After touring the facility, being told both my parents would be accepted and together , which was most important to my family & expressed deeply to the administration. I was encouraged to initiate the transfer process for my mother who is in another facility , after obtaining OCCO/PAS approval, and transferring one spouse into the facility all was approved , we were later informed that the second spouse was being denied due to "acuity level" and "payor "status concerns. What made this especially difficult was the lack of any clear clinical explanation despite repeated questions, while nearly every conversation focused heavily on finances, Medicaid pending status, payees, and financial responsibility paperwork & give us until Monday after a stressful week to discuss like it didn't matter . Families navigating long-term care are already overwhelmed emotionally, financially, and physically. Communication matters. Transparency matters. If there are concerns about Medicaid pending admissions, families deserve honest and direct communication before major decisions are made , especially when dealing with elderly married couples who may now face separation after relying on representations made during the admissions process. I strongly encourage families to: * read every admissions and financial form carefully, * understand the difference between POA and personal financial responsibility, * keep all communications documented * and ask detailed questions before making transfer decisions Be aware that federal law (42 CFR § 483.15) prohibits facilities from requiring a third-party guarantee of payment. As for the food , my father hates it and says it's disgusting . He said he has lost a lot of weight , he says his stomach is shrinking & he is not liking , or eating the food , he is hungry all the time however he said he can't stomach to eat the food . . I have ordered outside food to the facility upon my Dads request. My dad is Jewish , he's not at all kosher , however he liked the Kosher meals on wheels at home . Outside of PT and OT sessions, my father appears to spend nearly all day in his room/in bed with little to no meaningful activity or engagement , which to me is deeply concerning . Communication and responsiveness to the facility have also been extremely frustrating during our experience. This process has been heartbreaking and unnecessarily stressful for our family. Mother's Day weekend passed & my parents are now an hour from one another & separated due to what happened here . Disgraceful Not to mention I feel the admissions department is useless & they do not respond with any concerns whatsoever!! UPDATE JULY 9, 2026: In my opinion : The negligence at this facility continues to worsen. The facility recently lost my father's custom dentures for two full days--a clear sign that staff is not performing the daily oral care required by the facility . Instead of taking responsibility, the administration actually tried to blame my 90 year old father with vascular dementia to try to avoid paying for them, claiming "he said he put them in his pocket." This facility loses personal property, refuses to take accountability, and tries to financially exploit vulnerable families to cover up their staff's failure who was & supposed to be cleaning the dentures daily at night & had no idea they were missing for two whole days . A formal complaint has been filed with the New Jersey Department of Health ++!! Update - I feel the healthcare that my father is getting is unacceptable- it's now over a month that the facility lost my dads dentures - to top it off the owner's son is only interested in sending alleged bills to my home .He told me only families that have signed agreements that are Medicaid pending (my dad is pending ) not receive private pay bills , If I do receive 1 more I will post all of them . Communication is…read morepoor, my father, 99 with a broken back, was put in a room that was dark, no water, no pitcher, tv with one channel. Held captive by POA, I was powerless to move him. The aids came in one night to weigh him. Why? A man with a broken back to be hung in a net contraption? I watched him screem as they attempted to lift him and stepped in to stop it immediately. I unhooked the device and told them to get out. I had to find a nurse (no where to be found) to make sure they NEVER abused him like that again. Why in anyone's name would you put a man with a broken back inside a net? Zero compassion. No water ever in the room. I HAD TO FIND A CUP AND PITCHER for my dad. I was there often as well. He was ALWAYS thirsty when I came which tells me they were not attentive at all. He wasted away until he died with zero morphine on board because why? Oh they were "out". Grace hospice that partners with them, had to be called three times on 2 separate days to get care for my dad. Oxygen machine kept going off with a loud beep alarm. No one came and each time I had to reset it. It was extremely disruptive. It was horrible and I watched him die, his hand in mine, 8 hours without morphine and three calls out to Grace Hospice. The care is horrible, unattentive and abusive. DO NOT SEND your loved one here if they cannot advocate for themselves. This was the MOST HORRIBLE experience and I can never get it out of my head. My father is at peace but it was a horrible, suffering death. Type of Care: No compassion - all transactional Communication with family: Terrible Room: Dirty and dark Staff: Unengaged DO NOT SEND YOUR LOVED ONE HERE IF THEY CANNOT ADVOCATE for themselves. The care team meets with both resident and family member for the subacute rehab facility to discuss the care plan of action and goals. The subacute suites have ample space and privacy curtains and doors for patients to have rest and a quiet stay. Each room in the subacute facility is private, so no patient has to share the space. The beds are twin long adjustable beds. There is a large bathroom with plenty of safety bars and space for a short stay. We had planned on 20 days, but it ended up being only a 4 days stay. The staff is primarily Filipino and kind. Some shared their journey that brought them to nursing care and a rehab facility. While there are several Parker locations, Somerset is the only subacute facility. It's roughly a 15 minute drive from Robert Wood Johnson hospital. Many people are concerned about the quality of the food at a subacute center. Parker gave menus with at least two options per meal and a static list of always available items. You can have the opportunity to meet with a nutritionist to discuss personal goals and food challenges. They accommodate every meal to meet patient needs or requests. You do need to request salt, pepper, and butter, but it's better to be able to add than have everything over seasoned. There is a round the clock refreshment station available with juices, water, coffee, and tea. The upgraded facility is really beautiful and set back into the landscape, providing privacy for residents. There are plenty of areas to gather outdoors with the help of staff to exit or enter the building. There is a cafe designed for families visiting with patients. It's cash only and you can purchase a salad, soup, or sandwich. They also offer some of the hot meals available to residents so you acne sample what's available. The check-in process requires signing yourself in and out of the facility. It's very convenient and the by your second visit, the person at the front desk will recognize you. I highly recommend Parker and want to thank nurse Christen for welcoming me and showing me kindness while adjusting to the facility.

I was discharged on Friday the 24th and told I would hear from support in 48 hours. As of today the 29th I have not heard from anyone! I have left multiple messages to multiple people and still haven't received any answers. I live alone and was told I would receive PT at home as we'll has home health care for help with everyday chores. Seven days have past and now another weekend will be upon us, will I receive help next week? I would not recommend Parker for Seniors who live alone.

As of the last three months, he said he is satisfied with the amount of activity and friendly residents at this establishment. I was blown away at how beautifully decorated and brightly colored this place was. I've visited various rehabs, nursing homes, and assisted living places, and many, unfortunately resemble hospitals or are depressing looking. This one was not that, and instead looked like a fun hotel or resort. Looking forward to visiting my friends here again, and hearing more about their perspective.
