DEATH TRAP!!!!!!! DO NOT SEND YOUR FAMILY HERE…read more.. I am writing to formally report what was, without question, the most disturbing, negligent, and dangerous healthcare experience my family has ever encountered. The care--or lack thereof--that my mother received at your facility directly resulted in a life-threatening medical emergency and could have cost her life. My mother was left with an indwelling catheter that appeared not to have been properly flushed, cleaned, or maintained for an extended period of time. The condition of this catheter was visibly unsanitary and showed clear signs of neglect. Within two days, my mother became septic. When I raised immediate concerns to the night nurse, Marcia, and explicitly stated that the catheter appeared infected and required urgent attention, my concerns were completely dismissed. I was told that my mother "just needs to drink more water." No assessment, no escalation, no intervention--just dismissal. At that point, my mother already had a UTI, which subsequently triggered a recurrence of MRSA that entered her bloodstream. As her condition rapidly deteriorated, I was forced to raise my voice and demand that an ambulance be called. Had I not advocated aggressively, I am convinced my mother would not be alive today. Once in the Emergency Room, the hospitalist physician stated plainly that bringing her in saved her life and that this situation "could have been deadly." The physician further advised me not to allow my mother to return to your facility if staff are unable to recognize such obvious and dangerous changes in a patient's condition, calling it a major red flag. My mother was admitted and remains on intensive antibiotic treatment at a different facility. She is now recovering from an infection that was entirely preventable. This is my second formal complaint regarding this facility. Additionally, after informing others that my mother was placed in your care, multiple individuals independently warned me not to allow her to stay there, citing repeated patterns of neglect and lack of concern for patient safety. That alone speaks volumes. This facility is not just negligent--it is dangerous. Based on my experience, it is a death trap for vulnerable patients. I have photographic evidence documenting my mother's infection and the condition of her catheter. While this platform does not allow uploads, I am prepared to provide this evidence immediately upon request. I expect this complaint to be taken seriously and investigated thoroughly. What happened to my mother is unacceptable, unethical, and a gross violation of basic standards of care. No family should ever have to fight this hard to keep their loved one alive. THIS PLACE NEEDS TO BE SHUT DOWN OR JUST GET RID OF THE STAFF! THIS PLACE DESERVES A ZERO STAR

Patients rights are completely unknown. The first floor seems to have nice people, night 2nd floor…read moreis a nightmare. Patient care is rude , disrespect and down rights cruet.personal privacy doesn't exist.' It's the coldest anywhere, not only temperature but in kindness, gentle handling patients are screamed at talk to they don't have a brain may have dementia but they do not deserve to be screamed like that or treated the way they are. I didn't happen to have either one. I just happen to have a UTI. I thought I would be there for three days, a week later I'm still here. I did complain on yelp how many changes were made at least on the first floor I have no idea whether the second floor made any changes but those nurses need to find another profession. They may have a tough job, but remember, they are the ones that chose it. they figured it would be an easy job and it isn't privacy is no question I go to the bathroom and the door are left open. Also, one of the physical therapist is very good. Her name is Larissa the rest of them questionable they do as little as they possibly can get away with it. They go through my things at night when I'm sleeping and remove my own private belongings then they tell me that that they can't give them back to me unless they list every thing that I have brought from home even though majority of everything I brought from home is an over-the-counter cream bomb devils claw for pain, stool softener, little things that I would be needing to take care of myself at home. The TV is so low that you can't understand what they're saying tiny close caption is so tiny. You can't read it so that's a waste of time altogether. This is against the law that they go through my private things. This is an invasion of privacy and I intend to do whatever I can about it because this is the wrong way to treat anybody no matter who they are no matter what their mental condition or the physical condition they do not have the right to be disrespected and be treated like that. they didn't mean anything unless they made a noise. I happen to be one that's able to make a noise and I got some attention. They will be happy to see me leave. I'm sure but I'm not finished because invasion of my privacy and taking my medication and my things that belong to me and we're harming nobody, I wasn't taking them, I only carry them all the time with me because I never know when I'm going to need them in public. I checked the law and it is against the law so I fully intend to do what I can about it. I had a meeting today with the board apparently that would decide whether I wenthome and they decided that I'm not going home because of the medication things that they found in my pocketbook. Well, we're gonna find one way to get out of here and they will pay for it and it might cost them a lot of money in fines, which is exactly what they deserve. Some of the employees are really wonderful. I can say that Neville, who the pharmacist at night is really a gentleman and keep his word he gave me my medicine on time. Most of them come in, They tell me they don't even know who I am where did that come from? They never heard of me. The assistant administrator apparently came by to see me after the meeting today, to really give me a chewing out about the lotions that I have to rub all my torn shoulder to relieve pain. Now she wants listed every single thing that I take that helps me with pain and to get through life without prescriptions that is completely unnecessary. She does not have to have that and she needs to read the privacy reports again there are no danger to anybody, including myself. This has been an experience. I wish onnobody, but the reason I may be here was because so many of these patients have absolutely no voice they do not know that they can complain to anybody where they even have a right, and if they complain, they'll just simply be labeled as demented ,what they're doing that is unfair. Do not choose a place like this with your loved one because this is one of the worst places you could be to go to die this is certainly a place that I will either die in the place or die trying to get out because it looks like I'm not going to get out anytime soon not unless I complain enough and cause enough problem and they can brace themselves because they never saw a fighter like a lion like me . the state will be in here on them with fines if they don't get their act together real fast and it might happen before I'm out of here Dietitian has worked very hard and understands my condition. He's probably one of the few very capable people a lot of smiling people all promising all kinds of wonderful thing don't believe a word of it.

From my observation, the staff members are predominantly Haitians and most of them, from my perspective, lack interpersonal and communication skills. There was one nurse who was nice. I want to convey my appreciation to that particular nurse. This is what transpired to the best of my recollection. I was accosted by two staff members. They were questioning my job description. They wanted me to do their jobs and I declined. They wanted me to perform their duties which they probably believe was beneath them. The client is paying for services rendered and I believe the facility's staff members are the ones responsible for her care during her hospitalization. I don't mind assisting; however, I will not lift nor will I use my body as a Hoyer Lift. I made that quite evident to my employer that I cannot lift. I accepted the case because the client was ambulating and she didn't require toileting assistance. Basically, from My perspective, I was more of a companion. I made sure she eats, take her meds as scheduled among other duties. The family at that time, have never had anything negative to say about me or my performance when the client was being cared for in the comfort of her home. When I was at the aforementioned facility, I was minding my own business and one of the aides, walked up to me and she said, the other lady who worked yesterday, she did everything for the patient; we did not have to do anything. I don't believe the staff members should have accosted me. Additionally, I called for assistance when the client had to go to the bathroom. It took what seems like eternity for a response. Their response time is terrible. It took approximately 30 minutes for the assigned staff member to respond. I believe they are short staffed and I recommend a floater to help the staff as needed. There are times when the client defecate and urinate in her pants because no one acknowledged the call light in a timely manner. Are these people volunteering their time or are they being compensated? I informed the RN, Charge Nurse, that I would prefer not to be interrogated or harassed by members of her staff. I informed her if she has any issues with me, I would prefer for her to contact my employer as opposed to confronting me. She was disrespectful, boisterous, unprofessional and uncouth. The Charge Nurse came in the client's room while I was using my phone. I told my client I had to call my bank and she granted me permission to make the call. On her way out, the RN sarcastically stated, " You need to watch your patient." I went by the Nursing Station and she was not at the station. I saw her conversing with someone in the break room. I politely asked if I may speak with her. " What do you want?" I said, May I speak with you? I said to her respectfully, do you have something to say to me? I hope they have camera. She started swinging her hands and she got extremely boisterous. " What is your problem?" If I have something to say to you, I would say it." I called and reported the incident to the Director of Nursing. I told her I will, if necessary, climb the chain of command. Apparently that bothered her. She called me back to tell me the facility is a non profit and she does not want me back. She told me she spoke with the client's children and she does not think I am a good fit for the client. I helped this client in her own home for approximately eight months. Tables do turn and everything in life is transitory. The Director of Nursing has an Haitian accent as well and I believe she is upset that I reported her unprofessional compatriots. In conclusion, I want them to know not to make assumptions. I took early retirement and I choose to work part time with the agency. I have three College Degrees and the last one I got is from Nova Southeastern University. I am a retired teacher and private investigator. I am an educated woman who took early retirement due to my current health issue. The Moral of the story is, " Do not judge a book by its cover." We have to treat people how we want to be treated. Lastly, be careful with power. Power is a dangerous weapon. Use power carefully. God is the true power. I will continue to excel because I serve a God that will never abandon me. In conclusion, What they meant for evil, God is going to turn it around for good.

I understand that no facility is perfect, and some hiccups are to be expected, but a few issues made her stay more challenging than it needed to be. Unfortunately, her dentures were lost on the first night, which created a significant hardship. Without teeth, she was unable to eat most meals, and the puréed food provided was thick and honestly unappetizing. I ended up bringing her soup each evening so she could eat comfortably. What stood out most was the clear difference between what felt like an "A team" and a "B team." When V, Jennifer, and Mark were on duty, my mother was well cared for--knowledgeable, attentive, and personable. Other staff members seemed less experienced and less warm. The facility is clearly understaffed; while some aides were wonderful, others were not particularly kind. Cora was an absolute doll, but she had far too many patients and not enough support. Teri, the concierge, was truly five-star. Any issue we raised was addressed promptly and professionally. The rehab team--both PT and OT--were amazing and are the primary reason we chose this facility in the first place. They exceeded expectations. Areas for improvement include staffing levels, consistency of care, cleanliness, and food quality, which was mediocre overall. With more support and better oversight, this facility has the potential to be much better, especially given how strong some members of the team truly are.

Clearly Federated did not invest in a luxury designer. They hired a budget conscience decorator or service that found the cheapest solutions. They just completely re-carpeted the entire common areas with the busiest, cheapest looking mish mosh of vomit carpeting that I have never seen the likes of in a luxury setting. Not only is it extremely disorienting and distracting to people already feeling unsteady, but none of it matches from one room to the next, no cohesiveness and zero design or style. My poor mother gets dizzy trying to walk on it, other residents have mentioned they can not roll their chairs or walkers smoothly on the extra plush textured carpeting. The common conversation amongst the residents is the incredibly tasteless design decisions made for this reno, this has upset everyone in the building! I refer to it as vomit carpet because one would not notice if someone was to actually vomit on it. It would just blend in. Assuming they wanted something that would not show dirt - they succeeded because it is so incredibly busy and distracting you would not notice anything spilled or stuck in it. Federated took all the class and luxury out of this gem and made it look like public housing. For what the resident's pay to live there, they should have invested in a classic style and not gone so cheap, modern and extreme to have mismatched patchworks of various busy carpet designs randomly placed every inch of the facility. If we had toured it today my mom would never had agreed to move there. Sinai Residences is no longer dressed to impress. Classic, stylish, and luxurious has been replaced with common, cheap and institutionalized.
