Friendly, clean, and so much socializing! A chef in a white coat and hat greeted residents by name as he scratch-made dishes! We played games! Visited! Activities! Then it was bought out. The director was replaced, then replaced again. Then again? We lost count of directors and caretakers. Each visit revealed a lower quality of caretakers. They just became aides. Each aide was worse than the last. Very low quality. Disappointing. Forgive my rudeness. All this is FINE in the world, but when you rely on caretakers to wipe feces off your loved one and spoon-feed them for survival, you want good folks. Folks with passion for care or QUALITY OF LIFE? The 1 or 2 aides would be sitting in the chairs watching TV in the back room. Every time we arrived. The last 5 years have been sad and shameful. She has fallen out of bed when side railing was not installed. She broke her femur and spent 2 years in excruciating pain. The bones scraped together against nerves. It was horrific. 2 years of this. Each new aide had no idea, so they just pushed her legs roughly. The aides never knew she had a broken femur. She just screamed in agony. She developed a severe bed sore. Infected. Oozing. Green. Horrid pain. We begged for an mattress pad that rotates the pressure points. They eventually installed an air mattress. When I arrived, it was unplugged and deflated. Grandma was laying in urine on a deflated pad. My grandmother was always laying in urine or feces. Her mouth and lips were always cracked and nearly bleeding. I'd put a water bottle and straw up to her mouth. She'd gulp down the entire bottle. GIVE HER WATER AT LEAST! This is not a pleasant situation for anyone. It is a difficult job. BUT, this is why we gladly pay over $5,000 a month to receive the CARE. QUALITY OF LIFE. Even though she was declining. We just hoped that they'd step up. My grandmother spent 50 years as a NURSE. She cared for the injured, the sick, and the elderly. She wiped the feces and urine. She was a woman with dignity and compassion. She has EARNED the benefit of receiving care! Instead, she spent 5 years in a closed room with the TV playing an endless loop of a Pay-Per-View ads because The June couldn't find a remote. Or, the TV Guide just scrolling a black screen. Silence. This place has become like a silent prison. Meals taste like microwaved food. Hallways are vacant. Silent. DOORS stay closed. 100% uncomfortable. No one is EVER visible. Humans avoid this place. Some residents BEG you to let them out. PLEASE HELP. They beg. My grandmother was ultimately prescribed morphine to ease her pain. When I'd arrive to see her in agony, I'd hunt down an aide to kindly as for today's morphine. This was a brutal request. The med tech HATED to be asked anything. Asking the med tech a question INCONVENIENCES her. On the morning after Thanksgiving, hospice called with the information that grandma had died. Hospice? We didn't hear from The June? We called for the procedure. We left a message. No call back. We tried again later. Left message. No call back. It's been 3 days. Still a mystery. The June has no procedure for COURTESY. No call for condolences. No call for information. The old, frail, retired nurse with dry, cracked lips and the broken hip will no longer need anything from the 2 aides watching TV. But I'm sure those 2 aides will be replaced by 1 aide who will probably be worse. And they called her the WRONG damn name for 5 years. How's that for dignity?

Upper management often appears clueless as to the decisions made concerning day to day living and how those decisions directly affect resident quality of life. I would suggest you talk to a variety of residents when you come to check the community out. I would also visit the care facilities and check out the reviews. Last but not least, have your attorney preview any contract before you sign.

However, executive-level decision making was pro-management, and NOT pro-resident. The CEO announced weeks prior that he was closing down skilled nursing at the West campus, and all current residents in skilled nursing would be moved to the East campus about 8 miles down a busy highway. My husband I moved to a CCRC so that we could be TOGETHER as we aged. However, with this new scheme, if one spouse was healthy and the other required skilled nursing, the couple would be separated. Some affected couples no longer were able to drive, making this difficult. Residents could no longer just walk within our campus to visit the other spouse. We decided to move, because if they take away healthcare despite residents' calls to change his mind, what will be next? So I'd caution anyone to do more research and ask the hard questions before signing on the dotted line. And I read that according to US News and World Report, they have gotten some sort of #1 rating, but just know that where we moved to, our healthcare facility continues to have a five-star rating from the Centers for Medicare and Medicaid. And the ARC is ACTUALLY a CCRC -- independent living, assisted living, skilled nursing, in-patient rehab, and memory care -- all available at the same campus. Imagine that!

Driving into the complex there is a gate guard so you have to have a specific purpose or appointment to even enter the premises. As you walk into The Mission, which is the building where my mother is, there are more security measures in place. The doors are always locked and opened by ringing reception. Then you must stop at reception and sign in electronically so you can be issued a pass. You know what there isn't!?! Adult diaper smell! Hospital smell! Old people smell!!! It is set up beautifully with separate halls dedicated to different attend to different issues. The level of care is second to none. EVERYONE is kind and pleasant. I have seen more humanity and patience here than ever before in my life. The level of dignity they treat the patients with is heartwarming. Their attention to detail is such a welcome quality... there are not enough wonderful adjectives to describe my experience with my mother here. I know there is independent living, assisted living, memory care, skilled nursing and rehab here.. I think they also offer palliative and hospice care. If you have a loved one that you find yourself needing help with, I highly recommend this facility!!!

I really thought this was the place I would live the rest of my life. I learned about Blue Skies in 2015 and compared it to other similar communities. Blue Skies remained on the top of my list over time. There are several things that prospective residents must consider before committing to signing a contract. The organization's business/finance practices are not at all good. Poor business practices include lack of transparency to residents regarding pricing of apartments/homes (it is a secret), and lack of coordination between various departments when moving in or out. In my case, I requested and was allowed an in-house move to a different apartment. I did have access to the "secret" apartment price list and assumed I would be paying the listed price for my new apartment. Imagine my surprise, after spending $2000 for a move, and just over a month AFTER my move, I find out the price of my new apartment is $257/mo MORE than what is listed on the price list. I never received any documentation before or during the move that informed me of the new rent rate - a violation of the contract that states a resident must be informed of any rent increase in writing at least 30 days prior to the new rate taking effect. Unfortunately for me, I did sign a lease addendum that showed the new price. PLEASE - do not do what I did! I almost didn't sign the addendum because the rent amount was much higher than I anticipated. I should have consulted an attorney then. I did do that later and found out that I should not have signed the addendum because I was never informed in writing of the new rent price. Also, there is significant financial mismanagement of resident accounts. I received 2 outrageous debits on a monthly statement. It was impossible to speak to or email an individual in the Finance Department to discuss my concerns. My voicemails went unreturned and emails never received a response necessitating my going to the assistant resident director for help. An issue regarding a $1369 erroneous charge was corrected, but only after the assistant resident director contacted the Finance Department - where he received an immediate pick-up of his phone call. Additionally, I had to confirm it was, in fact, an erroneous charge after the person from Finance asked: "Is she sure this is incorrect?" I was livid! The assumption from this Finance Department is that every entry they make on a resident's statement is correct, and it's up to the resident to prove them wrong. Currently, I am in the midst of attempting to get a refund of MY money for an incorrect rent overcharge for November 2025. Despite disputing the amount of this debit via my bank, Blue Skies Finance states that I approved the amount. I DID NOT! Because of their response to the bank stating this, my bank closed the dispute. I'm currently exploring other options for getting this corrected. And just know that rent money is not all the money they actually owe me. Please, just be VERY CAREFUL if you decide on Blue Skies as your next home. Look at other continuing care communities in the San Antonio area. There are also very nice 55+ communities in the area, with more being built. Blue Skies prices are the most expensive, or close to it.

My in laws have resided at this facility for more than a year, after "A Place Called Mom"…read morerecommended it. The facility offered many amenities when they moved in, and they were quite happy with their new living arrangements. Over the last seven months or so, the entire facility has gone down hill, and many residents have moved out. A recent change of ownership further helped to deteriorate living conditions. Management has been made aware of numerous issues, but has taken no action to correct the complaints. Although there have been numerous issues with maintenance, dining quality, resident comfort, and mobility access, the latest issue involves the resident's transportation. Many residents, my in-laws included, no longer drive, and rely on the facility transportation to get to the store, doctor's visits, and other shopping areas. The single bus used to transport the residents is often broken down, and scheduled trips are cancelled without notice. Even when the outage is known ahead of time, communication with the residents is poor. For example, the Monday morning trip to H-E-B was cancelled because the bus broke down on Saturday, but no residents were notified of the cancellation! When the bus IS running, many residents avoid trips if a particular bus driver is driving. He is nasty to the residents and absolutely refuses to help them with their bags when walking down the steps of the bus. His reason for refusal: "This is independent living!" Again, management has been made aware of the employee's attitude, but it continues. This same driver will make a resident wait at the store for more than an hour before he picks them up! There are many other retirement residents here in San Antonio, and I'd highly recommend you research them before considering this facility. This is NOT a place you want your loved one to reside at.

In this small facility, the hall smells of urine and of course the "big-overdone production" COVID screening with all the paper work you have to fill out just to visit. My friend was transferred from a 4 month hospital stay to that facility the end of February 2022 so, Monday, March 14th will be two weeks. It's my understanding, he is in quarantine after my visit for another 14 days. After watching my mom in another facility and now my friend, I pray to God that I continue to stay "under-the-radar" because I would rather die than live like this.

Hartman, LNFA interpretation of Lakeside visitation policy is not in complete compliance with CFR (Code of Federal Regulation), specifically as cited in Title 42, Section 483.10 Resident Rights. Though a lengthy governing citation, I took the liberty to read and compare the citation with the Facility's Policy. Please refer to Electronic Code of Federal at www.ecfr.gov to educate yourself to understand how visitation applies to your loved one. Our personal experience: My mother-in-law, an elderly women currently in hospice care at Lakeside Nursing Home/Facility is visited by my wife between 10:30 p.m. to (depending the condition and attention she receives) latest 2:30 a.m. and has been doing so for the past 6 to 7 months. These are the hours my mother was more alert before her conditioned declined and at my mother-in-laws request she hoped someone would visit at night. My mother-in-law can no longer communicate but she mumbles when she hears my wife's voice; reason enough for us to believe she acknowledges the our presence. Approximately 2 weeks ago, my wife visited on days she typically does not visit and found the following conditions substandard: -11:30 p.m. mother-in-law had a soiled diaper/pamper to midsection of her abdomen. My wife called on staff for assistance, the CNA on duty told my wife it wasn't my mother-in-law's turn to get changed. When the nurse came by to check on my mother-in-law (approximately 1:30 a.m.), my mother-in-law had yet to be attended. My wife refrained from reporting this to avoid the possibilities of retaliation from staff when she doesn't visit. What transpired instead was my wife being called in to hold a meeting with the Administrator to be told she could not visit at those hours due to violation of the roommates privacy rights as governed by Texas Law. (Texas.gov | The Official Website of the State of Texas Texas.gov | The Official Website of the State of Texas Access official, secure online government services and information for the State of Texas. Take it online, Texas! Resident rights; Health and Human Services mirrors CFR). Regulation cites a resident can have a visitor at any time of the the resident's preference, unless there is a clinical reason and/or the roommate places a limitation/request (this situation does not apply at this time). The Administrator further stressed if my wife continued to visit at the same hours, nurses would be instructed to prevent my wife with visitation access to my mother-in-law. I requested the Facility visitors policy from the Administrator and she continued to express our violation of the roommate's privacy in accordance to Texas Law. I requested the policy 3 times and didn't receive it from her, I ended up receiving it from another staff member. -The room's air condition was turned off, extremely humid and uncomfortable. Regulation cites rooms are to have temperature set between 71 and 81 degrees (seasonal settings are to be suggested). -Resident's restroom hand soap were heavily diluted with water. -the roommates wheel chair was away from her reach and she simply wanted to go the restroom. The roommate is mobile to use the restroom with assistance. My wife found it disturbing the CNA neglected to assist her. We have reached out to the Facility's Ombudsman and have been assured our concerns will be addressed. We do not want to take away that 95 percent of the staff are exceptional, but it only takes one downfall to present itself to rate the facility with a one star. Furthermore, we are more troubled how many visitors have been banned to visit their loved ones and for the loved one to feel abandoned (that is; their loved one is a "night" person like my mother-in-law). How many elderly are being neglected when there is no presence of family visiting? If you have been approached with limited visitation access, we suggest you take time to read the visitation policies as governed by Lakeside, Texas.gov and ecfr.gov; or, contact the facility's Ombudsman.

I did get a call from a director of nursing (3 days later) who was not able to answer or address any of my concerns because she said she was just on her cell phone and did not have access to her computer. Why would a director of nursing make a phone call regarding patient concerns without having access to the patient's file?
