I visited this facility on 9/17/25. I was told these AFC clinics are franchises and the person who…read moreruns this Peoria facility, also runs the Surprise facility. I was also told that, even though they're all somewhat similar valley wide, each one does have the ability to tweek the way they run things. Apparently the Peoria & Surprise boss lady is adamant about new patients agreeing to, and signing a contract that obligates the patient to a care package that essentially would cost that patient thousands of dollars out of pocket. I explained my insurance coverage/limitations and that I was in no position to go beyond those limits. Even though these facilities are on the list of approved care providers through my medical insurance, this particular AFC facility denied me treatment because I wouldn't sign their contract and I was told that this also applied to the Surprise facility. This was very disappointing. I will give them kudos though, because the 3 people I interacted with were very courteous. I was glad, at least, that they were honest with me about how things worked at these 2 locations. The only reason I'm giving them any stars at all is because the staff was kind, patient and understanding, even though I was denied treatment.

Even though it was covered under my insurance, they persuaded me to sign a contract and pay them every month. After the first couple of adjustments I decided I DEFINITELY Did Not want to be stuck in a contract with a horrible chiropractor!! I told them that I wanted out of the contract and they replied that I could not. I was billed almost $2,000!! I called and spoke to Jamie. She said, " You lied about your insurance AND if you don't pay it then it's going to collections." She offered 1/2 off if I PIF today. I told her I don't have that kind of money! She told me that they accept credit cards. I told her that I'm not going to pay for it until I find out if it's covered under my insurance. She said if I don't pay 1/2 today then I'd be responsible for the entire bill or it's going to collections. How can she tell me that I " lied" about my insurance?!! First of all, I was shocked that someone in the medical field (or any job dealing with people) could speak to someone so rudely while trying to get $$!! I said, "1st of all I Don't Lie." I asked her how was it approved in the beginning and suddenly not now. She said the insurance was covering it and I discontinued my insurance! My insurance has stayed the same for years. Also, how is that possible if I am still using it at my new chiropractor Arizona Pain. I gave them my insurance card so how could I possibly lie about my insurance?! I called my insurance company and they said that AFC NEVER billed my insurance!! Yet they are billing me and making threats that will lower credit score by sending it to collections!!

I liked working with my physcial therapist. She seemed knowledgable; however, though the exercises were good, they didn't really help with my back pain. When my sessions ended, my PT asked me to go back and change answers on my questionnaires to indicate that the PT helped. As a medical person myself, I felt uncomfortable going back and changing answers. The other was that though most of the assistants were good, some of them seemed to spend too much time fooling around, and I would have to call to them to find out what the next exercise was.

Almost three weeks ago, our fear became a reality when Mom fell entering the bathroom, suffering a head injury. For our family, it was traumatic and frightening, especially when we watched her being taken away in an ambulance. We all know that as we age, this outcome becomes more likely, but we are never truly ready for it when it happens. When it was time to decide on rehabilitation, we wanted the closest location to our home. Thankfully, our option was Pam Health in Surprise, AZ. From the moment we entered the door, we were welcomed by staff and a beautifully kept building that is comfortable for both patients and their visitors. We were so grateful to everyone working at Pam Health. Our family had the opportunity to interact with all levels of staff from reception, housekeeping, dietary, medical staff, and physical therapists, including members of the leadership team. We were fortunate to witness Mom's physical therapy and see the inpatient and outpatient therapy rooms where she was able to build back her strength, allowing her to walk with confidence again. There are many staff members I wish I could mention by name, but to prevent this review from being longer than necessary, I wish to recognize one person in particular, Stacey. Stacey was Mom's main physical therapist, and she is amazing! She always made enough time for my Mom, providing excellent care and making herself available to answer any questions our family had. Stacey helped select specific equipment Mom needed after she was released from inpatient care, and made what could have been a stressful situation seamless and stress-free. Her kindness, compassion, and professionalism are everything we could have ever hoped for. The wonderful thing is that the professionalism and compassion expressed by Stacey were expressed by everyone employed by Pam Health. I double checked with my insurance the day before my appt and was told they were not in network. Cancelled my appt but they called to insist they were in network and they had checked. I called my insurance company again and again was told they were not in network. This practice would have charged me hundreds of dollars a visit instead of my $5 copay if I had listened to them. Makes me question their integrity and competence. Hard not to think this wasn't intentional when Im the one stuck with the bill regardless.
