My sister unfortunately has to be in this place. They leave the patients and NOT check on them, depending on who cares, never mind if I walk up to the nurses station and I stood right in front they not made an effort to lift their head to ask if i needed something, they worked really hard not to look at me. Lies, lies, lies. I needed help and the intake guy Cage was amazing and told me everything I wanted to hear. I got there and it was all untrue. I hopped the fence, they called the cops. Cop found me waiting in a hood waiting for a ride. He helped me unlike Haven staff. I could read his screen. Haven said I was aggressive to staff. This never happened. My daughter was in 3 hospitals in 2 months and never had skin issues. She has been at this place since Thursday March 26th. The first 12 hrs in this facility she had a bad rash from not being changed for over a hr sitting in her feces. This happened on Night shift care. I filed a grievance and they said please let us remedy this. She hears people call out for Hrs asking for help because call lights get ignored. Wound care came in and gave her creams that helped they rash went away for 24 hrs then Again last night she ended up with a rash Again from sitting in her urine and feces. I filed another grievance. All I get is We are so sorry. My daughter hears night staff say, Oh she went again, I just F'n changed her. Maybe stop giving stool softeners and laxatives if you are unable to care for the patients. My daughter is 30 yrs old. Had a cardiac arrest from a Pulmonary Embolism. Bled out for 13 hrs. Had over 55 blood transfusions. Had kidney failure and is still alive. Talking and had a trach. The trach options in Tucson is Worse, worser or worsest. How is this the care we have humans. She feels like animals get treated better and until the trach comes out I can not move her to another facility. The bottom floor seems clean but second floor is gross. It smells like mildew, diarrhea and urine. Today after cleaning my daughters poop they left the nasty diaper in her room. I can not get through to the facility they have it going straight to voicemail. My daughter called at 7pm tonight and I talking to her now at 8:15pm I hear her calling for help and she is ignored. Her light is still on I am so discussed by the care for my daughter how is humans treated with such neglect. Nurses will not help to clean patients they just say you need to wait for your Nurse Tech. As a Mom I hope none of the staff here ever has to have their children put in a place like this. Until my daughters trach is safe we can not bring her home. As she is yelling the staff are laughing I can literally hear them. Staff were very professional and helpful for my mom. She spent two weeks there for rehab after a hospital visit. It always seems that folks only share the negativity. I am not claiming that the facility is perfect. I believe most of the people are doing their best to take care of the patients. I could not do the job. Most of the time the staffing level seemed okay. I am sure they would appreciate more help. Glad that folks want to help out those in need. The RN and CNA that dealt most with my mom were awesome. Amy is one name. I can remember the other. Wish I could. My mom was able to go back to her assisted living facility to continue physical therapy. After a fall that fractured her wrist, my 88 year old mother was discharged from TMC to Haven Health. Long story short, she left in worse condition than when admitted. She was in great pain so of course they pumped her up with Oxy which made her lethargic and barely able to participate in PT sessions. Consequently her strength lessened, her legs began to atrophy and her mobility was nonexistent. I live out of town so trying to establish communication with this establishment proved utterly frustrating. I was always sent to voicemail and my calls were rarely returned. I was told of an instance where my mother had a few bites of a cookie and inadvertently left some crumbs on a table then went to a PT session. When she returned the table was covered with ants. Yes ants. They blamed my mother for leaving the crumbs. I finally had to travel to Tucson to speak to them and see first-hand what was going on. The food was barely fit for a dog. The bulb in the light above the room door to alert staff had blown out. An AC unit mounted on the wall made terribly loud noises. When we told a staff member about it she laughingly said it always does that. Swell. While her left hand was in a cast, somehow without explanation her right hand became purple, swollen and very painful. Nobody could explain it except to say it was over used while trying to compensate for the bad wrist. So now she has two bad hands and can't bear any weight for a walker. She lied in bed for hours without anyone checking in on her. I witnessed it myself. On top of everything else, there was a company that sits in on the assessment meetings that places patients in facilities if they have to move from independent homes. They receive commissions if they find a place. This was the case with my mother. One day I received an email from Nick with a rather high pressure sales pitch to move her to one of a list of facilities he had ready to go. I relented and accepted his list and the offer to take my wife and me to various facilities. Nick's wife Jayla met with us and gave us the tour. Nick and Jayla are fine people and most certainly had the best of intentions but at one point Jayla showed me a picture on her phone of a dietary restriction in my mother's file. A blatant HIPPA violation. Also, Nick informed me that my mother's insurance coverage was ending in three days and she would face a discharge hence the urgency to find a place. It smelled fishy as he informed me of the impending discharge before the rehab center did. The Haven "Health" experience was borderline detrimental to my mother's condition. A rehab center's purpose is to rehabilitate meaning improve one's situation. The opposite was the case for my loved one. It's entirely possible my mother will never regain her mobility after a hellish three week stay at this facility. This is our experience and not necessarily everyone's but it warrants the one-star review. Never again with Haven Health. read more

Not sure if care can get any worse. I've been visiting my friend here. They're supposed to give the people showers once a week. She's been here 2.5 weeks and still hasn't received a shower. They've only helped her to the bathroom to brush her teeth once now in 2.5 weeks. It's supposed to be a physical rehab, but they don't do rehab on the weekends. Seriously! Don't think they're not charging the same ridiculous rate on the weekends, because they definitely are. The food is beyond terrible. The care is pretty much non existent. Some people are nice, but just not effective. I don't believe they have any real nurses here. They definitely don't have any doctors. They say they have doctors on call. I would say on call pretty much means they're gonna Google answers to their medical questions. Funny thing was Banner hospital tried to suggest this place. They gave a brochure that said this was a 5 star place. Look up reviews anywhere and you will see the truth about what people experience here. This is the the place off Swan rd. This person was here for a broken femur bone. She was a fall risk for sure. It took over a week to get them to put rails on the side of the bed. Everything about this place is a joke. My original review I notice is on swan Rd location for the Haven! It's supposed to be for the Haven of saguar valley physical therapy rehab. The location was and is currently 6651 East carondelet Tucson 85710. I hope this clears things up. However, I also wanted to add to that review. If I can find it. It was not a good review whatsoever and if I cannot find it I will rewrite it. I do want to mention the wonderful people that are here and cared for me, physically mentally and emotionally! These are the few, I repeat only a few... Sadly at this facility that I was able to have mutual respect. These people take their work seriously and are here with their hearts and souls! I want to mention them by name. And of course thank them. Firstly, WILL , nurse. Not to mention he works two jobs and up to 80 hours a week! He is the kindest man I have had the pleasure of knowing in this facility! He treated me with utmost respect, he would listen to me every shift he was on here when he brought my medication in to me. When I needed to vent! I've never known such a good listener. And heaven knows I needed one, and he was there for me... Emotionally. He has so much compassion for the other human being. You can see how much he loves what he does in the medical field. He never treats you less than.... As so many many members of the staff here do. Not Will. He also has a wonderful sense of humor, we would both laugh. And at times I had to realize... Hey, I am not his only patient. So I would tell him, enough of me... Now go on to your next person." She has had it pretty tough her own self., however, she is always everywhere here and home to help others. She has listened to me so many times. When I had no one, I will miss her immensely. When I had no one to help me when I could not get in my wheelchair she would help me hang my newly cleaned clothes in the closet, this may sound small to anyone reading this but Judy, cares so much she would brighten my day just entering the room. At first I only knew her by phone as when I needed an extension and she was the' front desk gal' so kind, when I was frustrated about anything... She would listen as well, mind you, by the way, by this review... You would think I was complaining to everyone or spouting off my frustrations to everyone! Lol. Just a few of these wonderful people. My sister was admitted to Haven Hell two days ago after having a major surgery. The minute she arrived she called me and told me it was a mistake. Unfortunately I live in another state and couldn't pick her up right away. She had to spend the night there. She told me the bed was so uncomfortable and the sheets smelled like urine so she sat all night long on the chair. They supposed to give her medicine at 8:00 p.m. but around 4:00a.m. The nurse finally gave her pain and sleeping pill. Food sucked. Breakfast was two pieces of small sausage with a stale bread. When in the morning the nurse brought the breathing device it was not a new device and god knows it was in how many patients mouth. My sister refused to use it. Thank god my sister's neighbor was available and came and took her home. DON'T TAKE YOUR LOVED ONES THERE. They get out of Haven Hell even worse than the day admitted. read more

I was admitted to the facility for rehab for fractured femur. it was the worst experience. only good thing I can say was the PT/OT therapists were very informative and helpful. the rest was horrible. they also patients to have TV on and loud all night as well as night shift having a party. Nurses only pass meds giving med at 5 am. when you use your call light expect 1-2 hours before it is answered and longer if you need a med didn't get pain meds for 48 hours, nursing never did assessment or check post op incision. the food again sub par. and unable to eat. I have been at Haven Health for 22 days and before that 12 days in the hospital. I was transferred here because I had to learn to walk again and gain a lot of strength back. The first few days I thought that this will never worK. When those first few days were over you all of a sudden realize that you have made some improvements. The OT/PT teams are masters of deceit by being purposely almost indifferent while at the same time making you realize you're making progress. Same with bathing and toileting. They expect you to try and if you Insist you can't they will only minimally help and before you know it you can do it. We have all heard the horror stories about nursing homes. Granted it's not a luxury hotel but you're here to get better. They have both single and double rooms. The food could be better but it's not awful. I wish the food could get from the kitchen and still be hot. If you ask for something extra they will go out of their way to get it. So hopefully having taken the time to read this that perhaps your opinion of nursing/rehab facilities could be different. read more

My family member was transferred here from the hospital. She was left in bed 24.5 hours a day. Except for Anabel and Zach, the staff treated our family like a nuisance. Rolling their eyes when we arrived. Yet, WE were the ones to put in the hearing aids in mornings and take them out at night. This facility needs a severe inspection and change in management. Having been in other facilities, this place is a joke except dealing with lives, not a joke. The food is inedible, not fit for a dog or a pig even! Quality, quantity and selection all strike out. Was told they are only allowed 4-5 dollars per meal. Am anemic, lost 4lbs in here and no action taken. The call button consistently goes unanswered, 1-2 hrs at a time. Teamwork not in their job requirements. Have watched a CNA stand in front of my door, not assigned to me and my CNA busy offer no assistance. When I am having a heart/breathing issue escalated assignment not of importance, Help me! Weekends are a worse nightmare. Cat away, mice play. You have to advocate for yourself in this place, document everything, take pictures of every reading and time, have your own OMRON blood pressure cuff, demand to have emergency inhaler by your bed if breathing issues. There is no consistency on how blood pressure taken here. One puts cuff below elbow, one make feet on floor, one legs flat in bed, some old fashioned do it yourself, etc.! Traveling DR that only knows what nurses care to record so Dr not aware. Medicines given improperly. Last two days in extreme pain where dropped in a weight chair not locked but if said anything would of not discharged me and needed to get real medical care. 5 times daily of diarrhea, blood pressure high, improper medicine, denied prescription medicine from My pulmonologist, dehydrated etc. Physical therapy said I am in shape to go home. Well home long enough for bloodwork to be done and nurse practioner to view me and immediately told to go to emergency. Severe C-Diff, lung infection and broken tailbone both sides! Surgery required. When I get well and strong enough I will continue action on this house of horror!! The only reason even one star rating is can not proceed if nothing checked. read more

PLEASE DO NOT PLACE YOUR FAMILY HERE: My father was placed at the Haven Health Tucson to Participate in physical therapy to prepare his body for radiation treatments because he has cancer. He arrived communicating, in good spirits and with decent mobility. He left today near death as he was transported to the hospital. He was only placed at the Haven for physical therapy. However, once he was unable to participate in physical therapy, our family should have been notified so that we could have move him to a suitable assisted living facility. Instead they would not return calls for updates and kept my father in a skilled nursing bed so that they could bill Medicare for physical therapy he was not participating in. Meanwhile, they are not giving him proper care and he is getting sicker. The social worker Denise NEVER RESPONDS to messages unless you threaten to file a complaint with the Ombudsman. The Director of Nursing, also does not respond to messages unless you threaten to call 911 to their facility. Staff do not know how to care for terminally sick patients, which is why we should have been notified when he was no longer receiving a benefit from their facility. My dad is not a pawn to be used for Medicare payments. One of their doctors is involved in a lawsuit (the doctor told me this) and a lot of their staff is incompetent. I came to Haven Health facility after I had a total right knee replacement and spent 20 days here. The care that I received was top notch. The CNA's that helped me were fantastic and always had great attitudes. Thank you Maria and the rest of the CNA's you were great. The nurses that I worked with;, Amy, Stephanie, Chris and Christian were all amazing! They made sure I was well taken care of and showed such compassion towards me and my current situation that if kept me at ease. Thank you Stephanie and Amy for showing how real nurses take care of their patients. And let me not forget to mention the whole PT/OT staff who were amazing! They wasted no time in helping me get prepared for going home and I loved all of their bright and cheerful attitudes. Care was just so, so. 20 minutes of physical therapy and 23 hours and 40 minutes of laying in bed. After my moms dismissal, my brother and I tried NUMEROUS times for a call back from Denise concerning my mom but it never happened. Texts, calls and voicemails went unanswered. Great location ( n swan westside n of rillito River) , newer building than most . Under trying pandemic times ? Communication is difficult, no phones in rooms . Told many elderly have cel phones? Friendly switchboard operator has to call nurses station on floor of patient , if they answer ?? They bring a cel phone to patient to talk, not always answering at nurses station . Told physical therapy only in patients room twice a day due to covid virus. No patient movement outside of room for exercise or socializing with anyone else. It's frustrating not to have reliable communication with the patient! I had to come to Haven because I fell and tore my muscle or something from my hip and thigh. The staff here really cares about the patients and the therapists here are great they seem to really care about getting you better so you can go home. The only bad part is the food. It seems like the cook wants to make sure I at least don't eat there have been many meals that they take back because to me I can't eat it. Had grilled cheese tonight the bread was soooo hard I could not bite into it, so I end up hungry most days. But the rest if the staff is great! Some of the kitchen staff are very helpful, just not the cook. I gave 4 stars because the therapists and nursing staff were great but I would give the kitchen 1 star because I can't give a negative. They need to replace the main cook. He doesn't listen to what his staff says the patient wants. read more

I refer to this place as HAVEN HELL. I arrived on the afternoon of October 3, 2022 to Haven Hell to find my mom unresponsive in bed with someone else's clothes on. I asked my mom how she was doing and she did not respond so I put my hand on her shoulder and tried to wake her up. She was largely unresponsive so I asked the staff to call an ambulance. Please note, I had to ask them to call 911 as my mom was unresponsive. They chose to ignore this and were fine leaving my mom in this condition by herself to die, I guess. Upon arrival at TMC the medical staff frantically worked to stabilize my mom and they ran a bunch of tests including blood tests to try and figure out what was wrong with her. Her blood tests results showed that her glucose levels were critically low. I immediately went online and checked my mom's blood test results from HAVEN HELL to discover that her glucose levels were similar to her levels at TMC. I am sickened by the fact that the negligent staff at HAVEN HELL were not monitoring my mom's blood test results to even know what was going on with her. I called the Director of Nursing at HAVEN HELL to ask her about this and she told me that the blood tests they do are not always accurate as the blood degrades before it is tested. This in itself is negligence. Why take blood and run tests if you aren't going to check the results? Haven Hell FAILED to monitor her blood test results or were too incompetent to understand them and as a result my mom almost died at HAVEN HELL due to critically low glucose levels. I have a complaint in with Medicare and I am just starting my mission to spread the news regarding this HORRIBLE place that played a role in my mom's DEATH. Yes, my mom has died. read more

PLEASE DO NOT PLACE A FAMILY MEMEBER HERE!!!!! My Mother was placed here after hip surgery, for PT. One example of the mistreatment I observed, My Mother needed help to get up to go to the bathroom. She told me she had to go, pushing button brought no one, so I went to the nurses station. I was told the person who did that was doing vitals right now but they would send her to Mom's room when she was finished. Went back to Mom, she tells me "they won't come, I will just have to pee myself". I felt so helpless. When I left after my visit, still no one had come. When I walked by the nurses station, there were 5 of them all sitting there talking and laughing. I am in tears every time I leave her. My Mom told me, "you have to get me out of here, they are so hateful". Her room mate told me they put Mom's pants on her in front of everyone in the gym, during PT. My Mom wasn't drinking her water, I notice how dry her mouth was. I tried to pour her a glass, she told me it tasted bad. When I asked one of the staff, she said they give them tap water, but if Mom asked they would give her filtered water from behind the desk. REALLY!! Don't even want to get started with the FOOD! Her roommate was taking pictures of it, to show me. They should be ashamed. The people who work here need to find a new profession! They should not be in charge of caring for our loved ones, they have no compassion. I was told by Denise, to give them a good review, that all they get are BAD ones. My two weeks at Haven of Tucson is the worst experience of my 77 years. I was taken there from the hospital for a complication from my hip surgery. I was unable to put any weight on my right leg. I was brought in by their transportation, which was great and their driver was super. He brought me into the facility and I was taken to my room and placed on the bed at 6:15 PM. By 8:30 nobody came into my room and I was unable to move because the walker and wheelchair were on the other side of the room. I pushed the call button but nobody came by. I finally hollered and a CNA came in and brought me the walker. I didn't see another soul until 9:30 the following morning. No dinner, no breakfast. The center of the mattress had no filling in the middle and I was attempting to sleep on basically bare metal. I found out after a week and a half my call light did not work and that possibly was the reason I waited for 90-120 minutes for a nurse. 2 hours for pain medication is a long wait when I was instructed to get ahead of the pain with medications. The meals all came cold, I was taken to the shower one day and there was no hot water, the bathroom door is installed on such a way it's almost impossible to get in with a walker or wheel chair. The smell in the bathroom was unbearable and I had a can of Lysol brought in to me so I could attempt to deodorize it. There's so much more but I think you get the idea. I must say their PT/OT department was excellent, the director of nursing was very accommodating, the visiting doctors are wonderful and some of the nurses are great.
