I had extensive surgery to repair "massive tears" in two rotator cuff tendons (supraspinatus &…read moresubscapularis) along with a torn bicep. Many anchors, sutures and cutting...it was extremely painful. After surgery my recovery efforts led me to Reform Physical Therapy in Freeport where I met Nancy and Delany. The office is less than a block away from route 295 and very easy to access from north or south. They have plenty of free parking and the office atmosphere is pleasant and comfortable, and they have an elevator if needed. It is a tidy and professional space where I observed them routinely sanitize the equipment, change the linens, and wash down the surfaces for every visit. This is a very clean place of business. While fighting intense pain and trying to get my medications properly regulated, Care Coordinator Nancy virtually performed back flips to ensure that my PT treatments were covered by insurance and she made herculean efforts to get me the therapy I needed at a schedule that worked for me. She is friendly, accommodating and just an all around nice person. Nancy made, what could have been a agonizing and difficult situation, into a pleasant and welcoming experience. She establishes a great relationship with all of the patients and this makes every visit friendly and stress free. For my therapy treatments I was teamed up with Delany as my Physical Therapist. I was in a lot of pain and discomfort but she quickly recognized my situation and she applied a tempered and logical approach to ensure my recovery. Delany instantly impressed me with her vast knowledge, skills and ability to cautiously but determinedly challenge me to increase my abilities. She is thoughtful and very aware of each clients pain thresholds and she applies just the right amount of pressure to steadily improve each patients recovery efforts. As a testament to her Doctorate degree, Delany really knows what she is doing and I am amazed how quickly I was improving with each visit. Each patient recovery requires a specific therapy guidance that is sculptured for each individual and Delany has the cognition to recognize each patients tolerances. She applies the precise treatment options that allows each patient to achieve the maximum recovery in the shortest amount of time. This was certainly how Delany approached my situation and I am recovering faster and smoother with each visit, and I am on the road to achieving what hopefully will be a full recovery. I have discussed my injuries with others who had similar but less involved surgery and I am improving at a faster rate that everyone else I have spoken with, and I give full credit to Delany and the team at Reform. I can't thank Reform Physical Therapy enough for all they are doing to ensure that I achieve the best possible outcome.

Delaney is an exceptional therapist always adapting to my ever changing needs. Not only is she able to make sense of my poor communication, she seems to have a limitless knowledge of exercises and techniques to address these issues. Furthermore, Nancy, who handles scheduling, payments, etc., is incredibly organized and responsive. She operates as my executive functioning making it possible for me to consistently attend appointments. Finally, this is all done in a very friendly and positive environment and I look forward to seeing them at each appointment! I don't know what I'd do without them!

Good doctors, nice and helpful staff…read more The worst app I have seen in years. It works terribly and it's impossible to contact providers. I'm assuming it's designed by the CEO's son. If it wasn't, someone needs to be fired because the entire app is embarrassingly awful and just doesn't work for consumers. It's so bad that it's clear the staff doesn't use it either because they take far too long to respond. She is an exceptional physician and none of what follows reflects on her. The issue is entirely structural. Over the past year, Intermed has implemented scheduling requirements that make it nearly impossible to access routine care. Getting an appointment requires multiple calls, repeating your full medical history to different staff members each time just to be "approved," and even then telehealth is rarely an option regardless of how recently you were seen. Callbacks are inconsistent at best. As someone managing chronic health conditions, this is not workable. I understand physicians are given tight 15-minute windows and that this is out of physicians' control. But simple visits like medication management or a brief touchbase to relay specialist updates should not require weeks of effort just to get on the schedule. The friction rivals that of a specialist office, which is not what you expect from a primary care practice. Intermed is likely a fine fit for patients who need one annual physical and nothing more. For anyone requiring more frequent or complex primary care, I'd encourage you to weigh that carefully. It's a shame, because the quality of the physicians themselves doesn't match the experience the system creates around them. I will be transferring my care after months of trying to navigate their new system.
