Severely undertrained; understaffed facility resulting in catastrophic if not fatal caregiver neglect.

Having been in another facility where her care fell short of her needs, I searched the local area to find a facility that I thought could provide the care she needed. That led me to Poet's Walk which has turned out to be the right choice. From the moment that my family and I toured the Poet's Walk facility, seeing the staff at work with the patients, I was left with a feeling that they knew what they were doing and the level of care that my wife needed would be available. The staff works well with each other; each residence is assigned to a specific Care Partner. They assist each other as necessary for the proper care of each resident. In watching the care partners at work, most really show care for the residence. The building was thoughtfully designed. There are two U-shaped wings for the resident's rooms with a dining room for each wing. Each wing is connected to a central area that is called the Main Street. From the Main Street area there is an outside courtyard that allows the residents to enjoy being outside on nice days, surrounded and protected by the surrounding building. My wife and I often sit outside in the courtyard and enjoy being outside among the flowers, many of which are taken care of by the residence that have that ability. The Main Street area allows the residents to gather and enjoy the many programs that are designed for their abilities and pleasure. On my daily visits to my wife, I often watch them play games of different types and see the enjoyment they get from the entertainment often provided. Although my wife is unable to participate, she does enjoy watching the activities. My wife and I eat together daily at lunch. I really enjoy feeding her and look forward to our time together. She eats breakfast and dinner with the assistance of a care partner. The food is good and might be considered excellent for this type of facility. It has improved considerably over the past few months. Organization in the dining room has gotten better but patients is required by all as the staff must assist the residence to get to the dining rooms and then sort out the correct type of food for each resident. This all takes time sometimes causing a wait to eat. Poet's Walk is well managed. I see and talk to the different section directors, almost on a daily basis. They are always open to suggestions and strive to find a better way to do things, evidenced by the changes that have take place, always to the better. Is Poet's Walk perfect? Of course not but I can say with confidence that management is working on getting there. Gerald and Gloria Sutts

I'm currently a patient at Carriage Hill recovering from a surgery and have been here for some time. I can't say enough good things about the center as a whole. The first thing I noticed is the lack of urine and antiseptic smell. It smells fresh and clean throughout. It's very clean and orderly. The patients and full time residents are in good spirits. There are daily activities to keep residents busy, happy and laughing. Just last week there was a band and a cookout. The nursing staff and CNA staff are michelan standard and I don't say that lightly. These folks care about the patients here and it shows. They're not faking it. The administration is made up of kind people that took the time to get to know me when I came in. The dining room is wonderful. The food is so good. The kitchen staff treat us like family and the variety is there. Again, the cleanliness is unmatched. Every staff member that I've met has been helpful and kind. The physical therapy crew that I've been working with are patient and kind and have helped me to feel better and get on my way to healing. I'm only here for a short time and hope I will never have to be a full time resident anywhere. But, if that happens I would chose Carriage Hill in a heartbeat.

Throughout his stay, we witnessed repeated instances of negligence and lack of proper care that could have easily been avoided. On more than one occasion, my father fell from his bed because staff failed to ensure the floor mats were in place. Despite the first fall, the same mistake happened again a week later, when they forgot to replace the floor cushion after a meal. We also discovered that the wheelchair provided to him was faulty--it only had one functioning lock on one wheel. One day, while my mother briefly stepped away during a walk outside and after locking his wheelchair, he slipped down and off a curb, causing him to fall off the wheelchair. Even one of the head nurses, Heather, acknowledged that the wheelchair was defective. The most devastating incident occurred the night he passed. My father repeatedly told the nurse he was having difficulty breathing. Although she gave him oxygen, he was becoming increasingly confused and kept removing the mask. Knowing this, she should have checked on him more frequently--but instead, he was left unattended for nearly an hour, during which he passed away. To make matters worse, we are now receiving bills from the facility, even though his insurance confirmed that his stay was covered. I do want to acknowledge that there were a few kind and attentive staff members--specifically James, and Brea - who showed genuine care and compassion during his time there. Unfortunately, their efforts were overshadowed by the facility's overall lack of responsibility and attention to patient safety. Our family trusted this facility with my father's care during his most vulnerable time, and that trust was broken. I cannot in good conscience recommend Falls Run Nursing Home and Rehabilitation to anyone seeking compassionate or attentive care for their loved ones.

Cardinal Village kept standing out. The difference was their staff is such a positive, cohesive team, lots of comradarie and obvious empathy for their residents. Their directors (managers) are great; they are hands on involved. The care workers deserve praise; day in, day out, I watch them doing their jobs well, with love. I was told by management that they consider success is getting their residents to smile. A kicker in our selecting Cardinal Village was my next door neighbor had placed her husband there. She spoke highly of the place. She said there had been a few hiccups but they had always listened to her concerns and promptly addressed them to her satisfaction. We were sold. My wife has been there almost four weeks. By all accounts she has been thriving from day one. And because she is doing well, I am doing well. In addition to the loving care there, her day is filled with fun activities--mental and physical. It is so much more than we were able to do for her at home. Our family is convinced we made the right choice; we could not be more pleased with Cardinal Village.

I can say without hesitation that there is absolutely no lack. She is well cared for and the residents are able to roam the facility behind the lobby door, which is locked. Cameras are in every hallway can can be seen by staff members. The staff keep in touch with each other via radios. The residents are assigned to staff members so every staff member is not directly responsible for all residents at the same time. With smaller groups, the residents get more personalized attention. If someone doesn't show for a meal, someone serving lunch will use the radio to request a search of a resident by their room number. My mom has only been a resident for a year, yet I feel a bond with some of the staff. My mom is getting all of her prescribed medication, which she hadn't for the prior year. I would recommend Cardinal Village to anyone in search of a memory care facility for their loved one.
