From September 26-October 5 I was in this great rehab facility. When I first got there I was so impressed at how beautiful the place is. All the nurses were so nice and attentive. This place is garbage. Feces in the counters of patients room. Very dirty. They don't disinfect the rooms. We complained about how dirty it was and all they said was "we have someone coming tomorrow to clean the room.." ok soooo where they putting the patients if they have to disinfect the room?!? Don't think about coming here. It's very hot and use fans to "cool off the patient".. the nurses are rude and have no sympathy for patients. They had patients in beds in the hallway. Outside looks nice but one you walk into the rooms look beat up and not kept up. It's smells like dirty diapers and looks like psych ward . The showers for patients is a community shower like prisoners have in prison. These nurses bath/shower these patients in the open. This place looks like garbage. I felt like I took care of myself more than I was taken care of. I am not picky but most of the food was not edible. Administrative services is lacking work ethics and follow -up. There are a lot of good nurses and support staff but I did witness a couple of them treat my roommate and other patients poorly. read more

It's loud in every hallway, some of the rooms play loud music it also seems like they're over staffed but no one is doing anything. (If that makes sense) This facility is ridiculously outdated, abandoned, dirty , NO AC and it's not sanitary at all! Looks like the walls and floors are ready to tumble to pieces. The beds are more than 50 years old and do not function properly. The pictures on their website are very deceiving! They only show the nice part of the building. LITERALLY anything. my father has gunk all over him we asked if they could please get his nurse to clean his face and cut his nails. I been working at MGPA since 2021. MGPA has given me the opportunity to grow as an employee. I started off as a CNA for 2 months then got promoted to RNA for about three months then got promoted to work with the Rehab Department . Which I have been working for this department for about two years now. I started as a Rehab Aide and now as the Rehab Coordinator. No building emphasizes rewarding hard work and care towards our residents than MGPA. The leadership team sets a standard that is fair and adheres to ensuring quality care for our residents. Terracina won me over! This place is a well maintained facility. There's always a cleaning staff seen cleaning, a maintenance staff working on a project. We're always there weekly to visit my aunt and the nursing staff is outstanding and very attentive to my aunts needs and ours. The administration of this facility really goes above and beyond to take care of their patients. Julie the social worker has never let us down. Other than the RNs and a couple of aids, this place was disgusting. No real blankets, the floors were dirty in the rooms. There were long delays when asking for assistance. The day I went to pick up my mothers belongings, the front desk called the nursing station multiple times and no one answered. My Sister has been at Madison Grove since 6/22/2023 Station 4, And I'm not sure how long she will remain there. But what I do know is the staff that has cared for her in station 4. Has been very caring. They have gone above and beyond to make sure that my Sister feels safe and as comfortable as they possibly can. Her wound Care Nurse Martha, Always encourage her , And since Martha has been caring for her wounds everyday. 3-of her serious wounds have healed. She has a few more wounds that needs to heal . But I confident that Martha will get them all healed. As for the CNA's They have played a big part of her recovery. Making sure she doesn't feel alone when we can't be with her. Chris... is one of the first CNA's I met. He just simply sweet and kind. I typically see him because he works nights. And I go see my Sister at night. But my sister tells me about the other CNA's and how good they are to her. Juan, Jennifer, W'Nisha, Cherish, Maria, Jessica, Miguel, Como, LVN: RN Paulette, Monica, Glenda, Shelly. I know I have missed a few names of Nurse and CNA's. My Sister journey started last year in June. We have been to several Hospitals Post Acute facilities , Rehabs. But this Facilities has been the best for My Sister. Because of the caring staff. We understand that they don't have all the finest amenities and upgrades. But I rather have my Sister here, then to have her at some of the other facilties that she has been to with the upgrades & nice amenities. and a not so caring staff. Our family would like to show our appreciation to The All Staff in Station 4. Purchasing New Microwave and Refridgerator. As we feel those are the things that aren't usually replaced do to budget. This place is unorganized. It's hard to get someone to speak to. We always get excuses that the "phones are not working". We leave multiple messages to get a call back. I had to go as far as calling every 15 mins, sending emails and even sending messages on their social media accounts. When you ask a nurse or therapist questions about anything unusual nobody ever knows how to get you an answer. My family member has over three weeks and they barely scheduled an appointment to go over treatment and care. It's very upsetting to see how unwell this facility treats the patients family, makes me think they are not taking care of our family members the way they should. read more

Terracina is not perfect but from my experience working in different facilities, each has it's pros and cons. I've seen great improvements in the last 5 years that I have been working here as a therapist. Thought about working for another facility 3 years ago but I saw a bright future for Terracina when they started renovating the building. I love our rehab team here and we take pride in what we do. It may look like it's all exercises, dressing and walking when it comes to rehab but our team has a mix of characters and personalities that help our residents engage in therapy in a pleasing and enjoyable way. A simple conversation goes a long way specially in geriatric population and our team greatly values that. The best part of my job is our ability to touch our residents' lives and knowing that we're making a difference. There's nothing more rewarding for me than having patients come back to visit us and tell how our team has helped improve their lives after rehab. That's what keeps me fired up and going. I was there as a visitor and what I saw on my several visits were just opposite from the negative reviews on Yelp! & Google. This place was clean, smell fresh and well maintained. I visited a few other nursing facilities similar to this place, so far,.. this place is a better place compared to others. I watched how the staffs provided the care for my friend and they were professional and gentle. If I become ill, I'd like to receive the quality of care my friend had. I give it a 0 star if I could, I do not recommend this place to anyone, my grandmother was admitted here after she left the hospital for rehab, I will spend everyday with her ,all day, at this place, first off , they gave her a wet bed with bugs on it, I asked them to clean it and they never did, everyday she was there she was not rehabbed, she would just stay there laying down on the bed, where she developed a soar in her back and got to stage 4, she wasn't on a ventilator anymore when she got there, the 2 months that she stayed there she was send to the hospital twice Becuz she developed pneumonia again and again Becuz they were not giving her treatment or cleaning her, after the second time I started to give her rehab myself and they got really mad, I'm guessing they just wanted to keep her longer to cash out the insurance checks, they will not let her eat, they wouldn't shower her, the nurses will leave her pooped all night, I would have to shower her everyday, the doctors and the nurses where so rude, specially the speech therapist, he didn't let my grandma eat Becuz we took a picture in front of him, and that's not all, the social worker is so unprofessional, would never respond or help out when I would tell her what was going on, she would make me wait outside her office only to never come out, wasting my time, overall such a horrible place to stay, I am glad they send my grandma to the hospital one last time Becuz she was able to get better and go home, without a ventilator and her stage 4 wound healed, again if you have the chance to fight the insurance to place your love ones somewhere else,, Do It!! Also the place dosnt look anything like the pictures they have online, the place is so unsanitary, I saw one person go #2 on the floor and all they did was wipe the floor with a towel and let it like that for days. read more

Jon B. My wife was admitted to Madison Grove Post Acute {formerly Terracina} for 2 and 1/2 years. She was attended to by a very caring doctor while there. He always responded to her requests in a timely manner and was compassionate in his care for her. The staff is commended for complying with his recommendations and providing the necessary care during her stay at the facility. Also, special gratitude is extended to Social Services who demonstrated untiring and optimum support, and unparallel compassion. Enough positivity cannot be articulated about the good deeds of this department. For instance, they went beyond the call of duty by attending to her social/spiritual needs, which is a rare quality seen in a post-acute facility environment. We wish them all well as they are an invaluable asset to Madison Grove Post Acute. By the way, if we had rated Social Services as a separate entity, we would have awarded 5 stars. I don't want to give even 1 star. I've visited my sister here since 2014. Being unable to see her during lockdown was hard, but my visit with her on May 5th was a shock. Flooded bathroom, no bedding or hospital bed or chairs anywhere, no water to drink or any family photos or personal possessions! I'm making serious complaints and advise anyone to get your loved ones out! My sister died 5 days later. Her DNR was not given to the ambulance crew. read more

These elderly people are being neglected living in filth. Unsafe you can just walk in there. We didn't place my grandma here my grandma was in the hospital and hospital sent her to this place without even notifying us we would never put my grandma in such a s*** hole. I truly hope the other pts. are being cared for. I don't want to say that the poor care my brother us receiving is because of a particular person but maybe because the lack of personnel or education. My brother is in a coma and can't ask for help if he has thrown up or needs to be changed, repositioned or suctioned. I am a nurse in Orange county and drive here in traffic after work to check up on him and clean him. I every time I have to ask for a chair do that I can sit next to him. My mom is in this place right now, I am very upset about the way they are treating her, she was transported from St. Mary's hospital in Apple Valley because the doctor didn't want her there anymore. She went in for a broken ankle, and was tested positive for Covid, for the second time, from what I am hearing everyday from my mom, who is 80, is they have given her the wrong food, she is on a no salt diet for her heart and the tray had another persons name on it, they can't come and help her to the bathroom and literally told her to "poop" in the bed if she had to go, they are giving her meds late, her remote had no batteries and it took 2 days for them to bring some batteries to her just so she could turn her tv on, she has been feeling sick ever since she got there 3 days ago, and now has a rash on her back and stomach from her bed sheets. This is a place that needs shut down, if these people can't deal with people in need, they shouldn't be there!, We are waiting for a hospital bed to be delivered to my house and my mom is out of there, DO NOT LET YOUR LOVED ONES GO TO THIS HORRIBLE PLACE!! read more

This is this based on my staying there 11 Months. The problem is Terracina is that their management is practically nonexistent. They understaff all of their people all of their nursing aides I'll say. This means that there are very few people to take care of the patients that are there, leaving all of them who aren't treated laying and dirty or wet diapers. They do not get the care that they should. I was not given Physical Therapy when I should have been there for I went home 11 months later not knowing how to walk and I had to work with Medicare and then work with myself and my caretaker. There are some good nursinggood assistant that are there that give you care oh, they're not in a hurry, they're nice to you and listen to you. But I would say the majority of them do not care they want to get in and get out or when you use your call Button nobody shows up for at least an hour or two, again laying there in dirty diapers. I would not take anyone to this place if it's your parents or another Elderly person, I would think twice before going there, I would visit and then still look hard and see what you see and then think and then don't do it but that's up to you. Their physical therapy department tries hard and they're very good. The food is not real awful is palatable but that's about all I can say about it. read more

I have been a patient here for a few weeks and the care and facility has been great. I really like all the nursing staff, even the janitors are super friendly. Most importantly they helped me recover from a gunshot wound. The PT department has a nice facility and they got be back on track with my strength and stretching. The food is actually good besides a couple of miss hits. The front desk handled all of my requests about guests and packages well.. no complaints. The social workers were really fast to help me get all my paperwork in order. I would recommend this facility to anyone who is leaving the hospital and needs further care. read more

Terracina Post Acute is the best facility in Redlands and very soon it will be the facility of "Fist Choice" not only City wide but state wide as well. The management together with all the department heads are working extra hard in making the facility into a 5 star rating. I always believed that every facility has the-same operating procedures for all are under strict and must adhere to all regulations set by DOH. The only matter that will differentiate one from the other are the following which this facility has; Management - making sure the team is high spirited all the time. Nursing - making sure ADL's for residents are on its highest standards. Non-Nursing - making sure the facility, residents and other matters are met again meeting the highest standards. I was first employed with this facility middle of 2018 and left middle of 2019. I came back March of this year and considered myself as Terracina's "Prodigal Son". Im very grateful to this facility for giving me the chance to work again and be a part of an awesome team. I was assigned and trained with various departments and currently working as the lead person for central supply. It was a tough assignment and comes with big responsibilities. The first few months were really a struggle but the management wanted me to succeed and they generously gave me all the help and assistance needed in performing my daily tasks. Again Im so thankful to the management for the opportunity and for the trust and confidence to be a part of an outstanding team. To my partner Ms Ashley thank you for all the hard work and in making our goals, mission and objectives done and complied on a daily basis. Its teamwork that makes it possible. Unlike other departments, central supply' missions needs to be achieved on a daily basis. Every person is very important in this facility from top management to the newest rank and file employee.
