After years of building trust within the community, the company went bankrupt and sold to CenterWell Senior Primary Care, leaving many longtime patients -- especially veterans relying on Tricare as their primary insurance -- feeling abandoned. Veterans who served this country deserved continuity of care and transparency, not sudden disruption and uncertainty. Many patients had established relationships with their doctors for years, only to find themselves scrambling to locate new providers and navigate insurance confusion during an already stressful transition. What makes this even more frustrating is that public statements during the sale process suggested patients would continue to have access to their insurance coverage under the new ownership structure. Yet many patients later experienced uncertainty or loss of access depending on their plan participation and contract negotiations. The bankruptcy itself was tied to massive Medicare billing discrepancies reportedly totaling hundreds of millions of dollars. Unfortunately, it feels like the people who paid the price were the patients -- particularly seniors and veterans who depended on stable healthcare access. Healthcare organizations should prioritize patient care and loyalty during transitions, not leave vulnerable populations feeling discarded. I sincerely hope veterans and affected patients receive the support and continuity of care they deserve moving forward.

Impressions: Friendly, helpful including Security. The Nursing staff I interacted with were friendly and dedicated and focused, extremely knowledgeable and seemed born to this job as a well oiled team. The CAT scan Impressions were detailed and addressed long standing health chronic conditions never found in any previous CAT or MRI including recent. They were missed for many previous years and now getting the attention they deserve. Doctors on hand are available and follow your progress. FINAL THOUGHT FOR 2 VISITS: Every possible test was run for a complete DX to get you on proper meds and get you home. Merry Christmas.

    I had hoped to give them a 5 star review but i can't as there is no follow through and misplaced…read moredocuments which effect follow appts. Facts/overview 1st ever apt may 2nd with follow to review bloodwork and my history (i dropped off reports from past mris, pap biopsies, dexa scan, liver test etc on may 5). Follow up apt on may 14th - 1. my original intake form never uploaded so i was being asked all the same questions again 2. The reports with my medical history i dropped off on may 5th were lost. No one could find them 3. My apt was at 4:30, i was seen at 5:30 4. The follow up apt was to also compare bloodwork from past reports but my reports were lost. He had a tough time believing my past cholesterol levels which i told him were much higher but no reports as they were lost 5. They were to have ordered medical reports from past doctors but nothing had been done 6. Michelle apologized and promised to call me with an update as to where my records were and why nothing was uploaded or ordered. 4 days later still no call Sooo disappointing. I can NOT recommend them though they are all nice but nice doesnt cut it when we are talking about my health. Dont go there both offices contributed to the mess

    I have to switch PCP's. It has nothing to do with Abu Azizullah. He's a great doctor. There's a…read morewoman who works in his office who makes everything super impossible. Her name is Christine. I had all of my records sent over from my old doc when i moved here. Including my shot records. I needed those for school. She told me that they dont keep shot records. So I asked her for her fax number and had them refaxed. The lady at my old doc office also called and confirmed the fax number with Christine before faxing. I was told by Christine, that it would take 24 hours to recieve... I came in after that time, to pick them up. She claimed they werent there, refused to look for them, claimed they dont have shot records... I reminded her of our last conversation and how they were faxed. She said she would call me "if" they turned up. ...I drove the 3+ hour round trip to get a copy from my old doc. I wasnt going to play Christines games... even though their leesburg office is like 6 miles from my home (and the records are definitely there). .... flash forward, I found out I needed a physical form filled out by my PCP... so I called the office to make the appt. She said only their 2nd office was available any time soon. I chose today at 10:45am, as I had a 10am appt to get my TB results for school, in town. I had to get to that appt and then I needed to be in Orlando at 1pm for my special needs baby who has 4 appts today. So i thought 10:45am would work well. ...Then she got me. She got me so good, she doesnt even know how good she got me. She gave me an Ocala address. I asked if there was a suite number, she said no. Cool... I'm driving there... get there... it's a huge estate/residential/horse farm. Thinking gps was wrong... I try to access it from another direction off of another road. Same place... not the doctors office. So I call the doc office. Speak to a very sweet girl. She gives me the address. Its 12 miles away which is 22 minutes. This after I'm already late, and later still for trying a 2nd route to the same horse farm. Now, Christine doesnt no my car has no a/c. She doesnt know my clutch is trying to go out (we take my husbands van when we take the baby anywhere, but I try to leave him with his van incase of emergency, if I leave alone.) ... I finally get to the area in Lady Lake, where the office is. I go to the first internal medicine office- its not Abu's... back in the car... driving around looking at names on buildings... I find it. Im an hour late... its 11:45am. Theres no way to get home in time to head to Orlando for my sons 1pm appt. I'm in tears because me wanting documents to get into school (which I thought would ultimately better my kids life), is hindering the doc appts he so desperately needs after spending 182 days in the nicu. I feel like I'm letting him down... all because this lady has a sick sense of humor and a miserable existence. So, I was up front with Mr. Azizullah... told him I couldnt afford to come to him anymore. Told him I didnt have the time in a given day, to play her games... it cost my family dearly, today (the next Speech evaluation isnt until Feb, so this sets his ability to learn to eat, back months). Christine works at both offices. Theres no way to avoid her, though today she wasnt in the receptionist seat and didnt check me out after the appointment either. So theres a silver lining... not sure what she was doing, but she was off to the side sitting at a computer and not interacting with patients (thats a good role for her). Doc Azizullah was present, standing behind her, witnessing her beligerence at the other office when i went in asking for my shot records. He looked taken aback by how she was confrontational and rude when i was calm and communicative... but today when i told him he had to lose myself and my husband as patients because of her, he was stone faced. Anyway... go forth accordingly... he seems to be a very competant doc. ... I just cant afford the time and games :( ​

He did an evaluation and determined I had a mild case of pneumonia and was in respiratory distress and immediately arranged for transportation to the hospital. He was great
