This facility is now under Pinnacle Management. I agree completely with most of these previous reviews. They play games. From the nasty mouth social worker that try to belittle ($$$) you to the nurse practitioners trying to be MD, diagnosing medical conditions so they can drug you up and finish you off.. Even some of the dieticians make you feel low. Some people have issues with their weight due no fault of their own. Genetically challenging. Endocrine issues. They have could had emotional and traumatic experiences in the past going all way back to childhood! They deal horribly with memory loss, folks who are confined to wheel chairs, bedridden, cannot conduct their daily activities of living independently. They allow things to get out hand. Preventive measures. Proper notification to family members at the start of a wound. Neglect leads to the stage 4. The skip over too many details. They spoof their caller ID when they contact you. No wants to give there name!! Some of their staff english is not clear. When you have conversations with them. Heavy accents. You will FiNALLY see a MD appear and Pinnacle on your Phone announcing departure time!! This world has been through a rough climate in the past few years. I would have never selected this place if a knew any better. At that moment lockdowns where in effect.. There were not many viable options. For my spouse CHANGE came much too late. My Stepfather has been here for a few years now and from day one he has received great care. The staff on his floor is wonderful and caring. My stepfather is suffering from Alzheimer's so it's very comforting for us to know that he's been taken care of by people this care. The staff is good about keeping us informed about his care which will greatly appreciate. anytime we have questions or concerns the nursing staff or social worker have been really good about addressing them. When we first visited this nursing home we liked how clean it was but had some concerns because we have heard some bad reports. So for our experience has been all good and it's walking distance for us so we visit almost every day. read more

I was a patient a couple of years ago worst place I have ever stayed at. I have spinal stenosis an had stopped walking fir 3 months left hospital and went there there for rehab the first night got pain meds from the nurse 4 hours late. Was crying an screaming no one showed up but the aide she said the nurse was in the hallway laughing and talking. Next day was suppose to see the dr they sent me to therapy without my meds had to bring me back up couldn't do therapy. I asked what time is the dr coming they saud he was here but, left the patient in my room saud the nurse told the dr ivwas in the smoking room. I don't even smoke that night they said since the dr couldn't see me I could not have my meds until the next morning I called my daughter who lives on 107 st in Manhattan at 9 that night she came up and made them give me my meds. Nobody wanted to take me to the bathroom. I had to go on myself. The third day I had a UTI the nurse saud she would relive me the next afternoon I was in such pain. At 12am I started gathering and packing my stuff about 2am I call the police they sent ambulance and came with them I told them I wanted to go to the hospital I needed to be relieved I am in the nurse that was on duty said he would have relieved me if he knew I said its in my record. He told the ambulance staff to go I started screaming and said please take me he told them I was crazy don't listen to me. I said please take me. Thank God they did. Got to Einstein hospital they relieved me I had two bags if urine backed up in me. Wow what a relief. Never went back to me to a rehab in New Rochelle stayed there and left in two months walking with a cane. read more

I gave one star simply because you have to add a star in order to write a review. This is the absolute worst nursing home ever. Do not bring your family members or loved ones to this facility. There is construction going on around the patients there are leaking ceilings and the air conditioning and heat do not work. My mother has been there for approximately six weeks for rehab due to a broken hip. Since arriving she has had Pneumonia and an unexplained wound to her foot she's fallen off the bed (on the day she arrived I advised Bay Park that my mother is at risk for falling) and her phone has been stolen. The director John McKay is of no help whatsoever instead everything is an investigation, you fill paperwork out and nobody gets back to you with a conclusion to any investigation. John Mckay does not return phone calls, does not follow up with family members and for the life of me I do not understand how he was hired as a director. Last week I came to the nursing home to check on my mother and she was complaining that she was not going to stay there any longer she refused to get out of her wheelchair and get back in the bed and she indicated that she was assaulted. After my own investigation I found an aid that works there that let me know my mother did not want to go to rehab that day and she was taken to rehab against her will on that particular day by more than one employee and during that time her foot was injured. However nobody is getting back to me regarding that investigation and they're trying to say that my mother's foot was injured during her hip surgery. Let me make it very clear my mother had no injuries to her body with the exception of stitches where she had her hip surgery when she arrived at that horrible place bay park. I have filed an outside complaint through the state and I can only pray that this nursing home will be shut down completely. read more

The reviews (on and outside of Yelp) are all scarily TRUE- this is where you send your recovering/elderly loved ones to die. If there is anything you take away from this, it's GET THEM OUT OF HERE!! Talk to the social worker, talk to the administrator, PRESS THEM AND GET YOUR FAMILY OUT! Unfortunately, I learned it the hard way when I found out that Bay Park Center for Nursing and Rehabilitation killed my mother. My mother was forced to come here back in May 2016 after having extremely limited nursing home options that could accommodate her level of needed care - she had a foot amputation, tracheostomy and feeding tube (PEG) -and was extremely high-need and dependent on staff. Imagine, a person who is essentially bed-ridden and can't speak for themselves having to endure nursing staff that would sometimes leave her sitting in her feces for hours, tell her she was lying when she tried advocating that she couldn't breathe properly, and took HOURS to come see her when she rang her bell for help. Unfortunately, she really needed them one night when her trach became dislodged and, based on brain scans, our family learned she had been without oxygen for 1 hour! We also learned that staff hadn't checked in on her for 2.5 hours! When we pressed the nursing facility for answers, Nurse BROWN, the 3rd floor nursing administrator responded, " I can't answer because I have a license to protect." No apology, no answers for what went down that evening, and she refused to tell us who was on duty supposedly taking care of Mom. When I asked to speak to the building's Head Nursing Administrator, i was given some random number in which no voicemail was set-up guaranteeing that no one could get back to me. What a terribly understaffed, unprofessional nursing facility that clearly did not follow protocol and was not there for my mother when she needed them most. And absolutely nothing has changed or happened in response to it all. It's all so devastatingly sad that this institution is allowed to operate and can continue to abuse and kill more people. If you can help it, don't let your loved ones stay here and be sure to file your complaints with the department of Health. read more

I am infuriated places like this should be shut down. Not only for there health code violations but for the lack of respect they have for the sick and the families. I understand that this place is smack in the ghetto doesn't mean your quality of care should be horrendous. I am from the bronx my mom went blind to diabetes I could not tend to her needs. These CNAs were disrespectful to my mom, would leave her without food at times because they wanted to spite her. Why? Because my mom speaks up for her rights and her quality of care that should be given. Nurses would argue with her, because she asked to be seen by a doctor for pink eye which she didn't even know she had, well if they were doing there jobs then they would have seen it was bad. The case workers tho under paid for there are rude one in particular Shiela. She is rude, I mean so unprofessional with an attitude. Made comments in reference to some things my mom said to her in confidence. Is that not violating hippa laws. Is there no confidentiality. I feel that this place needs to be shut down the amount of suffering these patients go thru is sad. I am a caregiver I get paid nothing to take care of the disabled. Minimum wage but I would never treat my patients as if they don't matter. You don't like your job find you will, this field is not for everyone. The only person I felt that was nice and decent was mr. Reyes. He was nice even if he was having a hard day he is the only one who knew how to be a professional. This place is horrible. My aunt was placed here, you never see the social worker because she is never available. When you call you're given random numbers to leave a voice mail, but no one ever calls you back. When I finally did speak with a SW, she agreed to help the family move my aunt and asked for a list of locations near family, but nothing further occurred because she is no where to be found, will not respond to emails or massages left. My aunt is very depressed, she has lost a ton of weight, every time I visit, she continues to request to be closer to family, she says this is not a life, she believes that she is there to die, no one is visiting her. The nursing home doesn't care to listen to what the patient wants or needs, they just want to get paid. So sad that seniors who are high functioning can be placed in nursing homes by hospitals and if you do not have guardianship and are not their spouse, child or grandchild no one will listen to you either. This place is 2 hours away from her elderly brother (my father) and he is the only one that visits routinely, so why shouldn't she be closer to her brother. Don't the elderly have rights? I believe they deserve respect and should be treated with dignity. I give 1 star because there's a toilet w/shower in her room, it smells, but at least she doesn't have to walk across the hall. read more
