To make a long story short, here is are bullet points of what occurred: 1) Reached out to OM to ensure they accepted the new insurance plan I was considering. I was told by TWO different employees that they accepted it. 2) I used this insurance to see my doctor several times over a health concern. 3) Received a bill stating I would need to pay all out of pocket because OM didn't accept my plan. 4) Reached out to various departments at OM asking about this. After several attempts and many delays in getting someone to respond, I was finally told to speak to the office manager. 5) Office manager informed me they would look into it and see what could be done. Few weeks went by, no response. Reached out various times and finally heard back from a new office manager who told me she would look into the issue. 6) Several more weeks went by and still nothing. Left messages and emails and after about 4 months of this, was told I would need to speak to my insurance since they couldn't help me. 7) Due to the amount of time between having received the initial bill and finally being told I need to figure it out myself, I asked for them to please not send the bill to collections since I am very aware that medical offices will quickly take this route. 8) Was informed that they had already sent me to collections and there was nothing they could do about it and didn't even try to make it right by offering to refund my membership or anything else. 9) Had to pay the full bill through a collection agency which has now affect my credit all due to their staff being incompetent.

I was treated very rudely, which I asked her why she is being so mean. Her response was "I am the office manager did you say I am mean?" This was my 2nd visit here and the 1st visit speaking with the administrative staff at the front desk on my 1st appt was in the same manner. After this, I noticed all the signs they have that plaster the glass windows at the check in counter. One of which states that they will not tolerate disrespect, in so many words. . hmm. . In all the decades I've went to medical facilities I've never encountered such a sign. It's obvious the office manager and her staff want to be the ones to dish it out, but when you call them out on it they are not happy. Afterwhich and as I was in the room, room #10 (I believe), right across from the staff desk, I heard the office manager sharing my comment and she than returned a negative comment on what she should do, then as she was walking back into my appt. room her office staff start chanting, "do it, do it, do it" and when the door open they all shut up. Not sure I should have expected anything better from administrative staff, someday in their administrative career they will learn the consequences of their actions. I did record their actions on my phone and will be reporting this to the medical director AZ Pain. This woman should not be an office manager leading a team in her toxicity. I did report this to Dr. Dallas Smith, but the chanting part was after my appt with him and when she came back in the room. I would find it hard to believe that he did not hear the chanting, which makes me question his ethics as well. Reporting him would be the medical board, but administrative staff would be the director. On a side note and with regards to my appt. Dr. Smith advised having an MRI done prior to considering method of treatment, however, when I returned they were resistant on going over my MRI or even telling me if they had the results. They wanted to give an injection only, I asked don't we need to go over my MRI before deciding the method of treatment and that's when the office manager got RUDE. She said "we are already running behind because you were late." I advised I was not late, I arrived at 755Am for my 8AM appt with completed paperwork in hand, went to the restroom and checked in at 759AM. She said well we have to run your paperwork and that's when I asked "why are you being mean?" On a side note my paperwork was complete and it took them all of 10 seconds to scan my paperwork at the front desk and I had a seat. Learn some manners! You are in a medical profession and not everyone is going to be as kind and silent as I was after your little rant.

I spent an entire week trying to confirm my appointment through the automated phone system, but it repeatedly told me my appointment could not be confirmed. I called the office multiple times and was sent to voicemail without being able to reach anyone. The day before my appointment, I was offered a 10:00 AM appointment instead of my original 9:40 AM time due to "scheduling conflicts" by Yvette. I called back as quickly as possible while I was at work and left a voicemail confirming that I could make 10:00 AM but needed a call back to confirm. I also sent a text message stating that if I did not hear back after my voicemail and text message around noon, I would assume the other appointment had been rescheduled and that we were still set for my original 9:40 AM appointment. Instead of confirming either appointment, I later received a confirmation text stating that I had been rescheduled to a Wednesday--even though I had already explained that Wednesdays were not possible because of my work schedule. The following morning, I attempted to call as soon as the office opened and was sent directly to voicemail. I called two more times and was sent to voicemail both times. I ultimately had to drive to the office in person just to get an answer. I spoke with Rita, the office manager, who explained that the scheduling issue was related to a full-office meeting. I was given conflicting information regarding whether this meeting occurs biweekly or monthly, and it appeared that this time was not blocked off appropriately in the schedule. Rita repeatedly explained that there would not be a practitioner available until 10:00 AM and that I could keep the 10:00 AM appointment if I wanted. At that point, I was already extremely frustrated and in tears after spending so much time trying to resolve this situation. I explained that I could tentatively reschedule to another Tuesday while deciding whether I felt comfortable continuing with this clinic. I had also already reached out to my primary care physician regarding the situation. Rita then instructed Yvette to reschedule me for the following Tuesday. Yvette informed me that the next available Tuesday appointment was not until Mid-August. I had already taken unpaid time off work to make this appointment work, and this delay was the result of a scheduling and communication issue that I had repeatedly attempted to resolve. I understand that scheduling conflicts happen. My disappointment is not that my appointment needed to change--it is that repeated communication failures turned what should have been a simple reschedule into weeks of unnecessary stress and delayed care. Patients should be able to trust that when they return calls promptly, leave voicemails, and communicate their availability, those efforts will be acknowledged. How can you trust an office with your body or well-being when they cannot reliably answer a phone call, return a voicemail, or respond to a patient's message regarding an appointment? As a potential first-time patient, I no longer have confidence in this team.

    At first I believed this to be a great office. They got me in same day and the doctor met with me…read morepromptly. This is when things turned out to be horrible. I came in with a severe ear infection and met with Dr. Chapel for all of 5 minutes since he took a personal call while in the room and his phone kept going off. Then he told me he would send over an oral antibiotic and a ear drop antibiotic and I should feel better in 24 hours. His words exactly. I waited 4 hours to get the meds and trying to call into their office is a complete joke, their call center is completely rude and hung up on me multiple times and refused to help transfer me to the office just so I could ask when they were going to send my medication only for me to drive back to the office to finally get an answer. This continued for 3 days! I called the next day crying in absolute pain and finally got pain meds only for them to not work at all and today calling their on call doctor at 5am and not hearing back anything until I personally had to call to speak with his MA and was in such absolute pain that they were finally going to get in contact with the doctor. At this point I was so fed up I went to the ER and was immediately treated and cared for way better than this office even attempted. Their clinic manager, Sandy doesn't give two shits either and was almost blaming me for why my pain was so bad and making it out to be that I didn't follow my medications as directed. Absolutely ridiculous!!!! All I wanted was help and to get the adequate care I deserved and this place was a nightmare! Avoid it at all costs!!!!!!
