We were so grateful to find St. Anne's Mead on 12 Mile in Southfield. There are various approaches to memory care but we really like SAM's approach the best. They utilize an approach called Best Buddy...where SAM actually hires someone to keep the seniors engaged informally. They have PT and OT of course elsewhere, but I love that this person's main role is to sit with a resident to share a cup of coffee or visit with someone alone on a couch, etc. The Program Director has a very sweet little dog she brings 3 days a week and the residents light up when they get to hold or pet her. The staff are all very respectful of the residents and do seem to like them greatly. They have many activities scheduled throughout the day so there is no sitting and staring at the television for hours. Their meals are delicious. As noted by other reviewers, the building is an old-style nursing home, but the interactions, smiles, and kind staff make it a very hospitable place. The memory care unit was recently renovated with nice size rooms and large private half-baths (showers without assistance can be quite dangerous for seniors with memory issues). They have their own dining area and integrate into the activities of the larger facility as much as possible. Don't be put off that the building is from the 1960's. It has everything you will need for quality care for your loved one who is struggling with memory issues.

While the building is on the older side, the staff at St. Anne's was absolutely fabulous. My mother had the opportunity to return to the senior living apartment as she got stronger, but she didn't want to. She was happy staying with the wonderful people at St. Anne's. My mother passed away this week, but I am very thankful for the staff at St. Anne's helping make her last year as pleasant as possible. Thank you.

    Don't even deserve 1 Star... hi my name is Javon I worked for this place as a caregiver for about 5…read moremonths now..... it's a very horrible and a outdated place they overcharge the residents that's there and stay putting higher prices on rent they did have a good 25 people there but as of today 09/23/2024 there going to be down to 14 or 12 due to most of there residents leaving. If you want proper care or service wouldn't recommend putting you're mother father or grandparents here due to the nurse never being in she would come in maybe for 2 hrs 5 on a good week if you see her for more than 2 days there you're lucky . Management have a stick so far up the tail it doesn't make any sense .... They say if you have issues you can speak to management but every time a person do talk to management they doc the person hours or they would mess you your pay. Speaking of pay.... Payroll will mess up your hours then tell you you have to wait until you're next check to get your messing check...... in the healthcare industry family do not suppose to work together but that place is literally ran by nothing but family meaning everybody in there is related to someone else who's in a higher position..... they show favoritism only giving extra hours to people they favor if they don't like you. there's not putting you on shift.... The boss would get her "favorite worker" Ubers nd Lyfts and anything else he ask for but when it came to other workers she would cause a sense... I had a patient there who had a bad skin condition in the cream she needed took the nurse 5 months to get in between the time Frame of her not caring I kept requesting it to the point I had to get cream from other residents..... if it wasn't for the care team (night shift to be exact) and a few day shift members than the people there would not be taking care of. For the place to be in a nice location the building and the management is horrible. You have workers who come in the day shift in drinks on the job you got messing ceilings outdated walls the carpet ain't been cleaned since 1964 they just replaced the back door alarm after 3 months of the workers complaining about our safety. lol y'all don't have to believe me I just used to work there until they fired me for speaking up but if you don't care about your family then i highly recommend them because they sure will not give a rat tail about them nether but if you care about them i highly recommend you don't send them here.     This place is now called SKLD…read more My dad went here after being treated for a mild case of COVID at Henry Ford West Bloomfield. I live in California, so I couldn't check it out in person, but I wish I had. They were clearly overwhelmed, but still taking in more patients. Communications were a total mess. I was never able to reach staff when I called (phone just rang and rang), and when they occasionally did finally answer, it wasn't clear they even knew who I was talking about. I was never able to talk to my dad on the phone - it, too, just rang and rang. The social worker did arrange one short zoom video call. Otherwise, he was totally out of touch. Dad died at SKLD after 5 days. They discovered him unresponsive, but couldn't even get it straight when they had last checked on him. And when we went to pick up his clothes and effects, they had lost them. This pandemic has been very difficult all around. But SKLD was nowhere up to the task.

Sure things could have been done better but I think any medical experience could have been done better. For the most part things were kept clean and neat, the staff was caring and friendly especially the therapists and the night staff. I was only in for 3 days so please do not take my word as the end and be-all, but I think my view can at least give a viewpoint that most people would not get. I went in after pneumonia and left-side weakness straight from Beaumont. You begin with OT and PT (and I was told Speech therapy if needed, but I did not need it... so I cannot comment on it). Each consists of 30 minutes a day. Again the staff was friendly. They explain what is to be expected and are there for you and care about you. The *main* concern I have is the lack of personal information/HIPAA would have a field day with this place if they only knew half of the stuff that went on. I worked for 20 years in healthcare and I was amazed about the lack of personal health information that got passed along/shared/etc and was not protected. I am normally a quiet person and do not "listen in" but I could not but help myself as staff openly talked about other patients in front of other patients as if it was something to be expected. So while the rehab and facility itself are good, the staff is nice, some additional training on personal health information might be very helpful.

Instead, what we experienced left us heartbroken, frustrated, and ultimately fearful for her wellbeing. We had multiple meetings with management about her care plan, her daily needs, and the fact that she was a known fall risk (she was a dementia memory care resident) We made her needs very clear, repeatedly. Yet her care plan was rarely followed, and vital information was not passed down to the staff, or completely ignored by those who were supposed to be caring for her. Watching this play out time after time was devastating. Our biggest struggle was with management. Our interactions with Kim Briley, one of the owners, were discouraging and dismissive. Instead of responsibility or solutions, we were met with excuses and comments that made us lose confidence in the facility's leadership. She told us directly that many of her staff were lazy, improperly trained, or didn't have a high school education. She also made the comment that "they can't be watching her 24/7," which felt like a complete disregard for the fact that we were paying for supervision, safety, and adherence to her care plan -- especially given her being documented as late stage dementia and a fall risk. Amid the chaos, a few staff members stood out for their genuine compassion: Jessica, Tiffany, Colette, Christina, and Kiera. They cared about our mother and tried their best despite the lack of support and structure from management. They were the few bright spots in an otherwise deeply disappointing experience. The breaking point -- the moment we will never forget -- was when our mother fell and fractured her hip. In our view, this was preventable had her care plan been followed and communicated properly. After everything we had voiced and tried to address, this outcome was devastating. Obviously there was an ambulance ride to the ER followed by surgery and rehab. We removed our mom from Grace immediately. Not once did Kim reach out to check on Mom. Another disheartening find was learning that the facility is not even licensed by the State of Michigan! This only added to the pain and regret we felt. We trusted Grace Senior Living with someone we love dearly. That trust was broken. Based on our experience, we cannot and will not recommend this facility to any family seeking consistent safe, reliable, or compassionate care.

What really stands out are the activities--there's always something going on, and it's clear they put real thought into keeping residents engaged, active, and social. My mom genuinely looked forward to participating and always had something fun to talk about afterward. Most of all, the staff is incredible. Every single person we interacted with was kind, patient, and truly caring. You can tell they don't just "do a job"--they really care about the residents and treat them like family. My elderly mother was very happy during her time there, and as a family, we felt so supported and at ease knowing she was in such good hands. I'm very grateful for all the amazing people at Grace Senior Living and everything they do every day.
