The young women here are friendly but unfortunately every single time I have requested a document, records release, even my referral here etc it has taken at least three tries/phone calls or office visits to have things taken care of properly. Due to misinformation, miscommunication, and simply a lack of communication and execution among the team has been the cause of this. I have been told my information has been lost, that my requests were never passed along from whomever I spoke to. I was never contacted to schedule an appointment when my referral was received, I was never contacted to receive the information they needed to get me started with appointments, it took over a month of waiting for the office to reach out to schedule, which they never did, only after myself reaching out multiple times was I able to finally get scheduled and continue care after temporarily relocating from another state. Now as I transfer back to my original state and needing records transferred I have been told three times now that I had filled out the form incorrectly, when in fact I followed the exact instructions of the office staff. It should not have to be my responsibility to continually call or check in to make sure things are being done properly. It's sad to say but I am grateful to be transferring to a different office to continue my prenatal care, I can't imagine the other mothers who may have had similar experiences or the many mothers who rely on translators and have an additional language barrier. I hope things turn around for this office

She was very warm and welcoming, a great start. Next I had the privilege of working with Christine who was the MA. What a great sense of humor she had and a gift at making people feel at ease. A true role model for customer-centered care!! Janelle, The office supervisor was the one that checked me out. I can see why the staff is so phenomenal, it starts with her and works its way across the office. She was phenomenal I wished I had enough euphemisms describe how great this practice is!! Janelle Has a strong attention to detail and is most customer centered. Yet another role model of what customer service it should be. Dr. Kafrouni was truly outstanding. He listened, asked open ended questions, he had a genuine interest in me as a patient. This is a rare find for a doctor; he was most approachable and was very thorough and his assessment. Later I found out that he is the hospital administrator, which even was more impressive because not only he was truly down to earth but he puts his patients first! That is a true role model as well. Overall I can honestly say I don't think I have ever felt comfortable in a medical practice to the extent of this one. Starting from the moment I walked in till I left, I felt valued as a person!!

    What a horrible experience. This practice isn't a pulmonary practice, it's more like a co-pay…read moregenerating scheme. Today is May 30th, still do not have the CPAP I was recommended for in my sleep study from April 24th that showed severe OSA. Was told I had to come back and see a respiratory therapist as they had to get prior approval. Wasted a co-pay for the doctor to stand on the other side of the room and read the words on the bottom of the sleep study report. My insurance does not require pre-auth and they never contacted them either, I called and verified. They knew I didn't need pre-auth because they gave me a nebulizer and billed for it without calling. My CPAP order could have been called in directly to my DME company who would have gladly fitted me for free and taught me how to use it instead of charging my insurance company for an unnecessary visit for him to read the report, another unnecessary visit on June 12th with a respiratory therapist and then a 3rd unnecessary visit with the doctor in July. Tried to get the CPAP order sent to my DME provider, received no answer. DME provider reached out, no answer. My chronic cough was never addressed. I had been told by a prior Pulmonologist that I would always sleep with oxygen, when I questioned Dr Kosinski if I would still need oxygen at night with the CPAP machine he said he could schedule another overnight oximetry test to see if I was ok without it. Seriously? He was just going to send me on my way with a CPAP and not check if that was enough? I have been on nighttime oxygen since 2016. Just cancelled my upcoming appointments (which took 19 minutes on hold before speaking to a live person) and have found a new pulmonologist. Quality of care=0

They want to process you in and out as quickly as possible, and bill for as much as possible. When you seek additional information for the PATIENT's benefit, all of these tools are useless (Phreesia.) I asked for a copy of my medical records and HCP notes - was given a phone number to call that led to a phone tree with a dead-end voicemail. The process for getting access to your own medical records, requires you to send a blind email to an obscure email address, then wait 15 business days for any response. You'll notice that Baptist uses an app to provide rapid access to the patient's information for all kinds of different resources. RCSM does not follow the same process. If your doctor refers you to RCSM, ask for another option.

She made me feel very welcomed and as if I was the most important person that she was going to see that day. Next I had the privilege of working with Quan. I cannot begin to express her customer centered approach to the testing modalities that she had to do with me. She was most patient, friendly, understanding, and was able to explain the process thoroughly and in a way that was not talking down to me but rather in a manner that I would be able to understand. I was most impressed with the care she displayed. Now to Dr. Katz. I do not think I have ever met a more compassionate doctor! He was extremely approachable, knowledgeable, patient, and truly demonstrated an invested interest in me as a patient. Not once did he look at his watch kept open body language and was able to answer questions. His office staff ran on time, clean, and practice universal precautions. I cannot recommend this practice enough, I wish I could gift 10 stars! Thank you Amy , Quan, and Dr. Katz!

I met with Dr. Katz and his team this morning to have allergy testing and to consult about whether or not I was a candidate for immunotherapy (allergy shots). I would HIGHLY recommend coming here if you are experiencing sinus issues. Dr. Katz was not only thorough but he listened intently, made eye contact throughout our conversation and explained every aspect of what they would be doing today with the allergy testing as well as explaining immunotherapy to me in a way that a patient can easily understand. He had absolutely superb bed side manner and was very easy going. He put me at ease with all my concerns and made excellent suggestions for me moving forward. His team was professional, patient and efficient. I know I will always be in good hands coming here!

They provided me will detail instructions from hygiene to parking.. so nice arrive I park in the ER parking lot which was free and entered through the ER building and took a left down a long hallway. When I can to the end of the hallway I made a left passing the SuBway that was on the right and the elevators were on the right also. I took the elevators to the 5th floor. I checked in at the Front desk and The asleep Lab technician gave me a tour and showed me to my suite. The room was extremely clean and prepped before I arrived. He explained the procedure very carefully and asked if I had any question and then connected the wires to me. He assisted me to the bed and did a test to make sure all the equipment was working correctly and it was. 500am promptly he came to disconnect the equipment. I took a quick survey and I left at 515am. Over all a great experience. They made me feel very comfortable. I recommend Ascension Medical Sleep Group for anyone who needs it. 10/10

I am so happy to find this clinic. Everyone is willing to help you. The medications they have on site are a huge help. The Dr.'s really listen to your needs and take their time with you. The first apt is long but it is because the Dr's sit down and listen to what you have to say and between the patient and Dr you come up with where to start and the best timeline for your needs. They have Primary doctors, nurse practitioners, Cardiologist, Pulmonologist, Podiatrist, Ophthalmologist, Urologist, Mental Health, Weight Management, and more! They even have a Pharmacy! Since all the doctors volunteer their time when they are not attending to their main office, appointments for specialists can fill up quickly. However, when you have an appointment for 1:00pm, you will be seen at 1:00pm! The appointments are very "on-time"! VIM is also an excellent place to volunteer and give back to your community! I have been volunteering in the front office for 6 months now and I absolutely love what I do. The Director of Medicine, Dr. Findley, is THE BEST! She's a leader who isn't afraid to "get her boots dirty"! Military and Police phrase. ‍ So if you know someone in the Jacksonville Fl area who has a job or a family member who has a job but cannot get health insurance, call 904-399-2766 and ask for the "Qualifying Department" to see if you qualify. Leave your name and contact number if you get a voicemail because they respond back to you fairly quickly! It doesn't take long to find out if you qualify. Your health is very important to you and to us! I will tell People especially my children, don't wait until something "feels" wrong, go get it checked out! Be proactive; not reactive. I always say, don't be ashamed of your current situation because it doesn't last and it doesn't define you. Use the services VIM HAS TO OFFER. You will immediately begin to feel that you belong, the doctors know me and they are looking out for my best interest at heart. We see each other as a family member who whom we care about. We have had several appointments at the Fleming Island location over the years, but they have recently moved their offices to Kingsley Ave in Orange Park. The entire staff - from the moment we walked in until the time we left - were professional, courteous and gave wonderful customer service every time. Dr. El Sabbagh and Tatiana Grant, his physician assistant, along with the entire staff were clear and thoughtful at explaining test results. We first started seeing Dr. El Sabbagh after my mom's stroke in September, 2022 after her stay in OPMC. Each visit, we truly felt listened to with every question He's incredibly caring and passionate about his work, and it definitely shows. It's easy to talk to him, and he has a fantastic way of listening, which is something rare to find in a cardiologist or any specialist. If you're considering a great cardiology group, considering getting the Watchman Implant, and/or need a doctor you can trust, I recommend Dr. El Sabbagh. First Coast Cardiovascular Institute has consistently provided great care through its staff and excellent doctors.

    I absolutely HATE writing this review but I fear if I don't someone may die. I LOVE my physicians…read more Dr Lamba and Dr El Sabbagh are top notch and I truly feel that they care about me. My issue is with the billing/office staff. I wound up needing a loop recorder due to unexplained bradycardia and syncope. The billing person gave me my quote which was rough but at no pint let me know that every month when they pulled the data it would be a $240 charge. That's a car payment that I was not prepared for. Patients should be aware of this be wise it may not be within their budget for that much monthly. Second, on my way home from getting my loop recorder I had a 5 min long syncopal episode and generally felt like crap the entire day. My husband are both ER RNs and VERY well versed with cardiology. We immediately called the office thinking this kind of event warranted an immediate pull and discussion with the doctor. It took 10 days and 8 phone calls to get a call back from an MA. The message never got passed on to my EP who at my follow up found I had 8 pauses greater than 13 seconds in the first 7 hours after having the loop recorder placed. It was a complete surprise to him despite us calling the clinic like a crazy ex. Most recently I called to have them pull data as I was feeling some serious symptoms. 9 unreturned phone calls and 9 unreturned voicemails (even to practice managers Angela and Michelle) I only received a callback from Sophie (only after reaching out online via the contact us) who heard me out and said she'd have it reviewed the next morning and call me that following morning. I work mid shifts and don't get off until the middle of the night. I woke up early to make sure I wouldn't miss her call. That phone call never came despite more episodes. This stupid loop recorder also only allows me to tag events 5x a day like it's the free version of chat gpt . No one here follows through with what they say they will. I only found out I'm continuing to have concerning amounts of pauses from a colleague of mine who works with FCC who called in to see what was happening. At this point I have no clue until I see my EP in a week. If I, a health literate healthcare professional myself is having these kind of issues I can only imagine your average layperson patient. I could make contact with aliens in outer space easier than getting a callback from the 'medical' staff here. This is a patient safety issue 100%. I am a 43 year old mother of 2 toddlers and this condition could literally result in sudden cardiac arrest and death yet these MAs dgaf to return a phone call nor report anything to the providers. My life might not mean a darned thing to anyone in this office but I can guarantee you I matter to my spouse and the 4 year old boy and 17 month old girl who call me momma. Do better.
