Insane. 4 months later I have a debit card only to be locked out due to not entering HIS information for the prompts even though there was nothing asking that I give that information instead of my own.

There is no way for them to tell if the system is working at any time. After switching to Bank of America Santander somehow kept my checking account open with a -amount they sent to the rogue company. My account is fairly new only about a month old and the idiot banking manager who opened the account gave me the wrong routing number! I had just started my new job and gave them all my banking information and when it was time to get paid guess who didn't get paid. And on top of that the same day I was supposed to get paid my car note was also due. Santander charge me and my account was overdrawn. I had to wait an additional week just to get paid again and they took half of my paycheck.
