My father entered this facility with documented "good rehab potential." Until only weeks earlier, he had been ambulatory and self-sufficient. Yet almost immediately, Dr. Cha repeatedly dismissed his worsening condition as simply "old age." What we witnessed felt less like compassionate rehabilitation medicine and more like an elderly patient quietly being written off. One physician later intervened with IV hydration and steroids after recognizing a serious decline in my father's condition, and we saw noticeable improvement. Yet shortly afterward, according to staff, the IV was removed despite my father struggling to swallow even water. Communication with the family was deeply inadequate despite staff knowing my father spoke no English and communicated only in the Calabrian dialect of southern Italy. He was therefore heavily dependent upon proper medical oversight, attentive observation, and honest communication with family members advocating on his behalf. The medical records later obtained by our family contained troubling inconsistencies, contradictory notes, omissions, incorrect dates, and even the wrong gender in one report. Most heartbreaking of all was the feeling that my father was no longer being treated as someone worth fighting for. Despite ongoing weakness and COVID symptoms, he was discharged to a skilled nursing facility instead of receiving higher-level hospital care. Two days later, he was dead. After reading numerous prior reviews of San Joaquin Valley Rehabilitation Hospital, I was deeply disturbed by how many experiences closely resembled our own concerns regarding patient care and Dr. Cha's medical oversight. Multiple reviewers described issues involving medication management, poor communication, dismissive behavior toward family advocates, questionable treatment decisions, and inadequate patient care -- concerns that strongly parallel what is documented in our records. My concerns are not speculative. I possess extensive medical documentation, medication records, physician notes, timelines, and related evidence that I believe demonstrate serious negligence involving Dr. Cha and, by extension, San Joaquin Valley Rehabilitation Hospital. What is especially troubling is the consistency between our documented experience and the accounts already publicly described by multiple other families here. Because these matters are supported by substantial documentation, I am willing to privately share relevant records with legitimate investigators, attorneys, licensing authorities, journalists, or families seeking information regarding this facility. If others experienced similar issues and wish to compare records or discuss concerns privately, they may contact me. Patients and families deserve transparency, accountability, and safe medical care. I would never entrust a loved one to this facility.

He was supposed to recover there and return home, but because they failed to properly monitor his catheter, he developed a severe infection. He was transferred back to the community hospital, and the infection entered his bloodstream. Ultimatley, he passed away. At one point, we spoke with the director in charge late in the day. While we voiced my concerns, she didn't even make eye contact--she packed her bag, shut down her computer, and got ready to leave. It was clear she had no real interest in addressing the issues, and that moment reflected exactly why the facility runs the way it does--with indifference and a lack of accountability. They never followed directions, and any time I asked questions, no one knew the answers. They blamed the previous shift instead of taking responsibility. No family should have to suffer a loss because of such neglect. I urge others to stay far away from this facility. Their inaction cost my grandfather his life.

They are the only facility in the Fresno area that would accept someone with his diagnosis. When I removed him from his previous facility that was rated 4 stars by Clovis Community, he was so weak that he could not sit up or stand because he had lost nearly 30 pounds because of their poor care. The receptionist at Sierra Vista was very welcoming. The staff was friendly and I could ask any staff member about his condition. At his previous facility I was only allowed to speak to one person and she never gave me answers to my questions regarding his weight, BP, etc. Sierra Vista's dedicated Physical Therapy staff was able to get him up on his feet and walking again. I am extremely thankful for them doing that! When he was discharged to go home many of the staff hugged him goodbye or gave him a kiss on the cheek. They will be happy to know that he is still up and walking. Thank you Sierra Vista!

I was there three months b/c of a car accident. If I had to put some one in this home I believe I would feel good about the general care. I of course would have to bring meals for my loved ones because honestly their food was not edible. HOWEVER **** if you are there with a wound this is not the place to be. My wound nurse decided to oil up my wound then proceeded to tightly bandage my wounded leg. This was not the surgeons orders. Wound nurses are suppose to be a specialized skill. Well this one wasn't. I went home thinking I was finally on my way to recovery. Three days later my wound had opened and I could see the hardware in my leg. Hugh hole. Returned to E.R. Turns out the infection had turned to MRSA and I had to be readmitted to the hospital to open the wound, clean it out, remove the rod in my leg. So now I'm back to square one. After the hospital I went to Willow Creek. Not my choice but because I now had MRSA I needed to be in isolation. DON'T GO HERE. Everyone is lazy. You rarely see your CNA, your nurse is NEVER on time with meds, and sometimes my call button would be on for 1 1/2 hours. Not the kind of place I'd send my elderly. This place is ABSOLUTELY THE WORST! This is a long account and I'm skipping the next 4 weeks. The long and the short of it is: I loved the people at Sierra Vista. Watch out for the wound nurse, make sure you can deliver meals to your loved ones. And remember to stay away from Willow Creek. I would have given this a 5 because I loved the people. I subtracted for the terrible food and the unskilled wound nurse.

They said oh but it didn't touch the ground when it fell. continued pushing w/c out and left me with no call light. finally after 3 hours a nurse came in and I asked to be put in bed. well after lunch physical therapy ( female with glasses and long black hair came in and asked me if I'm ready to get up for therapy. I explained what happen. Guess what , she said that my leg didn't touch the ground. and then told me ( so your refusing therapy) I said no I'm not , I can't do it because of the pain caused by the negligence of your coworkers that were here today. well she put down that I refused and next day I'm told I'm down for discharge. Not one nurse, Doctor, case manager, OT, PT, ever looked into this situation. I told all of them and it was totally ignored. here I am still suffering , I had bars and screws in my leg when they dropped it (yeah maybe it didn't touch the ground but it was not suppose to bend. Come on San Juaquin Vally Rehabilitation Hospital. Dont your employees know what can happen if they are in a rush. I just had surgery in trauma center and had bars and pins in my leg. Everybody knew and nobody cared. it was ignored . I cried every night in pain. i was discharged because they said I refused therapy the same day of my injury. It doesn't take a genius. I WAS IN PAIN get it together, I will never refer somebody there. it is not safe. it shows that they don't care what happens to you

After my…read moreconcerns were raised, my mother's missing item was returned, and the staff made a genuine effort to ensure she felt comfortable and supported. That meant a great deal to her and to our family. My mother is in rehab and relearning how to walk, and she now feels safe, respected, and cared for. The rooms are clean, the food is genuinely good, and most importantly, the staff have been kind and attentive, which has had a very positive impact on her mood and recovery. I appreciate the steps management and staff took to address the issue and restore trust. Based on how things stand now, my mother is happy with her care, and that is what matters most.

I have no way to check on my friend!! She has no way to call anyone to calm her down when she is upset. I was told to go buy her cell phone. 1. She doesn't know how to use a cell. 2. She cant see a cell phone! 3. Cell phones cost a lot of money she lives on SSI only can't afford a cell phone thats why she doesnt have one!!My dad is same way he drives at 80 but can not figure out a cell phone to save his life. This is awful you just pull family away!! Shame on you Willow Creek its hard enough to be there but now your really leaving patient alone.
