The people that I interacted with were not only friendly, but they were over the top helpful and informative. Early in the process, his case manager contacted me to fill me in on the procedure and his progress. She also called me when his discharge date was confirmed to set up a family education and all the things that they will take care of surrounding his discharge. She ordered the equipment, the Medicare funded home health, and even the wheelchair transport to get him back home. At the family education session, we met with his very knowledgable and extremely friendly physical and occupational therapists. They helped us understand all of the things that he has been doing and how to do a lot of other things. They offered suggestions on things to purchase that insurance wouldn't cover. They even gave their cell phone numbers and offered for us to call if we have any questions. 5 star experience all the way.

My mother, is completely immobile and fully dependent on staff for her safety, hygiene, and daily care needs. During her stay, staff repeatedly failed to provide adequate hygiene care and failed to maintain basic patient safety precautions. Most alarming, my mother fell out of her bed while under the facility's supervision. At the time of the incident, the bed rails were down, the bed alarm was turned off, and the bed appeared improperly adjusted or overinflated. Despite her being a known high fall-risk patient, appropriate safety measures were not in place. This was not a minor oversight -- this was a serious failure in patient care and supervision involving a vulnerable patient who could not protect herself. Families place trust in rehabilitation facilities to provide safe, attentive, and dignified care. Unfortunately, our experience raised serious concerns about staffing attentiveness, patient monitoring, and overall accountability. We have formally requested an investigation into the circumstances surrounding her care and fall. I hope this review encourages the facility to improve its standards and helps other families make informed decisions when choosing care for their loved ones.
