I have so many good things to say about Creekside Rehabilitation. The staff was; responsible, respectful, patient and kind. Even though, I was under half the usual age of the average resident. I felt well accepted by everyone. I had to celebrate my birthday during my stay st Creekside. The nurses and aids even threw me a little surprise party. I felt very welcome and loved. I knew it wasn't home, but if I ever have to recover from a huge surgery inpatient again, I will be going to Creekside again. I wouldn't be walking as good as I am now. Not just NO. I wouldn't even recommend this place to my worst enemy. One point in time I found feces on my dad bed, his bed was made once in the 2 weeks that he was there. I brought the above to the attention of the staff who then took care of the situation. There is an orange form that you can fill out for any complaints but again I was afraid of retaliation or even worse care for my dad. read more

My uncle has been shipped to the ER three times in one year: for infections due to poor hygiene at this facility. The nurses are rude, lie and back each other up in getting away with it. Awful communication with family! All three hospital trips we were NOT NOTIFIED!?! (Patient is 81 with advanced dementia.) How scary for him and us, his loved ones. Go to Bell Tower. Please. It's just too risky as far as being unsanitary. Wow. read more

Creekside Village is Tops on my list. They have a staff of nurses assistance and workers for second to none. I have been there now twice in the past 4 1/2 years and not that I wanna go back again because there's no place like home but Creekside comes in a real close second there Department is great they take the time and make you feel like you're really doing a good job. I'd like to thank each and everyone of the employees there. Kudos to Creekside Len DeLorenzo south Bend. Indiana I give them a five star rating. I'd give them more if there was a way. read more

My Dad (with dementia) was in rehab late summer/early fall '22. About 6-8 weeks. I would NEVER send him there again (and I can't now anyway since he passed in November). I had to ask repeatedly for them to shave and shower him. He stunk. He wore other peoples clothes all the time even though they had supposedly labeled all his. Not sure if we ever got all of them back. He was left to sit by the nurses station most of the time because he was a fall and escape risk. I understood that, however, the last time he had to go to hospital was only because one nurse cared enough to say "something's not right with him and nobody else will listen to me". Ends up his hemoglobin was at 4.7! He would have sat there and died if it weren't for that one nurse! What really has me hot is that his wedding ring (of 71 years) was stolen! We reported it and have not heard one word back. Then they have the nerve to send me a bill for almost $300 after he has passed! How about you find the freaking ring first! My Dad was buried without his wedding ring because of these idiots! My mother is still living, but I will never take her to Creekside. read more

My mom was a resident at Creekside for about two weeks while recovering from pneumonia. I visited every day, and was impressed with the level of care the residents receive. Tasty meals (with alternate offerings), regular ice water/juice cart, kind staff, and attentive therapists. Though she is a Medicaid patient, she had her own room and bathroom, with a window that overlooked the lawn. The highlight of her stay was a group of tap dance performers (ladies ages 70-93) came in and danced to the oldies while the residents enjoyed cookies and punch. A residential center will never be just like home ... but if you're looking for safe, expert care for a loved one, be sure to check out Creekside Village -- right down the street from St. Joseph Medical Center. read more

My father was at Creekside Village for 2 weeks before he passed away. I love everything about this place from nursing to aides, dietary, housekeeping to the business office! I was able to stay with him night and day! They were not only wonderful with my father, but also with me! I recommend Creekside Village to anyone needing care for their loved one!! read more
