I am here for physical therapy. While the facilities are not the greatest, with outdated and incomplete therapy equipment, I rate them 3 stars. However, the therapy department staff is knowledgeable and friendly. Katish and Moe deserve 5 stars, and so does Kevin. The nurses are actually better than expected. I hear what they have to put up with. Some of the names I have heard night staff be called because they weren't there immediately. I get why the aren't in the best of moods, 4 stars. It is not like this happens often but I am not the one who upset you. Now there are a lot of people who come in and out. Not all of them nurses and I cannot tell who is who sometimes. Especially when I am half asleep. Now for Maxine. There aren't enough stars available by their scale. 10 stars. She has been amazing, she consistently goes above and beyond. I thank everyone staff wise. It is the facilities you lose a star on maybe it should be a 3 star because them, but the staff gives you a boost. If you have options and can pick somewhere you go. I recommend going somewhere else and for me Staff has been great but the facilities, equipment and pest issue. Until these issues are corrected. do not choose here. read more

Heartfelt Review of My Sister's Care My sister is currently a resident at this facility, and as both her family and a nurse, what I have witnessed has been heartbreaking, overwhelming, and unacceptable. I feel compelled to speak out so other families can make informed decisions about where they entrust their loved ones. My sister has a deep, gaping wound on her coccyx -- roughly the size of a grapefruit. Despite the severity, I have not seen the level of care, urgency, or clinical judgment that a wound of this magnitude demands. She spends nearly all day in bed, wrapped in layers of blankets -- not for comfort, but to mask odors that should never be present if proper wound care and hygiene were being provided. What hurts most is that I tried to have her transferred to a facility closer to me, where I could visit more often and personally monitor her healing. Instead of being treated as a concerned sister and a healthcare professional, I was dismissed and spoken to as if my knowledge and advocacy meant nothing. I suggested a wound vac -- a standard, evidence‑based intervention for wounds like hers -- and was told she "can't tolerate it." I can't help but question whether this decision was truly about her tolerance or about the facility being for‑profit, prioritizing cost over care. If my sister didn't tell me what was happening, I wouldn't know. I work long hours, and I trusted that she would be cared for. That trust has been broken. This review would be completely different if I felt the staff respected family involvement, honored patient‑centered care, and communicated openly. Instead, I've been made to feel invisible -- and my sister has suffered because of it. They may have changed their name, but they have not changed their care. I urge families: Do not send your loved ones here without thoroughly reviewing their history, inspections, and violations. Your loved one deserves dignity, respect, and competent care. Through all of this, I hold onto faith. God is the greatest of all time, and I pray for healing, protection, and accountability. read more

I WOULDN'T EVEN PUT MY WORST EMEMY IN THIS DISGUSTING, UNLIVABLE, CARELESS, ROACH INFESTED PLACE!!!! You walk through the front door and a horrible horrible strong urine Oder just smacks you in the face, to where you're scared to even inhale. My grandmother arrived Friday evening to this hell hole to hopefully regain some strength back. I went to visit her Sunday afternoon only to find her nose touching the wall, lights all off, tv off, no bed alarm on when she's a fall risk, water on a tray out of her reach, and no AC what so ever, we both were sweating like crazy. I turned on her light and she was covered in her own urine and feces, her bed protector was dripping urine all over the ground, dead bugs all in her bed, roaches running all over the ground, blood covering the sheets and blanket just pouring from her elbow. She was still in her hospital gown from Friday!!!!! Clean clothes we provided still sitting in the bag next to her bed. She was begging me for water and so dehydrated . I broke down in tears, crying so hard that these "nurses" would treat someone this way. My grandmother stated not one person had moved her since she arrived. But you know what the nurses sure did do??? MAKE SURE THEMSELVES WERE TAKEN CARE OF, Yelling in the hall there McDonalds order, brushing their hair at the god damn "nurses station" I asked for the DON, unavailable. I asked for the nurse, she came in 45 minutes later. I stated I wanted her discharged NOW. All we got was nasty, nasty attitude and not one answer to any of our questions. NOT ONE!!!!!!!! No one, no one at all should be treated this way, this place has not one care in the entire world for there patients and every single nurse there should have there license ripped away from them. I'm glad I got my grandmother out of there because who knows what this place would've done to her. You'd think after all the reviews stating the mistreating this place does, this place wouldn't exist. Don't even bother hitting me with the stupid reply of "please call and speak to our executive, your concerns are our top priority, we thrive on blah blah blah" ITS ALL A BUNCH OF BS AND WE ARE WAY PAST THAT. - Sincerely one hurt, disgusted, pissed off granddaughter!!!!!! read more

This place disgusts me and should be shut down immediately. We will do everything in our power to make sure it happens. My grandmother was brought there Friday evening. We went there to bring her more belongs to find horrific things. My grandmother in a ball, NO AC, roaches on the floor, bugs in the bed, still in her hospital gown from Friday, laying in her own urine and feces. If you absolutely hate your family member this is the place to put them. No rehab, no doctors and an exquisite version of rehab run exactly like planet of the apes. Incompetent administration will assault you then lie to police, will interfere with your specialist visits to make you very sick, food expenses is about 4 dollars à day and the aspire group controls theml you have to fight for everything you need to survive and thus should never have been approved. Place filled with bugs, and thus is probably the worst administration that ever existed. So if you want your loved one to suffer then this is the place to put them. They will overdose you to incapacitate you so you can't walk and when your heart is jumping out of your chest they will not provide you with a doctor nurse or ambulance you cannot conceive if the incompetence and cruelty of these animals. Be careful they charge names to hide. They have been known to not complete your papers to keep you here and interfere with leaving. I was admitted to Aspire Rehabilitation at 6 pm on Wednesday December 20th. The first sense you receive upon entering is an offensive odor of urine. Upon arrival, I had an intake done & discovered that I was sharing a medium sized room with 3 other patients, with only a thin curtain for privacy. I selected daily showers in my intake forms & no one has assisted me in showering over the course of 3 days, besides my daughters. The shower chair is an unwashed bariatric commode (that was unsafe for me due to the large hole in the center of the chair) & the door does not lock, my daughter had to lean on the door to keep it closed while I was showering. Loud alarms go off for no reason 6-10 times per day. My left side mobility is severely compromised due to several brain surgeries I have had in the last decade.  Normally, when I am hospitalized or in rehab a bed alarm allows me to receive assistance when I stand up & want to walk. There are no bed alarms at aspire and the bed head & feet move up & down mechanically with a crank at the foot of the bed which I am incapable of reaching currently, they are also all flimsy twin beds. I asked for a bed with rails & was told I would receive one after 24 hours. 2 days had passed & maintenance staff informed me that there are no beds with rails in the facility. I fell at 7 am on Thursday 12/21 & had to scream for assistance from the floor as the nurse call buttons are not responded to at all (not after an hour or any amount of time). My daughter called staff 5 times each morning on 12/22 & 12/23 with no response to her or myself, as I fell again & was left alone on Thursday morning. The meals have zero flavor & on 4 separate occasions I was given thin liquids when I must have nectar thick liquids due to a swallowing issue & risk of aspiration. The only walker available is wobbly & physical therapy staff suggested I bring my own walker as their supply is short. I fell again at 5 am on Saturday 12/23. Upon discovering the lack of care, basic sanitation (water damaged drawers, filthy kitchen, & bathrooms) and safety practices my family arranged for my documentation to be shared with a different rehabilitation center for transfer & our social worker, Anita, was remarkably kind & efficient in getting the documents transferred over. I was approved for transfer on Saturday 12/23 & when my daughter shared this info with the Physicians Associate (PA) & nursing staff they claimed a discharge could not be carried out on a weekend & I would have to sign forms stating I was leaving AMA  (Against Medical Advice) which could invalidate my insurance & force me to pay for this horrific stay out of pocket. The PA said she is going home and she refused to write any further prescriptions. The nursing staff were remarkably rude, indignant, dismissive, and unprofessional. My daughter had to fiercely & assertively advocate for my health in order for them to even call Anita, the social worker. As soon as Anita was called, she immediately stated that she would drive an hour on her day off to accommodate us & finalize the discharge process. Anita put more effort into my care than a single medical professional working at Aspire. Anita is a saving grace amidst these poor excuses for humanity. If you cannot afford having your loved one cared for at a different rehabilitation facility, I suggest you care for them in your home & have family , friends, & neighbors stay with your loved one in shifts while others attend to work & other family responsibilities & alternate. Beware, your loved one will be treated worse than a stray animal at Aspire. A more fitting name for this care-less facility is Expire. read more

This place is absolutely HORRIBLE! My father was referred to this facility by Lawnwood hospital, after receiving treatment. Not only was my sister and I not contacted and informed that he would be transferred to this facility, but noone from Aviata even bothered to contact us upon his arrival. DO NOT place your loved ones here! Had I been made aware that the would be going to this facility, I would have never approved and made arrangements for him to be placed someplace better. I am extremely disappointed in the lack of care he has received here. I received a call stating that he has not received proper hygiene care and I can't tell you how this has made me feel. In addition, I called to personally speak with the Supervisor and was told that he was not aware that my dad needed a bath, excuse me! Baths happen everyday! What kind of care facility is this?! I am working to get him out of this place as soon as possible and I'm warning anyone who reads this, please DO NOT allow Lawnwood medical or any other referral source to place your loved ones in this hell hole. I will also be reaching out to the so called hospital, to file a complaint as well. This place has one star! Why would they place him in his awful place?! This place needs to be shutdown! read more

I will be filing a complaint with the state based on the fact that Vohandra the social service worker and Keisha the business director failed to return my calls and request to have my fathers transfer paperwork sent to his transfer nursing home. They neglected my fathers needs to have all of his paperwork sent to his new nursing home after requests got over a week from myself and the new nursing home. My father was transferred to this disguising facility after he was released from Martin south hospital. It smells like urine throughout the entire facility and most of the upper management staff is very rude and will not return calls. It took me several request to have them send his medical documents for evaluation to transfer to new home. This place is a joke don't ever let your loved ones stay here. read more

This place is the worst. My nephew was sent here from Longwood. They keep him sedated so his family can't check on him. They won't answer phones or return calls. They are worse than inmates are treated in prison. Other facilities allow visitors with temperature checked and mask. They just don't want anyone to see how poorly they treat patients! We are very pleased with Ft.Pierce Healthcare. My husband has improved greatly. Now I may look forward to him coming home! Everyone is friendly,my questions are answered. There is a very special aide I need to mention,Joselin Collelo,she has been fabulous with my husband. She attends to him as quickly as she can and is very patient and sweet with him. If you love you family members you will NOT let them stay here. I am appalled that this place is even in business. When I stood and watched the cockroaches crawling in and out of the medication cart I was so disgusted!! I was even more upset when I told the nurse what I saw and she said that it was not a big deal because all of the medication was individually wrapped and the cockroaches couldn't get into anything. Most of the staff members I interacted with are miserable! The families/residents are not cared for with dignity, respect and compassion as they should be. It is visibly filthy and does not have adequate cooling for a building in South Florida. The staff have lost my fathers personal belongings not once but twice. I am so happy he will never, EVER step foot into such squalor again! read more
