This cost her much pain and led her to cry and scream, which the staff dealt with in the most humane and compassionate manner. They spoke softly, encouragingly, and continue to let her know that they were doing the best they could to help her without causing her any pain. She is always clean, dressed and clean clothes, and in a good mood when I come to visit. The staff to patient ratio seems extremely high, and the executive director is very approachable. I would refer my other clients here in this part of DFW without thinking twice. What a wonderful secret.

It is clean and cheerfully decorated with comfortable rooms. Family members are welcome to stay with patients overnight, which was perfect in our situation. The staff, from administration to housekeeping, is friendly and caring. You can see everyone is happy to be here, and they provide the most compassionate care in everything they do. The physical and occupational therapists are so encouraging and patient. There are daily, FUN activities for the residents, and the participation is high. The atmosphere is a happy one! If I ever need rehabilitation (knock on wood!), I will insist on coming here.

I was reminded of Parkwood while talking with my mom's home health nurse. She has a new patient in Parkwood. I was interested in reading reviews to see if anything had changed. Doesn't look like it. Whatever you do, do NOT place your loved one in their memory care. Take a look at it and you'll know why. Four years ago, we were all still dealing with problems resulting from Covid. I tried to explain the issues we had at Parkwood on that. Primarily, the caregivers were very inattentive. My parents were in Parkwood for six weeks rehab. One day I found my mother lying across the bed with her pants down below her hips. An aide had taken her to the bathroom and left her like that when she mentioned that at home her husband normally helped her with that. The aide said ok and left her. I was more than happy to get my parents out when their rehab was over. I don't know if the director is the same person, but the director at that time was totally indifferent and did nothing about the issues I brought to his attention. Only unpleasant memories of Parkwood. I do not recommend it.

    First note that there are two sides to this facility, the Rehabilitation and Physical Therapy side…read moreand the Long-Term Care Nursing Home side. The four higher ratings appear to be from the physical therapy and Rehabilitation side, I do agree that the physical therapy and Rehabilitation at this facility is generally good however the long-term care side is what we are reviewing and this has been our experience . My mother lived at this facility for over two years. While daytime care was generally adequate, nights and weekends were consistently poor and neglectful. For over a year, I raised concerns specifically about evening and weekend care, yet these issues went unaddressed and showed no meaningful improvement. There were multiple instances--especially on weekends--where my mother was left unchanged for extended periods. She often had to call me so I could contact staff to get basic care addressed. My mother frequently complained that her call light would go unanswered, leaving her unassisted in her room despite being bedbound. On several occasions when I came to help her myself, I observed CNAs at the nurse's station playing on their phones instead of responding to residents, or no one at the nurses station at all for extended periods of time. The food quality and variety were also concerning, with the same meals repeated several days in a row. Communication was one of the biggest failures. In two years, I was never able to speak directly with her physician despite multiple requests. I was never even given the doctor's name, and staff could not provide clear answers. This was especially troubling since I needed to be involved in her care. There were repeated hospitalizations that appeared preventable with proper monitoring and timely care. She was sent to the hospital multiple times for infections, including severe UTIs and pneumonia. In one case, an ER physician described her UTI as the worst he had seen in his career. In 2026 alone, she was hospitalized four times in four months. The facility was also reluctant to arrange hospital transport when needed. At times, my mother had to call 911 herself, or I had to intervene and insist on transport. During these critical situations, I was not kept informed of lab or X-ray results. When I raised concerns and asked what steps would be taken to prevent future incidents, I was told that if I was unhappy, I could move her. After that, repeated attempts to contact leadership--including the Director of Nursing and administrator--went unanswered. We ultimately moved her. No one from the facility has followed up to ask why. The building may look nice, but the level of care, communication, and responsiveness was deeply concerning. I cannot recommend this facility.

She was two week in intensive care, a week in a regular room and the released to start her treatment at a rehab. I had no idea which rehab to use, and asked for a recommendation, I was recommend Encompass Health, out of many options. My daughter read the reviews and there were a variety of mixed comments, this is mine! From the moment she was moved from the hospital to Encompass, treatment as a patient was very human and very respectful. Once in the rehab, the treatment was nothing short of perfect! The nurses and techs were all very very nice people who obviously take their profession and responsibilities very serious. The director for Encompass himself came to introduce himself and dropped his card in case we needed to contact him for any question or discrepancy, which we never had! The rooms were very nice, clean and kept that way. The therapist were just great, my wife's recovery has been nothing short of miraculous, God in place and the work done the Encompass team and therapist was the best combination to her recovery. The Doctors were constantly visiting and giving updates of my wife's condition. She was offered daily a menu for next days breakfast, lunch and dinner, and the food was just great! She entered the rehab April 6th and was discharged Sunday April 19th, walking, talking and on her way to being independent as she was before the stroke! It is very difficult to see someone you love and have know all your life, suddenly helpless and wondering what the future hold for that person and the family as well! Bear in mind that in this rehab there and many many patients, some in very delicate conditions, so we where not expecting that in a case of some need they staff could react immediately, they did the best at hand and that is accounted for! Thank you to the Encompass Staff on the second floor, thank to your for al you did for my wife recovery, God Bless!!

    Avoid Westpark Rehabilitation and Living. Our experience with this facility was unacceptable on…read moreevery level. The building is run down--peeling paint, holes in the walls, missing electrical faceplates, loose outlets, and damaged furniture. It looked neglected and unsafe. The staff was not attentive to our loved one's needs. One night he waited an hour for help to use the restroom. No one at the facility would answer the phone when we tried to call. I only reached someone because I had the Executive Director's cell number. Communication is almost nonexistent. They do not answer calls, rarely respond to emails, and repeatedly failed to provide basic information we requested. Even the external hospice staff caring for him was ignored. We also have serious concerns about the quality of medical care he received. While he was there, he had a ulcer on his foot that the staff repeatedly told us was "getting better." He is now in the hospital with a bone infection (Osteomyelitis) that required surgery. The surgeon told us there was a great chance he could lose his foot, but fortunately they were able to save it. This raises real questions about how closely the medical staff at Westpark were monitoring him--or if they were monitoring him at all. This facility failed our family, and I would never recommend it to anyone.

We had a wonderful experience from beginning to end. Their staff is polite, thorough, and thoughtful. I got to interact with their awesome intake/dispatch personnel as they collected key information relevant to the trip details. At the time of writing this review, we are experiencing the COVID-19 pandemic. I was happy to hear they were asking question to attempt to limit exposure. It let me know they care to protect their clients and staff. Drivers were very patient, careful, friendly, and respectful. They took care to ensure my grandmother was as comfortable as possible when they transferred her to the stretcher. They also drive very safely, attempting to minimize the impact of sudden stops, bumps, and potholes on the road as much as possible. I would definitely use their service again when the need arises, and I recommend their services to anyone who needs it. They serve a unique population which has limited transportation options in the DFW area. They provide a variety of options depending on the mobility impairment of the person. I know they provide transportation via wheelchair, stretcher and ambulatory people as well. Their drivers are well trained and specialize in what they do best: getting your loved one to their destination safely. If you have an elderly or disabled person who needs transportation, give Chisholm Trail Transportation a call. I am sure they will do their best to address you need. We brought her home that weekend and the loving and caring staff not only got her comfortable but more importantly, she had a series of falls the last few months before this hospital visit, so she had not walked for almost 8 months. She was started on some muscle strengthening exercises, given some tender love and care, and not only did she make it through the weekend, but she walked about a month later for the first time in a long time, and lived almost 7 months after receiving such initial devastating news. We collectively know that it was Accord Palliative and Hospice Care that went above and beyond for our family. Thank you guys so much for caring for us in our time of need. -Davis Family
