My father was in this facility​ for 3 1/2 weeks. I have visited him several times during his stay and the staff extremely​ rude. After this review Im reporting this trifling facility to the state. The administrator Gregory Dwyer is a rude idiot and it reflects throughout his staff. The ONLY person who is helpful and has some sense in the entire place is the front desk receptionist Cynthia. EVERYONE else is rude and a bit slow. Id give this place no stars if that was an option. My mother was in this facility for about 2 months and from the moment she got there, there were nonstop issues. For the first two weeks I couldnt get info even if I went up there because the Admissions people couldnt be bothered to call the hospital where my mother was transferred from to see I am her next of kin. They made no distinction for my mother being there due to other medical issues but including a memory issue (possibly dementia) so they would just sit her food nearby or across the room (I witnessed this) and my mother is immobile. We repeatedly told them she needs to be fed and will not remember that she hasnt eaten, etc. etc. They didnt care. Fast forward I get a call from a HOSPITAL that my mother was admitted. CC couldnt be bothered to call me and let me know this then lied to their leadership saying they notified me. No messages, no missed calls, no emails, no texts. NOTHING. I was also left out the care planning sessions until they finally added me to my mother's next of kin contact info. Care plans were not scheduled so I couldnt go in person. When I finally was a part of a care plan, the total call was less than 5 minutes and I was met with attitude when I asked what was next because all the "care plan" covered was progress, not next steps. Note care plans are to include general treatment (medications, etc.), PT/ OT, and wound care. Most of these topics were glossed over and I was told someone will call you back. They never call back. In the few phone calls I had with various staff members (because all they do is pass the buck, "thats not my job" or "Ill have someone call you back" - no one ever calls back and if they do its like groundhog day and you have to repeat everything all over again). Fast forward to I get a call on Jan 6th - a Tuesday (6 days after the new year) for them to say your mother is being discharged today (NO notice) because she doesnt have insurance. She in fact did have insurance. A new policy that started at the beginning of the year. Context, admissions submits pre-auths to the insurance at least a week prior to the week in question to ensure the insurance would cover. So how shocking it would be to hear on the 6th that I may incur a bill for 6 days out of pocket because these people "didnt know" about the new policy but named the policy to me. So again, had they been doing preauths like they should, there would have been no issue. By Friday, theyre telling me her new discharge date is now the following monday. Note I had been begging them to assist me with requesting the medical equipment needed for my mother to be cared for at home. So now we have to scramble and coordinate everything on a friday afternoon to ensure things would be available Monday. I go to the Administrator's office to address these issues and this man talked down to me and got loud several times to the point ppl started gathering in the hallway. I asked him if he thought their treatment would be suitable for his own mother and he said no but kept saying theyre a work in progress. In that same convo where Im asking them to confirm what medical equipment they ordered, these idiots literally told me they couldnt request a medical bed for my mother because she doesnt have a diagnosis for one but has been in one for months in their facility. Make that make sense.I mention the large bed sore on her back and then all of a sudden thats enough to get a medical bed so what were they reviewing while ordering the equipment if I had to tell them where to look in their own documentation. He also kept insisting we get rid of my mother's private insurance for state insurance. Who the hell does that if they have the means of being on a private plan? Anything to trap my mother there and they take all her assets. I asked what happens if monday comes and the equipment isnt available, this man literally shrugs his shoulders. She finally comes home and she is COVERED in bed sores, huge scratches and wounds all over her body. She also cannot extend her legs so she cant even be in a wheelchair. Explain how that happens if shes getting PT and OT like they swore she was receiving during those bogus care plans? Right.. And less than 2 days later she's back in the hospital with an infection from a dirty catheter. Condemn this place! Hope everyone in this dirty place is fired and dealt with including the social worker Ms. Daniels, the nursing supervisor Dio, Kayla in Admissions, Dawn in billing, Eileen in discharge coordination, and the Administrator over the entire building Gregory Dwyer. This place is the pitts and I don't receive the proper care that I deserve,the nurses come in and don't start caring for patients in till 3 hours later when they finish caring on like it's a bar,being all loud the only nurses that work are the ones that work M-F 9to5 because all the bosses are here and they don't want to be told on,after that time is when the low life's come in and you won't get the care that someone needs,I ask for medication and they tell me to wait 5 minutes and that turns out to be a hour when they decide to get off the phone or ipad and only when I'm down to my very last pill is when they ask the Doctor to sign the prescription,I been getting 10 minutes of physical therapy a day and the discharge person tells me I been getting 30 to 45 minutes a day,I'm using up all my physical therapy days and not getting any because what's going to happen is there going to suck all my days up and once I run out is when there going to discharge me and when I go where ever they send me to I will still be weak and fall and hurt myself and be right back to the Hospital and when she ask me after discharge did I have a place to go and I didn't she told me I would be discharged to a homeless shelter,the Hospital faxed over all the health care I needed and medication that I was on they could not care for me properly or have my medication ready the very first night I had to go to the ER because my pain and anxiety got to me and I ask the unit manager why did there take me in if they couldn't help me she said they never turn anyone away that's because they have bed's to fill and in need of body's,the food here is trash very little portion,cold,gross and uneatable and everyone gets a small lemonade at every meal no wonder people walk around here like death,the call button goes unanswered to where I have to get dressed to find my nurse just sitting there ignoring the panel that shows the room number and the amount of time that the patient was waiting and once I seen the red light going on at a patient room meaning they were in the bathroom most likely on the floor and I seen the time was 30 minutes and I brought it to the nurses attention they said they knew why don't you do something about it then,I spoke to the head of nursing,unit manager and even the director and was told that something would be done and not a thing was done that's because they cannot find any high quality people to work in a nursing home and they need every one they can get because they don't want anyone to quit over me,so if you need someone that needs nursing home care they will not get it here, anyone that needs help bathing, eating, cleaning them selves are in for a rude awakening. read more

My stepfather passed away on January 27, 2026, after residing at this facility from December 3, 2025. What my family experienced during that time has been deeply disturbing and traumatic. Issues we observed included concerns related to food quality, communication, and the overall treatment of residents--especially those with dementia or who cannot fully advocate for themselves. These are individuals who rely entirely on staff for safety and dignity. Since his passing, I have learned information through police involvement and from other families who had loved ones here. Hearing similar stories has been heartbreaking and alarming. If you are considering placing a loved one here and have the option to move them elsewhere, I urge you to do so before being reassured, dismissed, or gaslit into trusting care that may not be what it appears. Families are often told exactly what they want to hear. I was very involved and asked many questions while my stepfather was here. That should never make a family feel unwelcome--but it did. The only pleasant experience that I experienced in this facility was the front desk young lady by the name of Cynthia. The first night my mother had dialysis and it appears that she was given something because she was totally out of it. She didn't even know she had a visitor. We asked the nurse and she said she would find out from Dialysis because there's no record that she has but she never got back with us. My mother said the food is horrible and cold. The staff was not very nice maybe a few exceptions but overall the nurses were too busy and you could never reach a nurse. We did speak with one nurse name Donald who listened to our concerns and request and he went in and followed up with my mother. My mother also said that one of the technicians was mean to her because she needed to be changed and was waiting for over an hour. She felt like she intentionally left her uncovered, and she closed the curtains after she asked her not to so she couldn't see out the door. My mother had a doctors appointment to follow up after surgery and the transportation team did not pick her up so she missed the appointment. We had to call to let them know that she had missed the appointment. I called the supervisor of nursing line for a follow up call back and no one has called back. I don't recommend this place. There's room for tons of improvement from all staff. Personally, I think Helen was a good tech and Donald was pretty good because he followed up with us and heard our concerns but the other nurses not friendly and appeared not concerned. read more

This place is horrible... if you care about your love one.. DO NOT BRONG THEM TO THIS FACILITY! They af.it you on the 1st floor, which I'd give 2 stars. Then moved my relative and her roommate to the 3rd floor. OMG... rooms are dirty, floors look like they haven't been swept or mopped in a long time. My relative is a stroke patient who needs assistance with feeding, which I was assured would not be a problem... well they bring her food, already cold, come back over an hour later to feed her. I arrived at 5:30 she still had a gown on. She has clean clothes for everyday of the week, they put the same clothes off two and three days. I know because I wash the clothes. At 6pm, they came to give her a shower. Put in bed but she needed to go to the bathroom, nurse just pee in the bed you have a pad on. I could go on... bottom line... don't bring anyone here if you care about them. I have pictures to confirm and document. They receive this 1star because I can't put 0.​​​ First for rooms have a white board some you can write on, have necessary information posted but not on the 3rd. This facility is very deceiving because the decorations as you walk in are beautiful, once you pass registration, it's all downhill. I had to call the police on the nursing assistant for threatening my grandmother that he was going to punch her in the face twice. The staff members are very rude they walk around in street clothes. My grandmother's roommate was elderly and has dementia. She made her way over to my grandma's bed and the nursing assistant came in and threw her on her bed. Don't let this place you fool you by its appearance when you walk in. It full of African nurses and they treat the patience horrible. They are mean, nasty and lazy!!!! They gave one of the patience sharing a room with my dad a small plastic medicine pill cup to drink water from. They didn't have any blankets for the patients and didn't care that they were cold. My dad just died on Wednesday 8/14) from being in this place 2 months. You can't speak with any doctors. I complained to the Director who is also African about the nurses, not once did he ask for a name. My dad was afraid to tell them if something was wrong because they were so mean. I should've left when I had a chance. I asked for a remote to the the tv and no one knew where to find it. So I have a tv on CNN with no volume. Then I got in bed and in the ceiling over my head is a big water spot with mold. (See pic) Then the nurse said I will bring you some food. She brought me 2 frozen chicken nuggets and some coleslaw and a cup of juice. Horrible food. So now I'm in bed with tv with no sound, and hungry. I had to go to the bathroom and like I thought I would see when I went in the bathroom was roaches running. But here is the kicker later on that night I asked for something to drink, the nurse said it would be $1.00 I thought she was joking. But unfortunately she wasn't. I would like to first say that any nursing home had good and bad things , the staff make the stay for loved ones positive or negative. Mrs. G. Mbony, the social worker/director is the most kind, loving and Compassionate individual I have met. She provided great knowledge, support, time and patience with each member of my family. No matter when we were at the facility. Early or late, she was there at the nurses station working very diligently to solve everyone's problems. This center is so fortunate to have an employee who has been at the center since the day it opens and is still the director after 15 years and countless administrators. When a loved one is in a skilled facility, it is challenging enough. But when you have kind staff who truly care it makes all the difference. If your family is admitted here , I hope you are fortunate enough to have a discharge planning with mrs mbony! She is the ONLY reason I would give this center a three star. Would give zero if I could!! Horrible service! Sent my Aunt home with no discharge instructions, nurse calls a half ass report twenty minutes before she arrives. They sent another patients medications , ID, and insurance card home with my Aunt. Called for several days for someone to pick up, no one returned my calls. My father was in this facility for four weeks and visited him every other day. I'm not entirely sure if it was our race or my frequent visits and phone calls to smooth things over, but he was treated well and improved incredibly with the help of the physical therapists that are often contract workers. Pros: The morning staff was friendly, I would like to give shout outs to Ms. Edwina who was always teasing my father in a good way when I was visiting and Ms. Evelyn who was my father's morning shift main nurse. The food was enjoyable for hospital food. Cons: The evening shift was civil from what I understood though they were under staffed. The biggest problem was not having enough people during the night shift and not having enough basic equipment. The nurses and other staff within the building knew my father and my name by the time we took him home. TLDR: I wouldn't worry about him coming back here again, but I do hope they bolster with evening staff with competent and civil staff and get more equipment. read more

After having many family interaction with this facility for over 15 years, I am absolutely amazed by the lack of caring, morals, and ethics demonstrated by the administrator, AMI. When this center was a charity; it was run with care and compassion. Residents were treated with respect! After haveIng several interactions with the so call "administrator" it became clear she cares nothing about residents or staff and only about herself. And covering up. Standing at the nursing station, staff will bad mouth her not caring who is around. While I was waiting for the doctor, I heard from a nurse; she take food from the kitchen for herself home. That basically is taking food out of the mouth of the residents. This is not right! Please thing before you have a family member stay here. Check to see who the administrator is first. I would like to end on a positive note. Ms.grace Mbony was so helpful, kind and knowledgeable. She was the only staff I remembered from my 10 years coming yo this center. My relative is currently at Crescent Cities for rehabilitation. The service and level of care is unacceptable. Very hard to get someone to answer the phones to get updates. It 4 different relatives to call 4 different phone number at the same time to reach anyone who would answer any phone with no luck. When we are transferred to the nurses station nobody answers the phone. On one occasion a person answered the main number and said that she didn't know how to transfer calls. It is very frustrating especially during COVID when you cannot visit you loved one. We have placed calls to the Director who doesn't return calls. My relative's health has declined since being there. We sent them there for their health to improve. My relative is currently at Crescent Cities for rehabilitation and the service and level of care is unacceptable. This establishment need a complete makeover from the staff and all areas of what it takes to run a safe, efficient environment for residents, workers and visitors. I wouldn't advise or recommend anyone receiving care there right now. My mom cried to the point she checked herself out. No bed changes for 4 days, no working call light system, a cow bell to get help. No shower for 3 days and used potty chair with a bag in it no support back on it. Had to wait over 20 to 30 minutes for help and wearing a depends in the bed. No resident chair in room just a dining room chair. Severely short staffed.Sanitation in that form and she couldn't walk to help herself so it was sad. When I came to visit was the day before she checked herself out and I was told I had to provide my own face shield or I couldn't see her. So some of the PPE equipment they had been out of for 3 days I was told. I was given a N95 mask an was told try my luck seeing her. No temperature taken at front desk until I said hello my temperature. Bedside manner of staff was not inviting. I feel if you don't like your job go somewhere else. The last thing a individual needs is to not feel comfortable when rehabilitation or getting qualified care. A skilled facility has ups and downs but some areas absolutely have to run a tight ship. Two of her aides were sweet she said but some just were not so friendly and made I know when asking for help. Normally I don't say anything but this field has been a complete passion for me for 26 yrs I have been in it. This isn't a recommendation for me. I read on here before her coming praying it was better but I guess not. It's super sad because it was one great aide she liked but didn't get her again and it went downhill from there with how her care was presented after that first full day. read more

Any place that's owned by a Genesis is a horrible place. I was in Sligo Creek and got treated like garbage and witnessed a lot of the patients being treated the same way. No need to report anything to the Directors because they cover up for their staff. The nurses have nasty attitudes, are negligent and just down right trifling. I prefer a place that has diverse staff and they only hire African and Caribbean people that have no decorum on how to talk to people. crescent cities is also owned by Genesis and my Dad died 2010 in their care. They would leave him laying in his deification all night, didn't administer his insulin correctly, went all day without giving him anything to drink. The State and Joint Commission should have all of these locations closed. read more

My experience with Crescent Cities has been that it lacks professional standards that you would expect and require from a rehab or nursing home. The doctors,nurses, and caseworkers do not call to give patient updates to families or primary care physicians on a weekly basis. You can't expect to speak to your loved ones daily or weekly because calls to the main number are constantly rerouted to numbers that just ring or have voicemails with no name. The same phone situation occurs when you call to speak with medical staff therefore it's impossible to inquire about patient care.The initial call to inquire about the facility was misleading because they act like they cared and because Cresant Cities name is on hospitals referral list you want to believe they are well qualified despite previous negative reviews. I can only pray that they are treating the patients better than they are treating communication with outside doctors, families and friends. This review is simply to negate anyone else from experiencing the unfortunate low quality of care that I have experienced with Cresant Cities. read more

I called a meeting with the head of the facility but word started spreading and my mom's bf was concerned that there would be an even greater diminishment of care. So I called off the meeting. I could only be there once a day to monitor her care so I called off the meeting. I also remember the food being awful. I think if the state really wanted to make sure patients were getting quality care they'd actually have monitors stay there or require CEOs of these franchises to stay there so needed changes occur. read more

This place has absolutely no customer service. The facilities are clean, but in my opinion, please do not bring your family member here if you care about them. The staff seems to be uncaring, lazy, and unprofessional. When you ask for something they take a long time getting it, and then when you ask again they get an attitude with you. They leave your family member sitting on the potty chair for a long time and take a long time to clean them. Also, when you press the emergency button they take a long time to come. To me, that is unacceptable. I just had a nasty encounter with one of the nurses, named Grace. I simply asked her about using a wheel chair and she got an attitude with me about asking her to use a wheel chair. That same nurse was abrasive and rude with another nurse while she was trying to tend to my family member. It was very awkward. That's poor customer service. With nurses like that how can you feel comfortable with your family member at Crescent Cities. Majority of the staff is just inattentive. They need to get new staff. But please do your research before selecting this place. read more

My husband was there for a 2 week period. the intake nurse that met us when he arrived was AWESOME. Her personality made me think it was a pretty decent place....fast forward... the NEXT day .... He was asked to what he wanted to eat (of course they knew he was a diabetic + cardio patient). Not only did they NOT bring him anything he wanted but most of what he received (throughout the entire stay) was cold, non-descriptive or definitely something he shouldn't have even been eating. The juices were always watered down and there was never anything else... I was even told that there was no water.... . His catheter sprung a leak and I went to get a nurse... of course no one was at the desk (like most times and the couple time there was someone there she was not helpful at all) . when I did finally get to talk to someone she said "we don't use that kind of catheter"...someone finally came about an hour and a half later to clean him up. He was given an Incylin shot... the needle was just left laying on the table. I said something to the nurse about it as she was leaving and she caught an attitude and said you could have thrown it away..(ME ???)... I did report her and the next day she was not working his room but a supervisor was. She was back 2 days later with serious attitude. My husband said she was doing things that were very hurtful to him in retaliation. Lastly, the toilet overflowed and we were told the maintainece people were gone for the day...(REALLY) ... there was a young man who was about to get off from work and he and I took towels and sheets from a nurses cart and sopped water. He was apologizing the whole time... I reported everything to our Provider as it was occurring and I am hoping that this facility is taken off of their list of places. I also send a grievance to "Genesis" but that seems to have just been place in the oval file cabinet. It is just sad .... if this is what my husband was going thought while I as there (and I was there everyday when I got off of work @6:30 until midnight) I can only imagine what other patients are enduring that don't have anyone there.... So if you love your family, I would pass the word on NOT to use this place....Ever.... read more

My mother has stayed at Crescent Cities in Riverdale, Md a number times. It is overseen by Genesis--they manage the facility and billing, but they don't own it. She was always treated kindly and the staff ranged from fair to good. we even had her there for respite when the family went away. The other times were for rehab when she had been in the hospital. Its not perfect, but its kept clean and the patients seem quite okay. I have seen many and its certainly not at the bottom. read more

My mom was sent to this facility by Kaiser Permanente after a stroke for rehab.The only reason I gave one star is because the therapists weren't bad. They were knowledgeable, courteous and very patient with residents. The resident techs on the other hand were very rude. They were very slow to respond when my mom needed help and when they finally did show, were upset with my mom for ringing the call button more than once. They also did not want to help my mother use the bathroom. They wanted her to use her diaper to be changed whenever they got around to answering the call bell. I would not recommend having a loved one STAY there. If there is an outpatient program, I suggest taking advantage of that instead. read more
