Any dentist who is trained and experienced, knows you can't give a patient a waiver form asking them to sign their life away if you stick the needle in a nerve and cause paralysis, or take out the wrong tooth, etc. DO NOT SIGN THE WAIVERS. My dentist of 35 years and their office have NEVER had anyone sign a waiver to avoid any responsibility for their actions while performing a procedure. Where is the confidence in your work? Are you that uneducated and experienced that you need to cover yourselves?
