekends, after hours, whenever! Honestly, hand to heart, these people are angels on earth and I feel incredibly blessed that my mom and I were able to experience such love and kindness during her last days. She passed less than a week ago but I think because of their work I can bear the loss much better.

I am so happy with their care and I'd like to share why...I am respected, they honor my dignity and are very professional. It's very hard to be a patient that is in need, vulnerable and is 90% housebound. it makes being in a position like I am in easier when greeted by a caring nurse who is ready to help make a difference. i feel so thankful because i am healing. I know it's my job to be positive and take good care of myself in between my nurse visits and doctor visits. I have an exceptional team of doctors and I've shared with every one how exceptional the Asisted Home Health model of caring for the sick and vulnerable is. I can barely walk because I've been in need of a hip replacement for many years, but due to having an open wound I'm unable to get the hip replacement until my wound heals. I've got 5-6 cm presently June 11, 2026 to go, the wound was 14 cm deep. Since being cared for by Asisted Home Health and receiving aseptic care both at home and at the CMH Wound Center I have healed nearly 3 cm, and I'm so happy! It's slow healing because of radical radiation from stage four cancer, I'm also a survivor. My nurses are so loving to me and their supervisor makes sure I get all the supplies I need for optimal healing, and he's very well respected by his nurses. The leadership is exceptional at Assisted and encourages and supports their staff and nurses. My nurses have shared that they've been employed with this company a long time and are very proud to work for Asisted Home Health. I am nearly 74 in one month, I brag I'm nearly 3/4 of a century and I believe I'll heal by my 75th birthday completely. After being diagnosed with stage four cancer I was given three weeks to live had I not started radical chemo and radical radiation, which I completed barely. But after my second cancer related surgery a large boxer dog broke thru my backyard fence and caused me to get hurt as I was saving my little dogs life from the boxers jaws. An abscess developed 14 cm deep inside me because I had to pull and pull the boxer off my little dog going four feet into the air and down onto cement pavers as I broke my dog free. Sadly later the abscess broke thru my body and developed Into this open wound, it's a very sad story. The good news is I saved my dogs life and Kobe my mini schnauzer lived seven more years but I was really harmed. It's been a long journey healing but I have never given up due to my excellent doctors and nurses who truly care about me. Asisted is one of the best Home Health Agencies I've ever had and I hope they'll assist me to finish line healing this wound. I'm healing, but very slowly. I'm also getting exceptional care from Dr Alicia Haglund at CMH Wound Center, I really like her. And I believe someday around my 75th birthday I will get that hip replacement and be able to walk again and ride my bike and feel the wind on my face again. I loved riding my bikes In our beach city, I have a cruiser and an e-bike. If you have been refereed to this agency don't hesitate you'll be in exceptional care. Stay proactive in your healing and recovery. Help your team of doctors and nurses be successful helping you. They all work hard all day and need our kindness and appreciation of their care. They also need you to do your part and take the best care of yourself in your healing process and recovery. I love Asisted Home Health not just because I'm getting exceptional care but because they respect me and treat me with dignity. Before my doctor retired, Dr John Dexter wanted me to end my healing journey with them. I know they'll be a part of the journey to my full recovery and I'll be one of their success stories, and we'll all dance with happiness. My nurses call me a blessing and they tell me how they enjoy caring for me and coming to my home. Thank goodness...Molly has a lot to say about visitors in her home. Thank you Asisted Home Health from my heart and soul for all you do for me and for others. It takes a community and you're making a difference in my life and also in our community.     Thank you to Assisted Hospice for helping our family through our mom's passing. One day my mom…read morecould no longer get in and out of bed on her own. I called Assisted and within hours we met with a nurse and a social worker (Ariana and Jessica) had a bed delivered along with oxygen, a nebulizer, and meds. We were set up with a wonderful health aide - Elvia, three times a week. Throughout mom's journey, the Assisted team were there with loving care and excellent communication. When my mom decided in her last days that she wanted Last Rites, I called Ariana, who called the spiritual advisor and we had a priest at our door within the hour. I can't speak highly enough of the professionalism, expertise, love and care showed to my mom and our family by the whole team. Thank you from the bottom of my heart.
