Lost about $600 in insurance reimbursement last year for cleaning & xrays, because they were entering my insurance number one digit wrong in UHC system. They told me at least 6 times it was done correctly on their end and even handed me a claim form once and told me that I could mail it. New dentist is in my network, $0 for cleaning & X-rays and has not started adding 3% fee for using credit card yet.
