But, from the time he was admitted, he was welcomed, made comfortable in his spacious room. They are very well staffed & call buttons were answered within minutes. None of the staff ever acted bothered or too busy to take care of his needs. This place is very clean, my husband liked the food (we heard they had a new chef). His PT & OT was first rate! They were so supportive & encouraging. They have a nice outside garden area the pts. can go sit & talk. The cafe is always open for visitors with complimentary beverages. They are open at lunchtime for hot meals with a very reasonable prices for visitors. The entire staff was extremely helpful to us, from the doctors, PT staff, nurses, CNAs, charge nurses, managers of this facility go above & beyond. My mom has…read morebeen here for several weeks and was transferred from an acute rehab for continued nursing and physical therapy. It seems as though the care provided has been no different than had we put her in hospice... mind you, we are nowhere near that point in her life. Her weakness has only gotten worse, physical therapy has not been done as we were advised it would be, and she continues to lose weight, her muscle strength, and has been laid up in her bed for more than three weeks now with very little movement. She has become stiff and rigid in the position that she's in and is very painful to even attempt to move her into a sitting position. Communication from the staff and her case manager has been extremely poor. I was notified last week on Wednesday that Medicare was only providing one more week of treatment for her and have yet to be provided a discharge date. I have left several messages for the staff with no response and still don't know when I need to set up transfer for her to depart that facility. Based on the timeframe provided, it would appear that I have 24 hours or so from now, yet no information has been provided to our family on when she will be discharged. Her time at this facility has been a complete waste of time and only further set back any progress she had made prior to admission. While Mom says "most of the people are nice," she also said that they very rarely even check on her and never move her. Please do your research to find the best fit for your loved one. We thought we had, and while we understand that skilled nursing is a lower level of care, we feel that there is zero excuse for the lack of treatment she has been provided. It is also important to note that many times when we have been there visiting, there have been other patients crying out for help from their rooms that has gone on for more than 10 minutes before anyone responds to their request or urgency. This is uncalled for! The staff is there to care for these patients and they hardly acknowledge the calls for assistance... definitely not in a timely manner. We are extremely disappointed with our experience and how our loved one has been cared for, or lack there of.

    I want to start by acknowledging Brittanya, Kaylee, and Cammy. My mom genuinely loved them. They…read morewere kind, attentive, and took wonderful care of her and they deserve to be recognized. However I have serious concerns about the overall care at this facility. At one point the floor was left wet and my mother fell and hit her face. My mother is a Type 1 diabetic who recently had a stroke and should have been on a diabetic heart healthy diet. That was not even close to what she was receiving. I walked in one day to find her being served ice cream, cranberry juice, potato salad, and a sandwich on a roll so large I could not believe what I was seeing. All of this for a Type 1 diabetic recovering from a stroke. What made this even more alarming is that it was literally marked on her chart to give her a high carb diet. Someone made that decision and put it in writing. When I raised the issue they did change it, however they would still regularly serve her things like large egg salad sandwiches loaded with yolk on an enormous amount of bread. She is smart enough to know to only eat a little from the middle but she should not have to be outsmarting her own care team's meal choices. Thankfully I had been bringing her her own food in the meantime. Her blood sugar was regularly spiking to 350 and on her last day it reached 400 to 500. Staff were saying they did not know why her blood sugar was swinging, while simultaneously feeding her a high carb diet per her own chart. I was not always there and did not always have access to her Dexcom, so who knows what it was reading the rest of the time. Blood sugar at those levels in someone recovering from a stroke can cause brain damage. This is not a minor oversight, it is negligence. Additionally there was a nurse whose name I was unable to get who had a completely straight face, never smiled, barely replied when spoken to, and treated my mother as though she was an inconvenience when she needed her insulin. There was zero warmth, zero compassion and zero acknowledgment that she was caring for a vulnerable human being. Insulin is not optional for a Type 1 diabetic. It is life or death. I genuinely cannot believe someone like that is working in patient care. I also want to note that I brought my mother $100 worth of scratch tickets from North Carolina, something small to lift her spirits during a very difficult time. They were stolen. Taking anything from a vulnerable patient in your care is unconscionable. I am giving an extra star solely for the exceptional care shown by Brittanya, Kaylee, and Cammy. The systemic issues at this facility need serious and immediate attention.

They had encouraged us to discharge home, but my dad was still unable to walk, get out of bed, roll over or another kind of independent action at that time. We talked to Tammy at Sage Creek. She was so calming and reassured us that my dad would have the care he needed, they could provide transport for the office visits he needed for his heart surgery and would have the PT, OT and SLP that he needed. We were told at the rehab that a skilled nursing facility was going to just leave him in bed and he would lose any progress he had made with them. This is absolutely untrue for this facility. They got my dad up and dressed every day. He had therapy. The therapists were so patient and kind (Alexa, Vicky, Willy and all the others I'm so bad with names) Mo the nursing supervisor went above and beyond to take care of any concerns that we had. Judy, Tyler, TJ, Brandi, Lindsay, CC, Rosann, Malik and other nursing staff were all amazing. The facility doctors were wonderful with communication and the care they gave. The facility was clean and the food was good with lots of options. Danielle and Nicole in transportation were amazing taking my dad to his many doctors appointments. Paige, our case manager, worked so hard for us. Anything we need to get my dad in the VA Home she came through for me. Absolute godsend! We adopted the staff here as family and we already miss them. Their kindness and thoughtful caring was beyond any of our expectations. I 100% recommend this facility

It is a bit deceiving though cause when you initially walk in, it looks like a nice place, the rooms are updated but and yes there's a but.. the nurses aren't attentive to your loved ones in the room. My grandpa is currently in here trying to get rehabilitated to get moved to an assisted living or back home and he even hates being there. He presses the call button and they rarely checkin on him. He had to use the restroom as he's still able to do on his own, with assistance and they told him to use the diaper he's in cause they were short staffed and couldn't take him. I'm sorry but no, that's not right to let your patient "sh*t the bed" cause you can't grab another nurse or CNA for a few moments to help someone use the restroom. I've also noticed in order to get any sort of nurse in the room you have to walk up to the nurses station for them to actual come in and help, since they ignore the call button. I will say though the nurse he has right now has been very helpful and polite with him since I've been coming out to see him. As well as another nurse who was super helpful this weekend, but again they were only helpful when we called upon them at the station. However, the other nurses don't seem to really care much, they just sit around and socialize and they seem to have a little attitude when you ask questions, as we are just trying to get answers when his actual nurse isn't right there. The last few times I was in there the room was a mess. The bathroom was worse, the pee bottle he had on the toilet still had pee in it. He was laying with a dirty stained blanket all day before they came in and changed it. If you're the nurse or the cna look around your patients rooms and see if anything needs to be picked up. Make your patients feel comfortable even though they don't want to be there.

The facility is beautiful in a Tuscan theme. The staff is amazing, they are the nicest empathetic group of people. The food is a good variety. The only qualm I have is my dad isn't able to feed himself and has been going without food. He won't ask for help because he doesn't want to bother anyone. I would recommend this facility to anyone and everyone. Thank you for taking the best care of my dad.

    My 92-yearl-old dad stayed and rehabilitated at AHC (Sunset and Jones location) for a couple of…read moremonths. He had suffered stroke-like paralysis on one side for both his arm and leg. We thought his recovery was an extreme longshot at best. However, thanks to the truly superb staff, he was able to slowly but successfully regain his ability to walk as well as a lot of his abilities to be somewhat independent. This would not have been possible without the amazing medical assistants, nurses, physical and occupational therapists, wound care specialists, case managers and even their maintenance man. When dad first arrived, and for most of his stay, he was completely dependent on AHC staff for bathing, toileting, getting food, and all. The staff was so attentive, patient and understanding (he did get grumpy and stubborn at times as he was dealing with a lot of medical issues;-) They never missed a beat and motivated him to keep working hard and improving. Without hesitation, I highly recommend this AHC for rehab. A few names I must recognize: Himani, Jordan (therapists), Mary, Janet and Chanel (the CNA's), Luz the head nurse, her daughter the woundcare specialist (forgot her name:-) and Arnelle the maintenance man. Arnelle didn't even have anything to do with patient care, but he had such a positive, smiling and motivational presence and got to know our dad and kept him smiling and laughing on a daily basis which is hard to do when someone has just lost their independence and is in pain. A huge thank you to the entire team; they are really like a family.

    I've been coming here for about five months now, and I honestly don't want to stop, this place has…read moreknocked at least ten years off my body. I'm one of those older guys who always thinks he can do more than he probably should, so I'm constantly pushing myself to the limit. That usually takes a toll, but these guys keep putting me back together and keeping me active. I'm physically stronger and in less pain now than I was 20 years ago, and that's because of this place. The therapists here are extremely knowledgeable and great at finding the root cause of issues instead of just treating symptoms. I'm incredibly thankful they keep me in the gym and on the road, running virtually pain-free. And yes, they now see my entire family, because they're that good. They currently treat my son for a knee issue and are amazing with him, especially since he was nervous to come in because of his autism. My wife goes there for herniated discs, and they've helped her feel stronger and so much better. Even my daughter came in after waking up one morning with severe back pain, they fixed her right up, and she didn't even want to stop coming once she hit 100% (we finally had to tell her she was good!). I've been three times and every time I feel better and have a better range of motion. All of the staff work as a team and are very friendly and knowledgeable. I went without a referral and paid cash. I am very pleased with the results thus far. I highly encourage you to try them out if you're experiencing any pain, you don't have to live with it! They will help you! They use a variety of modalities too.
