Meds not administered correctly. Sometimes not at all. Sometimes left in room for her to take herself. Went without water a lot of the time. Filthy conditions. Carpets swept with a broom rather than using a vacuum. Sheets not changed for weeks at a time. Restorative care not done. Weight checks not done on a regular basis. Call lights ignored for 1-1 1/2 hours.
