Less than 2 months ago I went to this office, at the recommendation of my partner, because my temporary front caps fell-out along with two fillings right in the middle of relocating from Florida to upstate New York. Things moved so fast. Our remodeling project wasn't far, so, no brainer. I explained my situation to the receptionist -- I had no records, all cash -- and was given the attached form and a blue sticky note with an email address.
