Prior to going to Form I was prescribed by 2 of my other providers weight loss medication but my insurance said I must use Form. From the beginning of the appointment the Doctor was rude. She complained about my zoom not working the way she wanted. She complained about not being able to read the medical records my doctor sent. In the end she said she wouldn't prescribe me weight loss medication because she didn't like my eating habits and I needed to make lifestyle changes instead. I don't drink, I don't smoke and I don't do drugs. She said I would have to see their dietician for a few months and then she would consider prescribing me what my 2 other doctors who see me regularly felt I needed. I am not accustomed to paying someone to be rude to me and I shared with her I felt she was being rude. I decided not to use their services after this appointment. I had already scheduled an appointment to see their dietician but there is no option to cancel the appointment. I wrote their customer service to ask them to cancel my appointment the agent said she wanted to speak to her lead first. What I got instead was that they sent me 2 scales and then wrote me a letter saying they wanted my feedback which I did not respond to followed by a letter that said I was being dismissed as their patient. If you get the same doctor I did, ask to be reassigned unless of course you like someone treating you rudely. It's a shame that so many others have had a positive experience and mine was the worst medical appointment I have ever experienced. What is even more pathetic is that because I complained in the letter that dismissed me they try to make it sound like it's my fault by saying i was rude because I complained. TALK ABOUT NO ACCOUNTABILITY.....and by the way even after they dismissed me they keep asking me if i need help setting up the scale​

The thought of getting on a scale and being fatter as opposed to less fat is a good motivator. Their prepackaged foods - while a money-sink- are an easy way to keep track of points and snacking, and generally, I don't feel stabby or starving for missing out on foods I like. The meetings, however, are just not geared toward dudes. To be fair, I am usually the only dude in the meeting,so they probably shouldn't cater to me. But, since they are advertising using the dudely Sir Charles Barkley, and making it kind of man-centric in their marketing, their materials should be somewhat reflective of this. Recent tips included "Share a main dish with your girlfriends at lunch." Either they are substantially overestimating my social calendar or they are talking to women. But for the past two years I just have semi-annual check-ins that don't provide much value. They manage my GLP-1 prescription and have mishandled the most recent dose increase. It's been sitting for 2 weeks with no action; and haven't answered multiple voice and Gateway messages. Then today they sent me an email as "a patient who has confirmed they are no longer taking GLP1"!! Once this one is sorted I'm switching control to my PCP.

They are adamant that you're making the right decision which is why you must go through visits with nutritionists, psych MDs, surgeons, etc. I feel GREAT! I had Dr. Witkowski as a surgeon and truly, a well mannered, wonderful, down to earth guy. I've joined the gym on his recommendation (insistence). I've lost 30# from pre surgery weight and it's only 2 weeks post op. The nutrition department is important in ensuring your knowledge of eating and food selection is over par. I'm highly impressed.
