Stephanie, Shannon, Katie ,Tonya and Lindsey are the best at what they do ! I've been a part of the community longer than anyone. I'm not taking anything away from anyone else commended for their dedication, efforts , work ethic , care and dignity that they give to each and every resident of Grace Point Place . After working with the staff at Grace Point Place, it is clear that they do everything they can to create a positive and caring environment. The staff seems to have a wonderful understanding on how to fulfill the needs of those struggling with dementia. Management has also been really responsive. The few times I've had some feedback and constructive criticism, they have listened to me, taken me seriously, and fixed the problem right away. Grace Point Place in Oak Lawn Il is an amazing care facility for your loved one! My mom went there after a fall and was treated so extremely well! My dad, her husband of 55 years, was allowed to stay in her private room with her 24/7 if they wished. The facility was very clean and the food was very much to their liking. Daily activities kept mom busy and the staff were very kind and attentive. As visitors we were always welcomed and greeted warmly. Mom flourished in physical therapy and was able to return home even earlier than expected. Shannon, Stephanie, and Jeen went above and beyond for us making a difficult situation so much easier! This place is wonderful. My grandmother has been here for about a month now and i am surprised with the quality of care these healthcare workers provide. They are always there when she needs something and its honestly all i could ask for. If youre having second thoughts, i understand because i did too but my experience here warmed my heart. Grace Point Place has been my 95 year old dad's home for over a year. My dad has dementia and I tried having him in my home, and neither of us was successful. The staff are always very friendly and are always happy to see my dad. My dad is very lively. I have seen the wonderful staff, sing dance and lately take him on outings. Latrice is in charge of resident outings. She has been gracious enough to allow my dad to participate in baseball outings and now takes him bowling. My dad was a great bowler. He may not remember that evening that he bowled in the morning, but in that moment, the joy was there. I am so grateful for these kind-hearted souls that have made this their life passion. My experience with Grace Point has been excellent. The quality of care is something that I really take seriously when it comes to my loved one living in a community like this. I am happy to say that the staff have been very friendly and wonderful to work with and have answered any questions I had regarding my family member's care. It can be a scary transition to trust your loved ones care to someone else but they have done everything they can to educate me and keep me informed of their care. We had what we consider as a family to be nothing short of a disastrous experience with Grace Point Place. We trusted them with the care of my grandmother, but after a few short weeks we realized that care was sorely lacking. Staff is few and far between, and we regularly found her in her room crying and soiled with no help in sight. A fall in the middle of the night left her laying on the ground for who knows how long until help came. Medications were not administered regularly. Looks are deceiving, always tour a facility at different times of day and try to talk to family members of residents if you can. After our grandmother begged us to take her away from there, we made the decision to bring her home. I would not recommend Grace Point Place to anyone whose loved ones cannot meet their activities of daily living on their own. read more

Hello Everyone!! I want to thank the great staff at Grace Point Place in Oak Lawn, Illinois for all of the hard work and care they put into creating a very memorable 100th birthday celebration for my very dear, elderly friend Irene. This is a serious milestone. Irene was happier and so animated yesterday, more than I have seen her in years. There are not enough words to say how much I love and appreciate her, so the above and beyond kindness that went into making Irene's birthday special was enormous to me. The facility and the grounds are very attractive, clean, and a very nice atmosphere. I was really impressed with how lovely everything is at Grace Point Place. Very friendly and helpful staff in all areas. If and when the time comes that I would need a care facility, I would definitely like to live in a place as nice as Grace Point Place in Oak Lawn, Illinois. Everyone is treated very well, with class, kindness, and some good humor which keeps the spirits of the residents uplifted. They decorated an entire section of the facility for her, her bedroom, and the dining room. The staff gave her gifts, and some cute little gifts for her guests. All of the powers that be showed up for her party including the Marketing Director, Activities Director, Director of the Facility, Her Public Guardian, and the Guardian's Boss, Maintenance staff, and Engineers all joined in the celebration, sang, danced and ate delicious food, cake, and some very tasty cider. The setup was beautiful with two gorgeous backdrops and lots of balloons and flowers. They played records and even hired a wonderful band, which was a great surprise. They showed up and showed out!! God love them for what they all did in support of Irene. I've never seen anything done up like that for a resident of a Senior Rehab Facility. God Bless Everyone and Their Big Hearts For Their Generosity and Kindness. Btw, the soul food luncheon was Delicious. Fried chicken, ham, macaroni and cheese, sweet potatos, cornbread, and some of the best collard greens I've had in a long time. Denise, the Head Cook and her culinary staff did a phenomenal job. I made sure that everyone knew how grateful I was to them. Irene showed so much love and appreciation to everyone for being there for her. I believe there were some people there from the news media, but there was so much going on that I didn't get a chance to check it out. Again, many thanks on behalf of Irene, my friend. February 12, 2023 On January 10, 2023, my dad has an unexplained acute health event which landed him in the hospital for two weeks. During that time many tests were done: CT, MRI, blood work, etc. All results came back normal. Dad was hallucinating, unable to feed himself and restrained some of the time. We were told that Dad was going to need a memory care facility. I had researched senior living places in the past. I called Grace Point Place in Oak Lawn and made an appointment with Shannon. She welcomed me and my husband on the weekend. She gave us a tour and we sat and talked for some time in her office. I was at ease. Shannon was very easy to work with. I filled out all the required paperwork. Shannon made sure a nurse would come to the hospital the next day to evaluate Dad in order to keep the process moving forward. The next day, I met Jean, the nurse from Grace Point Place, at the hospital. She did an evaluation on Dad. She concluded and I agreed that Grace Point Place would be a good fit for Dad. I liked Jean right from the beginning. Her demeanor and expertise put me at ease. She was knowledgeable and easy to talk to. Dad would be able to receive physical therapy and occupational therapy on site. We moved Dad in on January 24, 2023. His room was large enough to accommodate all of his necessities. I began to know the staff by name. There always were residents, staff and visitors around singing, talking or moving about. The environment is not dark and desolate. It is bright and full of life. Lisa, one of Dad's nurses and the other care givers are pleasant and friendly. I see them treat the residents with kindness. The entire staff is really remarkable. Lonnie helped Dad with his TV and his phone. Lonnie took the time to research the type of phone that Dad may find easier to use. He shared that information with me. Whoever is sitting at the front lobby knows me and is very friendly. I'm sorry that I don't know everyone's name because everyone I've encountered has been awesome. James, the physical therapist, is doing a great job with Dad. He really knows what Dad needs after laying in a hospital bed for two weeks. I am there when James is working with Dad. He has taught me what Dad needs to do in between his visits. Each visit James introduces new challenges for Dad to build up his strength. Nick is the Executive Director. He did not hesitate to meet and talk to our family. He told us about support meetings and activities. He told us to call any time we had a concern or question. I would see Nick at the front desk late at night. He would not hesitate to get involved when necessary. Grace Point Place has a human touch that I have not seen at any other facility. My dad has recovered from whatever had happened to him on January 10th. I will be moving him to a senior living facility as he doesn't need memory care. I will miss everyone I've interacted with at Grace Point Place. If Dad needs memory care in the future, I would not hesitate to call on them again. Do I have to give it any stars at all? This place is a mess. My grandma was there for less than 3 months and she fell at least 4x (as reported) but two of those sent her to the ER for stitches, CT scans, you name it. She looked like she'd been in a boxing ring. They have no idea how she fell, even though she was listed as a fall risk when we sent her there, paid nearly 6k per month for the highest level of care and still got basic nursing home care. We took her out after the 3rd month and only because we had to give a 30 day notice. They never said they couldn't handle her, just kept taking that check and failing us as a family and her as a client. False teeth broken after 50 years and they didn't even tell us. That just cost us a few thousand to replace. But that's not all...swollen feet and legs always, even though we told them to keep them elevated. Their answer was to keep putting knee socks (not compression socks) on her and shoes that were too small on her swollen feet (even though we bought her new ones). They do not listen to the family. The staff was inexperienced and the nursing care was nonexistent. She came home smelly, dirty, and unable to walk. Even her purse was nasty, with food inside, her nails were unkempt and she had a sock in her clean laundry that had apparently been washed and dried, but it had a banana in it! I'll post the pic. It took me a few minutes to figure out exactly what the hard black thing was inside her sock. She was still wearing an old hospital bracelet which had dead skin and dirt in it once we took it off of her. Adhesive from an EKG 3 weeks prior. Did I mention dirty and PUTRID smelling with spots on her skin all over that weren't there before. Even her belly button had some type of skin infection because the area around it was pink, like the skin had come off. Extremely dry skin. They do not bathe your loved ones, the food is old, cold, and sparse. After returning from one of her ER visits, they served her an orange, a cup of old mandarin oranges (yes, you read that right) cold eggs and a piece of cold toast with a cold pat of butter sitting on top. You guys just sent her to the ER--what a way to welcome her back. There's so much more, but I'll just end with the unmonitored pressure sore that they didn't even say anything about. It was deep into her flesh but we couldn't see it because it was covered with skin. It looked like a bruise to me and why not? She was covered in dark marks, dry skin, and scars from falling, so why not bruises? You'd think constant monitoring by medical staff would have seen it, right? You'd think they would let us know that something was going on, maybe advise us to follow up with her doctor? Nope. Not a word. Well, let's just say that she ended up in the hospital because once the skin fell off we realized it was an active sore, that she did NOT enter the facility with! It was extremely deep and spread quickly. She's in bad shape thanks to the negligence of this "Memory Care Facility" that charges upwards of $5000 a month for a high level of care that they don't provide. Some of these pictures were probably already posted by my Mom in her review but they are authentic. I can't even post the really bad ones--not here to gross anyone out, just giving an honest review of the facility. read more

My experience is much the same as the previous commenter. It looks great...but dig deeper. My mom fell numerous times within three months, twice sending her to the ER. Once I gave notice that it would be her last month I was hard pressed to get anyone to speak to me in a civil manner. It's mostly fluff-- The room wasn't kept clean, my Mom's dentures, which she's had for over 40 years were mysteriously broken and placed in a drawer (it wasn't reported to us, we just happened to find them because she was asking where they were), the food as I observed was sub par, staff was continuously turning over, every week there was someone saying that they'd just started a few days prior, if not that day, the RN's were registry, not Grace Point staff, communication was nearly non-existent, she was frequently wearing someone else's clothes, no one could ever (EVER!) tell me about her day, her legs and feet were always swollen (which they were not prior to going there) and like any other NURSING HOME, she stayed in a wheelchair, full of meds, all day long. Whatever I thought I was getting for $5500 per month, I did not get and while it smelled a lot better than a nursing home and there are fewer patients, the care was no better. I was actually told by an administrator that I may have to pay for overnight care for my mom because they couldn't provide one-on-one, however when I sent her there, I was assured that they were experienced in providing care for all stages of dementia and that they were confident that they could provide loving care for my Mom, keeping her safe and engaged and providing the best possible quality of life while she was there. I can't say enough about how disappointed I was in what they provided vs. what they said they would provide. The photos of my Mom's injuries are shocking and no one ever knew how she fell, when she fell, why she fell...these are just a few. Once I got her home I found bacterial skin infections, she was dirty and smelly, even her purse was covered in food and what appears to be snot. She was also still wearing a hospital bracelet from several weeks prior and the adhesive from an EKG was still on her skin. Overall, even though I contacted them numerous times, and even had a Zoom conference with them after she left, I haven't heard a word from them since, although they promised follow up. They don't care. They're short staffed and have a lot of empty beds. They are focused on taking your money, not providing quality services. I've incurred costs for ambulance services, hospital copays and a new set of dentures. A full week after writing this review, two weeks after conferring with corporate and over 3 weeks after bringing her home and I still haven't received the follow-up that I was promised. read more

I can't keep my mom here any longer. Care staff work hard, but only a few who seem to truly care. A lot of changes this year. Not all good. Food does not follow menu, and mom gets whatever they serve her. There isn't a choice of 2 or 3 entrees; just one. Grace point had a very skilled older Director of Nursing Kathy Bence that they let go after she reported the Executive Director Lorie Cantway and Maintenance Director Andreas Villareal were a couple and living together!! How did they even get hired?? Grace Point Corporate finally fired the couple, called the DON back, but couldn't get her back. Really a shame. She was making a difference w my mom, getting her medications straightened out, and communicating with our doctor to help mom. Nurses and caregivers now are back to old routines where they sit around, look at you sideways if you ask for help or have a question. A lot of attitude here, that has sadly returned. No one to talk to that makes a difference. So I am moving mom to Aspired. I hope I am making the right decision. I just know there are too many changes for the worse that are happening and don't feel like moms getting the care or oversight she was getting. If you're looking for a place for a loved one, you get what you pay for, within reason. Good luck! read more

My mother entered this facility happy and fully mobile. After six weeks she was miserable and unable to walk. Her medications were mismanaged and I had to practically beg them to give her the proper dosages. It took a full month for them to finally get her two pills administered correctly. It must not be a good place to work because they have a different nurse for practically every shift from an agency which leads to inconsistency in care. The care staff is usually seen sitting in the back of the room sleeping or staring at their phones. I had to change my moms bedding every day or she would have been sleeping in urine soaked sheets. Every few days I would have to take the soiled linens home and wash them myself or Grace Point would just let them pile up smelling and creating an insanitary environment (picture attached.) The residents are not properly supervised and are rarely engaged in an actual activity. The wheel chair residents stare at a TV all day in the common room and the mobile ones walk around aimlessly and without purpose. There are no breakout sessions or programs specifically designed for higher functioning residents. The Executive Director Dashira is just the absolute worst and doesn't care about the residents or their happiness. Since I removed my mother from this facility for all of the previously mentioned reasons, the management has failed to return all of my calls relating to documentation I've requested and other business matters. There is something seriously wrong with this place. I just hope the Anthem corporation intervenes because the residents and their families deserve more than this. read more

The staff are caring and friendly. Residents are treated with respect and it's a privilege getting to know the community and being a part of something greater than myself. The location is good and the directions for visiting are getting better due to covid regulations. Families are important and I'm glad to support families and hear the process in groups online or in person or by phone. read more

The new management team and new ED Laurie seems to know what they are doing. Finally hired new staff this facility was being taken over by agency employees that were rude and didn't seem to care about the residents. Love the new staff. Sheri in housekeeping is great! The new director of maintenance Andres is very helpful. They cleaned the carpets and moped the floors in the the facility. It was so nasty and dirty. The food is still just average kitchen staff very unapproachable and not friendly at all. I'm hoping that will improve. Activities is great Donna & Steve do a great job!. I hope they keep up the great work it doesn't seem like managers stay very long at least the smart ones that seem to know what they are doing. I hope the new ED stays around and continues to make improvements this facility desperately needed. They had a concert last weekend that was great. I brought my daughters and they painted pumpkins and ate hot dogs. It was well put together they seem to really care. read more
