    Awesome place with compassionate therapists and staff! Also a great place for occupational therapist students to learn. Highly recommend!

Scott Booth, DPT, this clinics' Physical Therapist/owner, is a highly energetic, knowledgeable and experienced provider who practices traditional and manual physical therapy and presents with an array of effective intervention strategies. Scott is the best doctor of physical therapy that I have had, over eight orthopedic surgeries in 30 years. Scott owns Paragon Care, a home health company and I met him when he managed my in-home, post-operative care last year. Scott has helped me following two knee replacements, a cervical disc (neck) surgery, and a shoulder replacement. Additionally, I had a six disc back fusion five years ago, and he is currently treating me to reduce referred pain, which has returnedover time. Scott typically used trigger-point manipulation , cupping, electrical stimulation, massage and exercise during our sessions. No time is wasted, he is prepared, efficient, quite personable and VERY kind. Scott is the real deal and accepts many kinds of insurance. Hope you are lucky enough to get to work with him! He has made such a positive difference in my life many times.

In mid December I called (during the business day) and had to leave a voicemail. My call was returned the next day by a nice woman. She explained that she would verify my insurance benefits and then schedule an appointment for my daughter. She called back a couple of days later after verifying benefits and told me that my insurance covered nothing until my deductible and out of pocket had been met, so essentially I'll be private paying. I told her that was fine. We agreed to schedule an appointment after the first of the year instead of trying to squeeze it in the week of Christmas. She said she would call back after January 1 and schedule us. Since I hadn't heard back from them I called today, January 5 and tried to schedule an appointment. I spoke to a really rude man. He would not schedule an appointment. He insisted insurance had to be verified again because it's a new calendar year, even though my insurance has not changed and I told him I would just private pay. He also told me they were unable to verify BlueCross BlueShield because of technical issues and could not tell me when those issues might be resolved. In the meantime he told me appointments were filling up quickly and I'll be added back to a waitlist. I'm really disappointed! I was not told insurance had to be verified AGAIN if I did not schedule an appointment in December. I was not told I could not schedule an appointment without verifying insurance again. Why do they need to verify insurance if I said I will private pay? Why won't they go ahead and schedule an appointment while they verify benefits? This place is a nightmare to deal with. They therapy might be great, but it's not worth dealing with their office.

    My brother has been at the Meadows for a year and he's comfortable there. I visit 2 to 8 times a…read moremonth and always observe kindly, respectful treatment of the residents, even the ornery ones. The facility is clean, the food is satisfactory and adequate. Our family is grateful that our brother has a place there. He nearly lost his life due to careless medication management in a private assisted living facility in Hunt County where he stayed for 17 bleak months. That ended with an ambulance ride to the ER and 2 weeks in ICU due to bad medication management. In that process, our brother's cognitive faculties were so damaged that he qualified for Medicaid long term care. I'm sad about his medical status but the Meadows takes good care of him. I regret that Medicaid reimbursement rates are so stingy that life is not very diverting and interesting for Medicaid-dependent nursing home residents. Of course I have fears about the general trend in Medicaid reimbursement. But the Meadows does very well with the resources it has to work with. I'll pay special tribute to Mr.Bill, the loving heart and soul of the place! He's not in management nor is he a licensed professional therapist or anything, but he is all over that facility doing his best to ensure that spirits are high. He has been doing this for decades and I believe he truly is the house Angel.

    Last year I praised this facility and rated it highly. I retract all the nice things I said back…read morethen. Don't let your loved one end up here! Several outstanding employees quit or got fired when the facility was sold to owners. Several key employees who had worked for the previous ownership and now retain their jobs under the new ownership, are people I now regard as dangerous. As of late 2025, my rating of this nursing home bottomed out. I'm sorry to say the place is unacceptable. My brother WAS well and comfortable, but he has deteriorated after being abused and physically assaulted by a mentally ill roommate. The Facility lied to us about it. At 6 ft 2 in, my brother now weighs less than 130 pounds, and looks like he was just released from a Nazi concentration camp. The Director of Nursing looks me in the eye and declares he's fine.
