    Clean facility. The staff are very friendly and attentive. Management is very open to feedback and correcting any issues quickly.

However, thanks to the very focused health care professionals who work here, I am up and about and semi-ambulatory. The standard of care at this facility is very well balanced. Nursing care, weekly visits by the staff doctor, nutritional planning and diet, rehab classes, housekeeping services and daily social activities all contribute to successful recuperation and favorable outcomes. I am so impressed with my progress that I have elected to complete my full 9 to 12 physical therapy program as a full time resident here. I am confident that the continuing care I receive will lead to my full recovery and prepare me to resume a fully independent life and lifestyle. Mr. Terry Lamantia, Facility Administrator, is to be commended for assembling a very fine group of professional and service staff whose well directed and focused efforts work together seamlessly to deliver a health care environment well suited to attaining successful outcomes. The fact that all of the staff genuinely respect and enjoy each other's company is a bonus which does not go unnoticed by the residents.     My wife needed rehab inMay of this year She was labeled…read moremild dimentia so when she being disatisfied with their demands and bad food choices being diabetic they labeled her, it seems as unruly.She had a fractured tailbone and pneumonia so had little in tough rehab sessions .They gave her bad ratings to the ins company They told me if I paid up her deductible she could get more time,up to a hundred days of care and rehabilitation.I paid in cash over two thousand dollars so my could have longer care.Right after they took and caught her up with the deductible they got her to sign herself out. I have the signed documents to prove it.They were well aware her signing these papers was wrong.She left for home hospice with a urinary infection and extreme pain and pneumonia and died two weeks later.I believe they cared nothing for my wife's conditions. I lost my best friend and soul mate just that easy. The staff is second rate they couldn't even handle a catheter change,sent her to the hospital for the procedure. I believe its a resort for the staff not the patients .If you have anyone needing special care this may not be the place.l give this place one star .My opinion is based on how they treated and lied to our family. The team controlling this place should be reprimanded for false statements and bad service to aging patients. BEWARE where you waste your money or trust based on my horrible outcome....,Heads up!

    Where do I begin. First, my mil was deposited here after a short stay in another rehab. In the main…read morelevel the rooms are 3 to a room. The room size was built to only accommodate 2 people, they squeezed in a third bed in between. It was dirty. The floors looked like they hadn't been washed in months or longer. There was a lingering fragrance of urine and feces. Everything was very old and unkept. To the staff. There were a few people that were nursing care that appeared to appreciate their jobs. A couple of CNA's did also but then there were the majority that were just there because they had to be. They needed a job, any job with a paycheck. The care they delivered was subpar at best. My mil has functioning dementia. She goes in and out of lucidity. We were told by neurology and psychology at the hospital that she could no longer make decisions for herself. In a nutshell, she was unable to care for herself including personal care, meals, and daily financial and other living needs. She had been living in very poor conditions and had been taken advantage of financially by family members, neighbors and home care staff including doctors and nursing. She had survived a fall in which she broke her neck. Social workers at the hospital were great they placed her at a very nice rehab facility. There was a problem with her MPOA and they could not get rid of her fast enough. Supposedly the only place in town that had an open bed was Sundance Skilled Nursing and Rehab. No one in her immediate family was able to care for her advancing needs. She is a smoker so options were limited. She was placed in the middle bed between two other woman. First thing we became aware of was her personal belongings went missing. She was given a tall dresser 5' plus with the tv placed on top of it. She had a neck brace that was ordered to stay on. It was taken off by staff and not put back on. She began hoarding food in the dresser and when the staff was made aware they claimed they were not allowed to go through her personal space. We cleaned out the dresser several times and request it be taken out of the room but it never happened. We removed her cherished belongings before they became someone else's. She was left with basic clothing. The next crisis came when another patient thought my mil had stolen her cell phone and physically assaulted her. The staff realized what had happened and removed the other patient and put my mil and the other patient under 24/7 guard. We were horrified! We began our own search for another facility. We soon found another placement and began the paperwork to move her which involved the office staff of Sundance. Once it was leaked out that my mil was moving the staff would say stuff to her like, "we love having you here, grandma" and "you know if you don't want to move you don't have to." By then we had secured a MPOA and FPOA and were very much in charge. She was moving. The staff continued to undermine our attempts by stalling paperwork, making her afraid to move to a new place using fear tactics. Even telling her they were going to miss her so much as she was part of their family. We threatened to turn them in and they did back off once litigation both individual and facility wide was threatened. We were taking names and threatening licenses. Also at this point my mil began to smell. We asked when she had been bathed last. The staff said she had not had a bath at all. She had been refusing a bath all along. So no bathing was done. The staff claimed that they can't make her bathe. Her linens were not changed in the month she was there ei It was basically a frat house for the disabled and elderly. The day before we were moving her we went to pack her things. It was Halloween. As with many nursing homes, children were visiting to trick or treat. No one noticed that when the children left on their way down west Colorado Avenue my mil left with them. We arrived and she was gone. No one knew when she left nor how long she had been gone. We panicked and drove around looking for her. Six blocks away we found her. She was scared and disoriented. Not anyone took responsibility from Sundance. We took her and left. If I could have given a negative star rating I would have.

They are always engaged with the residents, they perform tasks with a smile on their face and are attending Resident's needs promptly. The residents all look content and well taken care of. The leadership team is dedicated to making this a great place. They have implemented several things since I started visiting the facility about a year ago. What sticks out is the sensory room, it is a quiet place for residents to relax.

I am not sure where to begin and I need to apologize I didn't post sooner. We moved my mom into The Retreat a little over a year ago. When we first moved my mother over there they did very well with her, very dedicated staff and always felt warm and welcoming. However within months they were continually short-staffed and without a wellness director or MCC. There are so many issues that began to happen. The CNAs and caregivers were always overworked because there was not enough staff. The Retreat executive director Leslie Brown would blatantly lie about how many staff were working and their experience. She would state that they would staff according to Acuity and not numbers so there would be two staff on with more than 27 residents. This is in the memory support area. Often the really good caregivers would leave because they would get so burned out. The overnight staff would not check on my mom and change her as needed for up to 6 to 9 hours at a time. My mother had a doctor's order that she be checked and changed every 2 hours around the clock . This was extremely concerning and an issue that I seem to address on a weekly if not daily basis with the executive director. Their day staff were amazing they had a team of four that really cared about and for the residents. In their own frustration they documented how urine soaked several residents were upon their arrival one morning. All were immediately terminated. We had a video recorder in my mom's room and if you don't do that for your loved one you're crazy and it is totally legal! On one occasion the day staff were having a difficult time waking my mom. A very young and new caregiver made the statement that the new Wellness director had double dosed her with her sleeping medication. When I brought this to the executive directors attention I was informed they would investigate but in the meantime I needed to post a sign that stated the room had video surveillance and sign a agreement form regarding the cameras. Even though we had had the cameras for 7 months. Within days I was notified that the caregiver was talking about somebody else and the wellness director was no longer with them. A staff was investigated for possible sexual assault on female residents. We were notified by the investigator that my mom was possibly a victim, at no point where we notified by Sunny Vista or the retreat April 2024 my mom fell and broke her hip. The night my mom fell about 8:30 p.m. there were three staff working the entire building upstairs and downstairs. I do not know the amount of residents they had at that time but if I was to guess I would say probably more than 30. Not knowing quite what to do after she was released from the hospital a week later we moved her over to the Sunny Vista rehab. (See that review) While my mom was at rehab I would go over across the street to the retreat to check on the Staffing. The executive director asked to speak with me and said she wanted to be very transparent and told me they were very very short staffed. On another occasion my niece went over there while looking to visit my mom and not knowing which building she was in and found no staff for over 20 minutes of walking around in the retreat memory care support area. Although there were several residents she could not find a caregiver. At that point we knew it was probably just dangerous to move my mom over there and chose to move her to a different facility. I would like to add that in one point of this horrific Journey I walked across the street to sunnyvista to speak to Steve Downs this was shortly after they had fired the young ladies that tried to document the neglect. After meeting with him he assured me that things would be getting better and provided his cell phone number to me. And then pretty much ghosted me. While my mom was in rehab Medicare and Tricare paid for her rehab. Which I'm sure Sunny Vista charged $392 a day if not more because her insurance was so good. They also charged us the full price for an empty room and no waiting list for residents to move in. In fact several residents were being moved out by their family members for various issues like hygiene and medication issues. I myself and my brother both tried to speak to Steve Downs and Leslie Brown the executive director at the retreat about at least a discounting the costs especially where we had to hire private caregivers to ensure that my mom was being taken care of while in rehab. Both of us received the runaround, the ghosting and in the end they denied to give us a discount. It's sad because they do have a few very wonderful staff that work really hard and don't get any support and minimal compensation.

I wasn't asked if the wage was something I was seeking as I get paid $2 more with the experience and knowledge. I was informed of when the company had opened. And I was offered a position right on the spot, I have another job as I mentioned only M,T,W were my open availability. Interview took place on Tuesday and I was given a drug screen slip with only (24 hours to complete) And the lady that had called her name was "Luanda Vick's" or Wanda Vick's. Left me a voicemail claiming she didn't receive my orientation completion. After speaking with her over the phone with her rudeness she clearly said "sorry if this may sound rude, but regardless if you have it done and it shows on your end it's showing that it's not complete, so imma need you to come in and complete that" called me on a Thursday talking about can you come in tomorrow. No. I can't just stop, drop and roll, because y'all call me and expect me to finish everything within 24 hours. Saying Mandatory meetings every FRIdAY, when clearly that's impossible for me. The company seems to only care what you can do for them and not how much of a value that you can truly be to the company. (Rude, not compassionate) My opinion this is one of those facilities that looks so good on the outside, but you'll have to find out what truly goes on in the inside. So, with that being said My personal experience was not a great one!
