This is an amazing facility. I was there for rehab of a hip replacement and fall after surgery. The OT's and PT's did an amazing job in getting me up and running after my injury. The nursing staff and caregivers held the high degree of efficiency and care. The administration staff did a great job coordinating my rehabilitation. I would highly recommend this facility. I would certainly give it five stars plus. read more

My father was sent here to recover after back surgery, it was a horrible experience. He had to share a room with another man who was up all hours of the night, who repeatedly soiled himself, left his soiled diaper on the shared bathroom floor. Breakfast was served by way of slamming down the tray next to my sound asleep father. Anyone with a modicum of medical knowledge KNOWS how important restorative sleep is for recovery. This place also has harsh lighting, not conducive to recovery as well. Do not send your loved one here!!! read more

The quality of care my mom has received during her short-term stay (less than a month) has been very appreciated/heartwarming. Being in her 80s, my brother and I both strive to visit as much as possible. While we witness firsthand the attention and care that she receives, we are equally impressed to observe how the other patients are cared for who do not have family/visitors present. Rehabilitation is key to one's health at an advanced age and the number of therapists on site is truly impressive. Truly wish I knew the names of the many therapists, but the smiles toward the patients/residents are reassuring. The camaraderie among the therapists is also encouraging; they appear to take great pride in their endeavor and work well together. As far as those working the front and administering care, medications, comfort and such, there are several talented staff always at the ready. Among the MANY positive members of their care team, individuals such as Jesus, Yancy, Espi, Victor and Valentina have made a HUGE difference in my mom's positive experience during her stay. There are several others (Harold, Myrna and Allan, to name a few) who provide great contributions as well. Thank you so much to the staff for all that you do in caring for the patients! Food was horrible. No doctors on site . Nurses, physical therapist,janitors like peeking at you when you're going to the bathroom "creepy" . Place is crawling with roaches and mosquitos. Nurses wake you up every 4 hrs night and day . You're better off at home. This place was insane! 90% off the workers hated their job . Understaffed. Workers never opened the door for me when I was in my wheelchair. Would not recommend this place. read more

Someone is skimming money out of the food fund. My mother stayed there recently and I am appalled at the quality of the food. They're supposed too be making people better not worse. If you are here for PT/OT you are at the right place. The staff is upbeat, friendly, supportive and professional. The staff members that worked with me personally were Daisy, Kathy, Peter, Cristelli, Daryl and Pablo aka 'Mr. fixer.' Although I did not work with Laramie, she was always happy and supportive to see me. I came to this place not able to walk and was able to walk out of it. Of course the meds were a big part of my recovery too. All the CNAs were great. Keep in mind they are human beings like us so they have their moments too. The ones I especially enjoyed taking care of me were Rosario, Augie,( Augie is the best. He makes your bed all up in the middle of the night while you are on the commode. He works the graveyard shift.) Tina, Marieta, Agnes, Noe, Steven and Alyssa. At the beginning when I needed assistance to get out of my bed to the commode the longest I waited was 20 minutes. So my suggestion is as soon as you feel like you need to go press the call light. Their regular nurses who took care of my daily meds were wonderful. I felt the safest with Norma, Allen, Emely, Able and Destiny. The housekeepers work hard in keeping this place clean. I only met Christina and a gentleman who I never got his name. He's bald so you can't miss him. The facility itself is definitely old and needs a make over in my opinion. I especially DO NOT like the Jill & Jill restroom. That is 4 women sharing one commode. We asked if we can have a private room even if we had to pay the difference. They said there is a wait list. Which I do not think it's true. Because we were able to roll around in my wheelchair and check out the facility. However I was also told that the insurance doesn't allow for you to pay the difference to get a private room. The activities department is very active in getting patients who want to participate in whatever that is going on that day. Larea (I'm pretty sure I misspelled her name) is super energetic and funny. Rae is quieter but always there to help you if you need her. She also has a real cute dog she brings by to cheer up the patients. I saved the worst for last which is the food. We were told this facility shares the kitchen with the residents in the other buildings. If you like your garlic bread, toast hard as a rock then you found the right place. There are stuff they serve that is good. It's a hit or miss thing. However I tried to eat what I can because there are so many people in our country and the rest of the world who do not have enough food or any to eat. Just have you loved ones bring food to you from the outside like my Husband and Sister did. Bottom line is I was not there for the food, I was there for the rehab. There were things that I didn't like about the place but they were so small compared to the greater good of the place. Like one day I didn't get a shower because the CNA that was assigned to me didn't know she was my CNA, but I got my shower the next day. Or that I was going to get PT on Saturday but turned out my insurance only covers 5 days. All and all it's a very good place to rehab, get better and go home. read more

I'm beyond grateful for the staffs and services that Mission Care Center provided for my dad. He suffered a stroke in February 2024 and stayed here until his discharge in October. Everyone from CNA, RN/LVN, PT, OT, Case Management, to the custodial staffs were nothing but angels. Betsy and Diana, both CNAs are tentative even on their busiest days; no patients left behind they'd say. RN's Victor and Allen are helpful with any questions you may have. They're on top of things if any incidents happened. Their physical and occupational therapy team are miracle workers. My dad was unable to walk when he was admitted to Mission Care. Today he's walking with a Hemi walker and a little assistance. Charlene, Daisy, Kathy, Laramie, Peter, Pedro, and Pat at the therapy "gym" are passionate about their jobs and I can see the care they put into each patient's progress. "Gabby" Gabriela, my dad's social worker, had helped us so much behind the scene from the day he was admitted to his discharge date. She always kept us up to date with his progress. All in all Mission Care Center staff are passionate, professional, and caring. They know what they do and they love what they do. I appreciate you all. read more

I will try to be as objective in this particular review as possible, because it is my only observation of a facility like this. My father was a resident for four 1/2 weeks at Mission Care Center in Rosemead. I waited until we were able to remove my father into a different facility to write this review for his protection. My father was placed here by Scan and Optum Insurance who told us this was a "Five Star Facility" The night my father arrived, everyone I encountered from the facility said the same thing, "This is a five star facility", which seemed very rehearsed and unusual. The first night, my father was placed into a room with another male who was in bed calling for the nurses who were in the room. He kept calling them and they ignored him. That was concerning. My brother and I came to the facility everyday, seven days a week, to make sure my father whose speech was slurred, right arm and leg paralyzed received the help he needed. We were there up to five hours everyday. At least six or more times, my fathers oxygen was unplugged when we arrived, where it connected to the machine. My father never tried to remove the hose, and he couldn't if he wanted to because his left arm and hand is weak. When we asked the nurses about it, they just said, "I don't know". Every other day, we had to do a covid-19 test before entering, in addition to showing our vaccination cards the first time. But the employees at Mission Care were only being tested twice a week. Only one of us were allowed in the room at a time. Three covid-19 outbreaks occurred during the month (June 2022) each of them were from employees. Two out breaks spread to patients. During the second outbreak the state visited, suddenly there were tons of staff everywhere, all of them wearing plastic gowns, masks, gloves, all of the office and administration staff dressed in nice clothes, everyone following strict rules. Two days later everyone was back to wearing their normal nursing cloths and mask only. On weekends and days when the administrative staff were off, the nurses and other staff were slow to answer calls, they seemed to be very short staffed, they didn't make visitors wear the plastic gowns, gloves or N95 Masks, and they allowed multiple visitors in the same room with double occupancy patients. My father began complaining about his butt hurting. So everyday I asked the nurses about it, They told me he had no bed sores. The last weekend my dad was here, they were extremely short staffed so the person helping change and reposition my father didn't make me leave the room, so I could assist. My father had a six inch patch above his butt. I asked the staff who only works on weekends to look under it. There was a bed sore. Two days latter at the new place, they looked and he had two bed sores where my father had been hurting, under the patch. The last Friday my father was here, I never saw anyone come to feed him. I arrived early on purpose that day. He had been switched from a feeding machine to hand feeding via his stomach tube a week earlier. I told my brother who switched places with me, to make sure he got fed before he left at 5pm. He asked the nurse. She was confused about his feeding schedule, and admitted that he hadn't been fed anything since 9am. He was only getting 1.6 litter at a time, supposed to be every five hours round the clock. No one ever suctioned his mouth while he was there. Every morning he had strange brown stuff all over his teeth and in his mouth. He wasn't taking anything oral. We had to clean it. His speech therapist had to spend much of the time cleaning his mouth out, before she could do therapy. We told the nurses multiple times and they were clueless, didn't bother to investigate. The overseeing doctor never updated us or met with us in a month. We had to reach out to her after a concern. She told us a negative view about his recovery progression, 100% different than the nurses, therapist or what we saw being their every day. She said he wasn't cognitive, couldn't communicate. My dad was talkative all day, fully cognitive. We reached out to her one other time. One time she was in the building and she wouldn't come to the room to talk to us. Had to talk on the phone. The first time we confronted the doctor about the difference in what she said she observed, she had a meeting the next day with the nurses. Suddenly they all repeated what she said. (Except one or two who were transparent and honest the whole time) One nurse put her hand in my brothers face and told him to talk to the hand, when he asked to take my father outside in a wheel chair. She said she had just put him down 15 minutes ago. My brother had been there for over an hour before he asked. The speech therapist, Sue was always a welcome sight. She really treated my father well and reported issues that concerned her. There were other staff who cared too. My dad said, "Get me out of here". read more

One start is the minimum I can give them, they deserve a negative 10 stars, this place have no professional service, there nurses in the front desk are rude with attitude, any little thing gets them ups set specially nurse Vanessa or receptionist. I already posted this review and it was removed. Just one more reason to HATE this place and the people that work there. My elderly father in law was sent to this facility a few months ago (out of our hands) and said the facility was nice at first, most of the nurses were very kind... UNTIL he was physically assaulted by a night nurse. This "care center" failed to contact us about said abuse. My husband and I happened to show up for a visit at the same time the sheriffs arrived to report it. I overheard the man in charge discrediting my father in law and saying that he was confused and had no idea what he was talking about. That is a tun of B.S! His story remained the same about this abuse until he passed away last month - 6 WEEKS after the nurse physically assaulted him. "Mission care center" threw him out the same day the police report was made, stating that "his health had improved and he no longer requires service of this facility." They knew he didn't have a safe place to live, could not walk or care for himself, and had 15 diagnoses' on his chart (that weren't even counting his 3 chronic heart conditions that had him there in the first place)! They didn't contact us to let us know they were sending him home, and when they got him in his apartment he fell down and wasn't able to call for help. After finding out he was no longer at this facility my husband went to check on his father to find him crying on the floor, unable to get up. The next day, my father in law wanted to go back to the facility because he knew he needed round the clock care and no one explained to him why he was being sent home. He didn't blame the facility for the abuse, only the nurse that did it to him, so he still wanted to stay there. Gabby, very unprofessionally and rudely made it very clear that my father in law was NOT welcome. His spirit broke right in front of me as we desperately had to find him somewhere else to go for help and the only place that could take him other than mission "care" facility was an hour away. So much for "residents rights" and "respect and dignity." This place needs to be shut down! Their mission statement is bulls***, and they don't practice what they preach. I feel bad for the poor, defenseless people that come through those doors, because they are going to be neglected, abused and lied to throughout their entire stay. If anyone is reading this in the hopes of finding a care center for yourself or anyone you love or care about then STAY AWAY FROM THIS PLACE! I have documentation proving everything I wrote and the packet "Mission care center" wanted us to sign that we refused. They should all be ashamed of how they treated a dying man, and cover up for someone who should be in prison! I hope and pray that nurse gets hers! read more

My mother was sent here following a hospitalization to recover. At the time I really liked the facility. My mother was especially happy with the meals they provided and selection. However she is hospitalized currently and needs to go to a rehab facility to regain her mobility, and they refused her referral based on "behavior". I spoke with the case manager because I didn't remember any behavior issues, and was confused by their refusal. When I spoke with them, their objection was that she "liked to walk around". I am a puzzled by this because at this time she isn't really mobile so that wouldn't be a problem. Additionally, isn't that the goal of a physical rehabilitation facility? I didn't think of them as a place that wanted people to stay in their beds all the time when she was there, but I guess I didn't realize that was the preference. I know that "covid" has been the basis for all sorts of changes in how patients are cared for, and how much interaction the family is allowed with their loved ones... but if the expectation is that because if covid a patient has to stay in their bed all the time, it seems a little too convenient for the facility, and not so beneficial for the patient. After all the goal of a rehab facility is for a patient to regain their mobility... and it's hard to do if you are expected to remain in your bed at all times unless you are having PT. I am sad about this, as I said I had a positive impression of the facility at the time she was there. I guess I misread the situation. Probably a good place to go for rehab, as long as rehabilitation is not the goal. They prefer patients that lie in bed all the time! read more

My mother was sent here following a hospitalization. Mission Care Center is clean and well staffed. The staff members are friendly and helpful. Food is quite good for "institutional" food. Family care conference was very comprehensive and I felt that the entire team is working really hard to provide my mom with all the that she needs to be able to return home in the best possible condition. I like the seamless coordination with her HMO facility physician. We feel blessed that they had a bed when she needed it. read more

My mom will be here for 2 weeks so I'll be there a few times. Oh, the admissions girl actually remembered who I was which was impressive because of all the patients and we just checked in. One of the Worst! My mother was admitted on Thursday night Sept 30th, for Short Term Rehab, following a major Surg she had a City of Hope in Duarte. We looked for a positive facility, good reports, no violations etc...They began to evaluate her on Friday Oct 1st, and apparently it took Fri, the weekend and the entire next week to determine what course of rehab treatment they would plan, they Never came up with a plan Nor did she ever see the doctor there. They started No therapy in the 10 days she was there. However they did leave her yelling in pain for 3 hours before someone came in to help her. She had urinated and the nurse actually told her oh go 3 more times then I will change you. This is completely UNACCEPTABLE for any place! She was very dehydrated as well. They would bring a tray of food in but leave it and not make sure she was eating.. When she left to keep her appoint with her doctor at City of Hope, she had no instructions, nthinh! What kind of people are they that they think this is ok? They did not move her positions around to prevent any bedsores. I am so angry and disgusted with how this place is still in business, I just can't believe it. They found out how many days they would get paid for and just planned on billing away until they maxed it out. Definitely Do Not Recommend read more

My grandma was in-and-out of this nursing home a few times these past few months after she suffered from a stroke. The first time she was admitted into the home the process was very smooth and the staff was extremely professional. I am young so I do not have a lot of experience handling paperwork and unfortunately, my parents are not local, so I had to do it myself. The person going over the paperwork with me was extremely patient and explained everything to me clearly. The overall vibe and first impression I got of the place was that they are a very active facility that is eager to work on getting their patients healed and back to their homes. I was hopeful that the therapy would be great for my grandma. In the short amount of time she was there the first time she was admitted into the home, I felt like all the nurses and therapists were very attentive. I appreciated that they were patient in trying to understand my grandma despite the fact that she was unable to verbally communicate a lot due to the stroke. Although it is a very busy institution just like any other care center, and at times it would take some time for a staff member to answer the call light, someone always did and the staff generally exhibit a positive and helpful attitude. Due to many circumstances, my grandma ended up having to go back to the hospital and back to a different nursing home. Because of our family's frustration with the other home, we wanted to bring her back to the Mission Care Center and waited for a bed to become available. Unfortunately, the second time around my grandma's condition was not as good and she was communicating less and less. Because of my grandma's age, the administrators of the center bluntly told my parents that they did not think my grandma had a good chance of improving much and did not feel that continued therapy was worth it. While the delivery could have been better, I understand their thought process; many people end up dumping their family members in these care homes without ever visiting them or checking up on their progress. Perhaps this home thought we were going to do the same because my parents are not local and my grandma is 90-years-old. However, we explained and could prove that my grandma has a hired caretaker that comes five days a week, along with several other family members regularly visiting throughout the week. My grandma ended up staying there maybe a few weeks but was ultimately told that therapy would no longer help her condition. Although my family and I have accepted my grandma's condition and understand she will most likely not be back to her pre-stroke self, we are dedicated to giving her the care she deserves as she is a human being that deserves respect. My grandma's condition is stable and we will do the best that we can to keep her comfortable. It is a shame that the Mission Care Center was not willing to accommodate her, but perhaps it is because they are more of a home that is interested in getting people in and out. After all, you'll notice several of their trophies and awards decorating the front lobby and I'm guessing they didn't earn those by having patients "who have no hope of getting better" taking up their beds. read more

staff kept complaining about their work? like why are you guys working here if you are gonna get mad when we ask you to do your job? most of the male staff is nice it's the women who have the attitude. my advice to the workers is to find an office job where you are away from people. don't bring that negative energy around my mother. if you love your family member don't send them here. this place needs to be shut down, my mom had to wait 20 min + each time she pushed the help button in her room. even had to use the restroom on herself a few times. staff claims to be busy but they can be seen in the hallways on their phones while help lights are going off in the rooms. lastly we took my mom here after surgery hoping she can get proper therapy which was promised. the 5 days she was there she did a total of 15 min of therapy. that's embarrassing, had better care at the hospital then this place. please don't take your loved ones here! read more

Great And truly caring Rehab facility. Staff are super friendly and ready to assist. The facility is spotless. The menu offers a wide variety of foods. The Rehab team is great, they even encourage the family members to stay and watch the patient as they go thru their therapy. The patients have arts n crafts, movie nights and even special days were outside food is catered in. There is a lovely garden in the back where families and patients can sit and visit. My sister was sent there from Methodist Hospital after a fall. She didn't break anything. Although I had her advance directives neither the hospital or MCC would tell me why she was sent there, I knew she had extreme edema from too much salt, and needed to keep her legs elevated. That was something I could handle. They wouldn't let me bring her home because over half their beds were filled with Covid 19 cases. She had never been exposed to Covid. I kept in touch with her by phone daily. and she would tell me about the care she was getting. She was supposed to be gotten up daily, and to have physical therapy 3 times per week. During the 6 weeks she was there, they only got her out of bed 3 times. I could tell on the phone she was getting weaker and weaker She soon told me she couldn't lift her head off the pillow without lifting it with her hands. She said there was one nurse who didn't like her and would deliberately put the phone and her glass of water just out of her reach. When I complained to the partient advocate, I was told they were doing the best they could do because they were short-handed due to the Covid. When she had been at MCC for 5 weeks I got a call that they had finally tested her, and she was positive for Covid but asymptomatic. They were moving her into the Covid wing. Eight days later, (not even the quarantine time, let alone the isolation time), they called and said they were sending my sister home the next day, for me to care for. I asked if she had been tested again for Covid and I was told she wouldn't be tested again until 90 days after the first test. I called our family doctor who said he would see that she was tested. I was told that MCC wouldn't allow him to come in and make a test. She arrived home in a hospital bed, on oxygen 24/7. with two bed sores and catheterized, After two days at home. I got a call from a hospice company, advising me that my sister had been put in their hospice care without even telling me about it. She began to improve at home but the lack of care she had gotten at MCC had taken it's toll. She only lived three more months. I would definitely NOT recommend this place to anyone. read more

We just have my mom transported from Huntington Hospital to MissionCare for rehab, and so far so good. She have a private room, with nice staff. She can have overnight stay and kids can visit at the time. Most reviews were old and the new ones were 50-50 1 star and 5 star. Made me worried but the reviews were old and had one or two 1 star for other rehab centers. We chose this one because we had good impression from when my grandmother was there. read more

The entire staff at Mission Care was loving and very patient with my step mother with end stage Alzheimer's. During Covid it has been a challenge to surrender and trust others to care for a loved one who is incapacitated. I do not recommend this place. They manipulate things to prevent family members from questioning actions. They discharged my relative saying he was "ambulatory" when he could barely stand on his own two feet - with a walker. They said he refused PT but the man in the other bed said he never refused - he was just unable to stand to do it! His surgical would was leaking yet no one gave him a fresh bandage or raised concern about the blood staining all the medical wrap (and everything it touched). He had to maneuver a flight of stairs when he got home, yet he had no PT or OT to teach him how to deal with stairs. Sure enough, we get him home and he falls flat on his back on the driveway before I can even turn around. (He weighs 290 so I could not catch him). He then had to SLIDE on his butt into the house and down a flight of stairs. Again he had to slide to get near the bed. He has not been out of bed since. Shame on you Mission Care. read more

He is very young but it's obvious he loves to work with older people. His upbeat personality is just what's needed when going through tough times. The place is clean and everyone is super friendly and caring. The Physical Therapist Daren is very patient and encouraging. Let me say upfront that this review only applies to the Healthcare Partners part of this facility. My mother (in her 90's!) was recovering at this facility from a broken hip. I found the facility to be clean, well run and the people very friendly and helpful. If you are a Healthcare Partners patient, however, they will try to discharge you even if you are not capable of functioning at home. Both my mother and her roommate, who also had a hip fracture, were unable to even get out of bed without full assistance, yet Healthcare Partners tried to discharge them both to home. On a Friday afternoon, the coordinator told me that I would have to arrange 24 hour care for my mom at home as she would be discharged Tues morning. I'm very surprised there have been no legal cases brought for patient dumping. read more

This review is long overdue, but oh so merited. I had to have a very complicated knee surgery. It was daunting to face being in a rehab unit once again (May/2016) (And, I didn't wish to have another horrific rehab experience like the one I had for it over at Kindred, Glendora, a year earlier (written up by the County Department of Health based upon my report and their inspection) I had pre-screened local rehab options and based upon my personal visit, and internet research, Mission Care was my first choice if I wasn't admitted to the Arcadia Methodist rehab facility. There are many notable features to share about my two weeks at Mission Care which deserve specific mention. They are listed below: 1. Mission Care employs sufficient, caring and compassionate staff so that one is not assaulted by the all too frequent smell(s) characteristic of too many nursing facilities. 2. During my two week stay at Mission Care, the following staffers offered me exceptional support: o Mariletta , CNA - who was willing to go way beyond the call of duty to assist me with a fecal impaction so I was relieved of unrelenting misery in time to enjoy my birthday. She recognized my utter discomfort and in a thoroughly compassionate manner addressed my needs and respected my dignity in spite of the primitive nature of the task at hand. o Leah and Joe, CNAs - who remained unfailing cheerful and kind even while my roommate loudly berated them for issues not of their making. They maintained their professional attitude under really trying circumstances. They persisted carrying out their duties and kept their composure. o Charlene and Hien, PT assistants - were so understanding providing me physical therapy, while recognizing my trepidation due to the rare and fragile repair - my 5th knee surgery over the course of my lifetime. They were so encouraging and Charlene's sense of humor lifted my spirits and relieved me of my fear. o Cherie, Patient Manager - not sure of her exact title, was so sensitive and understanding when I shared how difficult it was to room with the unhappy patient in my first location, and was quickly responsive to my dilemma by moving me to a wonderful new setting. 3. Mission Care is deserving of commendation for contributing to the comfort of patients at a time of great vulnerability. The delivery of this mode of care requires extraordinary patience, understanding and competence. Health care professionals who elect to serve this population are unsung heroes. They willingly see patients through devastating medical events. I am most grateful for their dedication. read more

My 93 year older dad fell and broke his hip and got a partial hip replacement 3 weeks ago. Before discharge, I frantically looked for a Rehab facility and consulted a few friends who are familiar with elderly facilities as they are either in management or acting as consultant.One told me Mission is Care great especially its PT program, the other told me not to rely on the bad Yelp reviews on any facility as you only get the feedback from one side. I was told to check the overall CMS rating as the State does random visits and usually checks the facility for the entire week. Thus if a facility is considered by the state as one with 5 star CMS rating, that would be good. I then checked the CMS rating of Mission Care and found that it has 5 stars. After my dad moved in 18 days ago, I go to Mission Care everyday. There is a charge nurse, a CNA assigned and my dad can always get a response shortly after he pushed the bed button. He has a private room with shared bath which works out well. He is having care from the PT and OT Dept. Peter of the Dept is amazing and very experienced n encouraging. The activities director Lera always remembers getting my dad to activity and brings so much joy to all the attendees. Lee from Admission is extremely attentive and offered to go with my dad when he needed to see an eye doctor last week. Casey who runs the facility managed to instill a culture of care and compassion and you see that in each of the staff which is not a easy job. Food services Dept would make changes on the menu upon my dad's request. Remmi, the nurse supervisor returns my call promptly and discusses medical status with me. Sue the speech therapist helped my dad to exercise his mouth so he can eat, swallow and talk easily. The podiatrist also came and prescribed ointment for his feet. I wish to mention especially that Anneth, one of the charge nursess is most caring. Tighe, the CNA looks 35 even she is a grandma would keep checking on my dad n does beyond her call of duty. she speaks Indonesian and Chinese which helps my dad a lot. Maritise, another CNA is so attentive to my dad and would think of his needs in advance without us asking. I always get a warm greeting at the front desk and the hallway as I get to my dad's room from Tamica, Tran, and Dean. One suggestion is that there is a 24 return call policy. There was an incident at my dads feet feet were dropped because the foot rest was not attached to the wheelchair. This happened when he was being pushed out of his room for exercise and it caused great pain to his foot. Nevertheless the charge nurse and the CNA found out later and insisted that an incident report be filed. And on site x-ray was immediately performed thereafter. Fortunately nothing was impacted at the surgical site. Update- date of discharge: Annett the nurse followed up till mug night last night and 6:30 am today for the gout medicine as my fad just had a gout flare up. Gabriella the Social Service director got the beautiful wheelchair, walker and comote within 48 hours via Medicare, arranged the in home nurse and PT. What a fabulous team. read more

At first I was skeptical because rules and regs but they really worked with us to meet our mom's needs. Nurses were all very carrying and my mom said the food was decent. Coffee sucks but least of our worries. My mother cried the day she was released, that's how much she liked it there. So yes, if you have a loved one you want cared for I would recommend this place but you have to do your part as well. Yes they took good care but we were there every day. I saw people in there who never had visitor's. Don't forget your elderly family members. A lot of their hospital care depends on how you care for them personally. read more

My father stayed at this rehab facility for about 5 or 6 weeks. He went in when he was very weak. He couldn't pull himself up and he came out in the best shape ever physically and medically. - The place is super clean. We have tried other rehab centers and they smell awful from all the soiled diapers. - The staff is super friendly and nice. They treat their residents with love. - The therapy is superb and exceptional. Dad would spend 1 to 2 hours in therapy almost daily not including the group exercises that they performed regularly. - The outside grounds are so beautiful. Dad enjoyed sitting near the fountain in the afternoon. - They had a doctor and nurse practitioner that visited weekly (plus the nurses on staff). Dad would get weekly blood tests and they adjusted the medications regularly when needed. His health and physical being is the best he's been in over a year! 5 weeks at the place created magic! read more

5 STARS!!! From the moment I entered the building I was greeted with warm smiles and a kind nurse. I had several questions and they were all answered. I was given a tour of the facility. It smelled great and was clean, something I was looking for and that many around were lacking. I was impressed because not only did the nurse who was giving me a tour answer my questions, but others came up to introduce themselves to me. Highly recommended for your loved one! read more
