My family members and I generally find that most facilities have their strengths, but unfortunately, our experience with Cypress Village left us disappointed due to a couple of major issues during my sister's stay. She is diabetic and spent about a month there, but encountered two troubling situations. The first incident involved her request for snacks and water, which she needs to help maintain her blood sugar levels. When she politely asked a staff member for assistance, she was told the cafeteria was closed and was refused a second time when she asked again. It wasn't until the issue was escalated to management that she was finally provided with what she needed. This delay could have been dangerous for her health. The second issue was even more concerning. On August 6th or 7th, my sister noticed that $50 and a bottle of perfume were missing from her bathroom. She attempted to report the theft to the administration team (Melissa and Joy), but received no meaningful response. Instead of support or investigation, the situation was brushed off, and she was even made to feel as though she might not have had the items to begin with. At no point did anyone take accountability or offer reassurance. We expected a supportive and professional environment with strong management, but what we encountered was poor customer service and a lack of responsibility. I sincerely hope Cypress Village takes this feedback seriously and makes improvements to ensure future residents and families do not experience the same issues. this review is specifically focused on ms. kelly mccartney of this facility - the most caring & professional social service coordinator that helped me for weeks in assistance with my 95-yr old mom...kelly is an angel & a blessing to her profession in kindness & compassion. she gave me continuous feedback on steps in insurance, medical interactions, care concerns & suggestions overall, including hospice. she never failed to return a call - no matter what time i reached out to her. her knowledge was recognized by both my wife & myself immediately, as well as her kindness always. she is a blessing to this home & should be recognized for the gifts she has given in this matter. i feel blessed to know that there are individuals such as her that care about their loved ones. with blessings in christ, dennis & deb strickland jacksonville, fl read more

My mother has lived at Cypress Village for 6 1/2 years, first in an independent living apartment, then in Assisted Living and now in Long Term Nursing, where the staff treat her like family, they are so caring and compassionate, especially CNA's Sharonda, Mymrose, Martha and Tawana and nurses Edward and Maria. Mom loves them! It is wonderful to see mom so happy and for our family to have peace of mind that she is very well taken care of. read more

I wanted to like this place. I really did. It's pretty and it's nearby. They have great industry ratings. My 80 year old mother was here for post-surgery rehab from 4/27 until 5/18. She has been in several rehab facilities in the last five years based upon where in the state the event giving rise to the need for rehab occurred. Without question, this facility was one of the least diligent and most frustrating. Yes, the physical set up of the facility is nice and my mother advises that the PT services were good. Food was mostly edible. But there were issues. (1) staffing is woefully inadequate and/or staff are too busy playing on their phones to respond to resident call lights or provide care. My mother (who was non-weight bearing and VERY low maintenance) routinely waited upwards of 30 minutes for someone to respond to a call light for assistance to the restroom. On more than one occasion (in 21 days), the response time was more than TWO HOURS. And that's 2+ hours without someone so much as bothering to pop their head in the room to see if the call light was for something urgent/emergent. As a result (not to mention the indignity of it), she suffered a diaper rash from sitting in her own urine waiting on assistance. (2) Staff did not bother to attend the mandatory care conference that THEY SCHEDULED--not just one department absent, but several departments. Specifically, PT staff failed to attend a care conference setting expectations for a REHAB patient. (3) Room was not kept tidy, trash not timely removed. (4) My mother came home with scabies. She absolutely did not have the bites upon entry to the community, as evidenced by her admission skin assessment. Apparently, there is an outbreak in nursing homes in the area. Unfortunately, while we noticed what appeared to be scratches or bites on her arms and legs immediately, she was home for several days before someone advised the sores were actually scabies. She'd had the sores for about a week before she was discharged from Cypress, but no one there made any indication they were anything requiring treatment and she did not mention them to us. Of course, once she was home, and (because of Cypress' poor discharge planning) there was no hiding the sores from us as we provided personal services to my mother. (5) the facility suffered from a complete failure to properly handle discharge planning. From the day she was admitted, we had a set discharge date. We confirmed and reconfirmed it. Family double checked with facility the necessary equipment orders and outpatient services. We saw the doctor the day before her planned discharge date to be sure discharge was suitable (it was). The last four days my mother was there, no one returned calls. Not admissions, not nursing, not social work. Repeated calls. When I kept calling until someone answered the phone, instead of handling the issue, I was told to call the DME provider to confirm the equipment they ordered (it had NOT been properly ordered). I was also told by the facility that my mother's insurer needed to provide something for discharge (a flat out lie-according to the insurer). I can't help but think that they wanted us to exceed the covered 21 days, so they could bill for private pay. And if that's the case, shame on them. When their failure to properly manage and execute a timely discharge became apparent, staff kept threatening her that if she left as scheduled and as we had repeatedly previously confirmed, she would be leaving AMA (another bold face lie--her doctor agreed, her insurer agreed, there was no reason for her to stay, especially given the lack of attention and care she was receiving) and staff further told her all sorts of falsehoods about what the consequences of leaving AMA would cause specifically to scare her (no insurance payment for her stay, no DME, mark on her "permanent record" etc.), all of which were untrue. Not to mention that she was NOT leaving AMA. Oh, and referral to home health for post discharge assistance (a requirement imposed by law on the facility) was to an office that DID NOT SERVE the patient's address. I mentioned that the office location seemed remote for the service location and asked them to recheck. They did not. I did and as I suspected, the home health agency to which they made a referral does not serve the location. As the result of Cypress' complete and utter failure to perform their obligation to discharge plan, it took SIX DAYS after discharge (non-weightbearing and in a wheelchair) for an assessment for PT/OT and nursing and TEN DAYS after discharge for her first PT appointment. My brother had his stomach & esophagus removed so going to rehab after the operation was scary. Cypress Village nurses, Administration & and Social Worker were the most caring/empathetic people he could've ever wanted to provide care for him. After two weeks he had to have another major operation and after 17 days in the hospital he chose to return to Cypress Village. He was welcomed with open arms & once again provided outstanding care. This isn't my first rodeo with rehab care...Grandmother/Mother & Father. This experience by far was the best!!! The staff truly care about the people. Rehab was excellent! If I ever have to go to rehab I would choose Cypress Village. I am my brother's caretaker & I cannot thank everyone enough who took care of him ! They listen to your needs and provide top notch service. Thank you for everything you did to get him well! read more

My husband and I are in our eighth year of independent living in Cypress Village JAX. We came here because of the multiple levels of care. A loved one going from one level to the next is a trying, stressful time for families. We have confidence in of our nurses, wellness and management staff. We have a world-famous clinic beside us (look that up) and I volunteer there, who send recovering or rehab patients to Cypress Village. So, please don't pre-judge us. Come and see for yourself. We feel safe, secure, and cared for before, during and after covid. We have no family members in Florida, so the staff feels like our own. My husband was in the hospital two times in January, 8 days each day. He became so weak and still had all the underlying health problems. He needed to go to a rehab before coming home. His insurance is United healthcare complete . We placed him in a nearby facility in a shared room and it was the worst nightmare imaginable! After 7 days of this terrible experience, we transferred him with Debbie's help into Cypress Village. He has a beautiful private room, wonderful staff, responsive staff and is getting the help he really needs! God bless everyone involved in this transfer and God bless. Debbie and Cypress Village! My only regret is that we did not go to Cypress Village directly from the hospital. read more

Cypress Village rehab is a shit hole. I've been here for two days the place is filthy the furniture in the reception area and in the TV area is old old nasty dirty blood on the seats place is not clean the people are not that friendly or give a shit about your medicines your medication and your pain meds it's already 1130 and I haven't received my medication yet and I take it at 8 o'clock in the morning. Go to a hotel it's cleaner nicer the food is a lot better and so is the service. when I first came in all they cared about is me signing the paper so they get paid after that I haven't seen anybody got all big mess up with my medication's even though I had the papers for the hospital saying what to take they didn't have them they didn't wanna give them to me they had to get authorization from somebody else it was just a mess half of them don't even understand English. REHAB Skilled Care Quality of care was not great They messed up the dose of my Meds for over a week after me going on and on about it being wrong. On 2 different Medications. They were suppose to have a CPap Set up when I arrived and it did not for over a week and then it was wrong and repeated attempts to get it right. Issue with nurses so much so had to have director of Nursing to come in over the weekend to make something right. I would not send anyone there since read more

I was in Cypress Village rehab center after hip surgery and had excellent. Their Occupational and Psyical Therapy folks were some of the best And I have seen more than a few over my husband's long illness. If I ever need care again I will insist on going there. read more

My 93 year old Mother was put into CV Health Care rehab after hip replacement on Saturday, 12/3/22. We arrived late in the afternoon and she was given a meal which was decent. After that it all went down hill. We were trying to get her settled and turned the TV on for her. The remote was not working well so the Supervisor got a new one for her that still did not work. Then the bed would not operate properly and a new remote was needed for that. We asked for a walker because that's what she had been using in the hospital and they said she would have to use the wheel chair until evaluated on MONDAY! We tried using the wheelchair to get to the bathroom and it was very hard. We asked again and they said they would help her to the bathroom. We finally left around 8:00 and by 8:30 Mom was calling us saying she had been pressing the Nurse call button for a half hour for a pain pill and no one had answered. A CNA did come in while she was on the phone and Mom said "never mind she's here". Then the CNA told her she didn't have any pain meds and Mom said she was wrong. They FINALLY gave her the pain meds at 11:30 that night. She was given just enough water to take the pill. When she called to go to the rest room, which they said she could not go by herself, NO ONE CAME. She basically laid there all night without water because no one would answer the call button. She called at 7:00 the next morning crying to be removed from there. We were there by 7:30 and removed her. She was so dehydrated her BP dropped super low and it took us all day to re-hydrate her and she was exhausted from the ordeal. The next day we called the administration person Stephanie Markoski 3 times and she never returned out call. We did get a call from another women but the only thing she said was oh sorry, that shouldn't have happened. This place looks great from the outside but think twice about putting your loved one in this rehab! read more

My grandmother has lived here for 7 years and we couldnt be happier. She inititally moved into their assisted living but developed Alzheimer's so we transferred her to their memory care about a year ago and she is doing really well and is very happy. The staff had always treated her very well and she tells us that, she really likes the food and has made a lot of friends while living in both areas. She still has lunch with her assisted-living friends. We are very pleased with cypress. read more
