Staff never notified me that he suddenly became delirious until he fell and broke his hip, wearing DAYS old dirty clothes. This was caused by a simple UTI which never crossed their mind to check for. Its a common thing known to anyone in the medical field. One thing lead to another and he was sent back to the facility for hospice. Quail Creek only allows hospice companies of THEIR choice. The hospice is only allowed to take readings TWICE a day. They are not allowed to stay, not allowed to give pain medication, not allowed to give physical care, cleaning etc. They are allowed to take him to the funeral home though. Others issues and Quail Creeks response: 1. Patient falling out of the bed. Response: mats on the floor. 2. Not able to give pain meds once an hour. Response: increase dose and decrease when they see him. 3. Not cleaning Patient or checking he is still in bed or hasn't pulled out catheter. Response: changed the topic. 4. Nurse not remembering what the last med was that was given. Response: changed the topic. 5. Never gave Patient wet sponge to moisten lips and mouth. Response: changed the topic. 6. Nurse to busy watching TV to do duties. Response: changed the topic. If you are in the facility for some short term rehab, its prob fine. But if your loved life might experience end of life, be advised they may experience pain and suffering through neglect. And yes, we reported them to the STATE.

The DON was INCREDIBLY nice. She didn't make me feel as if I were being interrogated! It was a great interview. She told me what was expected, I expressed mine as well. The flow of the building was smooth. Everyone looked and sounded very happy. I was impressed by the staff, they all helped pass trays, Administrator, DON, ADON, they all helped. They answered call lights. It was truly nice to see a place work as a team and work well together, and I was only there for about 4 hours and witnessed thus myself. If you're looking for a place for a loved one, I recommend Springfield Villa. The staff appear to care, the residents look happy and clean! The activity dept is the best I've ever seen, and I've seen a lot! The therapy dept is huge and have some fantastic PT ppl there. Didn't room area is huge, plenty of room for everyone and family as well. I look very forward to my experience and what kind of experience I can bring with me!

She came in with no bruises from a hospital stay from a stroke and left with bruises from head to toe (witnessed and documented with the State of Missouri advocate). I went to visit her and found her in a dark room on the floor and lots of staff at the desk talking. She was found AT EVERY VISIT to not have a simple glass of water in her room, no snacks, no t.v. I was told they don't have enough glasses for everyone to have their own and keep them in their room. Had to end up having a family member stay the night for her safety. These residents, in my opinion, are not being taken care of. What I saw would make your mouth drop to the floor! The temp is so high that the pee smell overwhelms you! So nasty! Would never recommend this place and I feel sorry for anyone to has to put their loved one here!

My father was a resident in the Maple Wood Alzheimer's Special Care facility for the last five…read moremonths of his life. This facility was absolutely wonderful for him! He had previously spent a few weeks in a different high quality nursing home, but he did terribly there due to his desire to constantly be on the move and becoming overstimulated by the noise and activity. He actually was expelled from that facility due to his restlessness and interference with other residents - such as going into their rooms and bothering them. Since all of the patients in Maple Wood suffer from similar cognitive limitations, his behavior was not as disturbing to the other residents and he enjoyed the quieter environment. He loved to walk the hallways and could stop and sit in readily available and comfortable seating. [By regulation, chairs are not allowed in the hallways of most nursing homes.] The staff were WONDERFUL. They obviously cared for dad and did their best to keep him nicely dressed and groomed - which was not easy due to his resistance to "help" and his penchant for "borrowing" other peoples belongings. The food was also wonderful, which resulted in his gaining weight (he had been anorexic) shortly after moving in. When dad began his terminal decline, the administration and staff gave us the opportunity of involving hospice and he passed away peacefully in his room with family members at his side. I never fully relaxed prior to dad's living at Maple Wood. I always had concern in the back of my mind that he would get himself into some sort of trouble and would either harm himself (accidently) or that I or my sister would have to drop everything and run to his rescue. Once dad had moved to Maple Wood, I was able to fully relax when I was away from him, as I knew he was safe and being cared for better than I could!
