My experience with Dr. Brian Pell deserves to be publicly documented so others know exactly what they are walking into. I booked an appointment specifically for an ADHD assessment, clearly stated as such. That was the entire purpose of the visit. Yet, during the call, Dr. Pell casually informed me that they are not conducting ADHD assessments at this time and may resume, possibly in January or February of next year. This immediately raised a red flag: if you are not offering the service, why accept an appointment for it? Why bill insurance for something you never intended to provide? The entire interaction lasted 18 minutes, and not a single part of it resembled an evaluation. Instead, we spent the bulk of the time discussing my career. He asked about my former psychiatrist's nationality; he seemed very interested in why he didn't take insurance "as a Black doctor" (which I perceive as a common conflation between race and poverty). He then asked only two quick, surface-level questions about why I was seeking an ADHD assessment. Then in the same breath that he told me they are not doing assessments, he also told me that I probably do not have ADHD. He said this despite the fact that I was coming to him for a second opinion after already completing a comprehensive, legitimate ADHD evaluation with a diagnosis from an actual specialist. (I did not share I was coming in for a second opinion, as I didn't want it to be biased). He provided no testing, no structured questions, no clinical methodology, nothing that even remotely resembled evidence-based practice. The whole thing felt dismissive, and frankly, like a complete waste of my time. I left the appointment feeling misled and deeply uncomfortable, as insurance had been billed for a service that was never delivered. If you are seeking serious mental health care, diagnostic clarity, or responsible clinical practice, I strongly recommend looking elsewhere. Waste of money, time and energy. Very hurt by the whole experience. We sought care for our daughter and were misled. The provider did not review the data we submitted, then sent us for further testing that we later realized was repeated testing and ultimately we couldn't use any of the evaluation from NYC Psychiatric Associates for the school meeting (in two days) because it has been deemed invalid because the same testing had been completed four months earlier by the school and due to the time frame was invalid. We sent records from many different sources, and it is now our understanding had they actually reviewed that data the testing they recommended wasn't even warranted. We spent a lot of time and energy on these providers. They are kind, but ultimately careless, not thorough, and incompetent. Ultimately the "results" hurt our case with the school who we have already had many struggles with. What they provided us with was literally not usable and we told them many times what we had hoped to accomplish with that documentation. Now they say "we provided the diagnosis" and dismiss the other reasons we went to them. Justifying everything, and it was expensive. My family invested in it! They did not review all of the testing from other providers, and had they done so it is now our impression they should not of recommended further testing because it was unnecessary. My family feels like we were provided poor guidance, our concerns for our daughter were capitalized on for profit. We requested a refund since the documentation could not be used at the school as we had told them we needed multiple times (even sharing concerns it could not be the same testing multiple times) and I have multiple emails in my inbox in which they justify the quality of their work saying "we have her a diagnosis" - then minimize and deny the validity of the concerns we have posed regarding the entire experience, and they are refusing to refund us. They have offered to revise the documentation, but I have already submitted what they originally sent me to the school. They have lost credibility, and so have we. It has already been dismissed by the school as unusable. The school meeting is tomorrow, and the "magic bullet" we thought we had invested in is a ZERO and a major hit to our bank account. The staff seem very sweet well meaning, but results are NIL. Cold competence that gets results is far better than the lip service we received. They seem as though they genuinely care, but at the end of the day the results did not get us what we needed for our daughter. The staff seemed kind, but the competence simply wasn't there. Reviewing available data is important to the process and *standard in this field and it shocked us the way things have transpired. 100% unprofessional. When we had issues, I had to ask many many times before a supervisor ever even got back to me. A literal nightmare. This has now been a back and forth email for over a week with no refund in sight. I'm sick of this situation, and we are already so stressed addressing things with the school this has just been another problem. They knew we had limited resources walking into this, and we feel used and burnt out and like we have less faith in providers of this caliber than we did when we walked into this situation looking for help. I encourage you to strongly question any of the recommendations they give you and "evaluations" they recommend and whether or not that is even warranted. If you have alternate providers that you trust, I would start there instead. They profited off of a vulnerable child and scared family in out experience... and when they made mistakes, they doubled down. Not a place I would invest in to help my child read more

I had been suffering under the care of another psychiatrist for a year and a half. Finally out of desperation I googled the best psychiatrist in NYC and Dr. Mekles came up. That description of him is very accurate. He is without a doubt the best doctor I have ever dealt with. The attention you get from him is outstanding. You have to review your automated text and email reminder policy. 8 reminders within 2 weeks is too much and actually anxiety producing. I get so many reminders towards the approaching appointment I feel my heart skip a beat the I forgot the appointment or that the appointment is that day, not 24 or 8 hours later. The reminders are nice, but there is just too many of them. You run the risk of being mentally filtered out and forgotten. The exact opposite of what a reminder is supposed to do. Yes, on one occurrence, the system's "over remindering" actually caused me to nealy miss a scheduled appointment. Fortunately for me, I have a good track record with the provider and he went out of his way to contact me to have my scheduled appointment. You can have too much of a good thing. read more

As with many offices with multiple doctors, who you see changes everything. Dr. Faith Consiglio is patient, kind, down to earth, empathetic, realistic, willing to listen to concerns and give solutions/recommendations based on those. I have never felt rushed. She makes me feel seen and valued as a patient. I cannot recommend her enough. The office staff as well are so incredibly kind and helpful! I do not recommend seeing Dr. Mekles. While he is friendly, he is not careful to listen, does not take thorough notes, and regularly repeats questions and/or treatment recommendations already discussed at length. I did not feel like there was a clear plan to meet my health needs, nor did I feel heard since I so often had to repeat myself. Updated review: Minutes after submitting my review, I was harassed by Dr. Mekles by phone and text: "If you have a few minutes to talk that wokld be great. We work as a team. I helped you see faith and the comments do not help our morale or mission." And "It's unhelpful. I work so hard." I had to reply 3 times that his contacting me made me very uncomfortable before he stopped. I have changed my 4/5 (generous considering my poor experiences with him) to a 1/5. Not mentioned above, in October, he sent the wrong information for a claim, and as a result, my disability was canceled, and I've been unable to get it reinstated. Uncomfortable experience. 30 minutes late to the call with no apology. He wasn't even at home or in an office during the video call. Seemed like he was driving or something as he wasn't fully focusing on the camera and I could see sunlight moving in and out of the video. The video quality was also poor on his end and it kept cutting out. I did not feel he paid any attention to what I was saying and I felt awkward. I don't want to speak to a healthcare professional who is distracted. First time (and now last) meeting with me. He talked to me for 10 minutes (maybe less) and was eager to prescribe me something. He sent it over to the pharmacy and I'm not picking it up. I don't think I feel confident in his "diagnosis". Anything he said was superficial and didn't go beyond the surface level. "You need to get better", "Sounds like you have serotonin issues" - wtf? Didn't ask family health history, if I'm sleeping well or anything that may actually help a doctor determine if a patient needs treatment. Randomly asked about my childhood, but it doesn't seem relevant. Never had a care provider who was so disinterested in their patient. read more

Its uncommon for my opinion to change about anything, but after more than a year and a half under Dr. Mekles' care, I feel compelled to update my previous review. I highly recommend Dr. Mekles for those seeking a straightforward and candid psychiatrist. He has a rare talent for calling out BS without making you feel belittled and genuinely listens to well-reasoned arguments from his patients. His method uses elements of CBT and DBT to focus on present issues and your sphere of influence. What are you cognizant of, and how can you act on it? He won't coddle or cater to you; he will treat you. Additionally, I appreciate the practice's administrative approach: treatment above all else. While most of the Healthcare Industry treats the patient as a commodity, NYCPA goes out of its way to let clients know they will work with financial hardships. They understand how crucial continued access to medications and treatment is, even if you've lost your income/insurance. Considering the delicate nature of mental health, I find this just as important as the care administered. read more

My recommendation is to avoid Mekles. He railroads conversations, doesn't respond to any of your concerns about his treatments, and is chronically late and unprofessional. My advice, there are plenty of better professionals who can help you that aren't this guy. I found one shortly after leaving this guy and they are light years better. I saw him for only one session. Highlights are: * he was late * session lasted a grand total of 15 minutes * he asked me what I'm here for. I spent 10 minutes telling him. he then asked flippant questions like "are you sad?", "do you think you're depressed" * he told me he was going to prescribe me antidepressants. this was 10 minutes after talking to him. I voiced my concerns that I've never been on antidepressants and was hesitant. he instead told me "it's a shame people who need help won't take medication". then he immediately moved on * I told him my pharmacy and he said "I don't like that one, they aren't a good fit. We're going to use a pharmacy I like instead" I was scheduled for a second session. This time he was 15+ minutes late. I called his office, canceled the appointment, and have not seen him since. He called me 20 minutes after the scheduled appointment time first saying "you can't expect your psychiatrist to be on time. there are other patients ...". That explains a few minutes, not 20. He then started asking if I don't want to see him again and when I said no he bombarded me with questions to understand why. After him ignoring my concerns in the last session I simply told him we're not a good fit and ended it at that. Have not seen him since, but I have found a great psychiatrist who is light years better than this guy. read more

Both TMS and spravato have side effects and can make the treatment unpleasant so you want an environment that is most comfortable for you. It can make or break your treatment and mean being successful or giving up/stopping treatment early. For spravato: Sometimes you need to share a room with others since there is only one private room. If you are someone who is going to throw up because of treatment since nausea is a biggie you may want a single room for privacy. TMS: It can hurt. Full transparency Ara is not a doctor. He is a TMS technician and Fiona is a fresh out of university. A lot of places just train people to be TMS certified but they don't need to be nurses or physician assistants or even in the medical field. There are some places where the psychiatrist does the mapping of your brain and a physician assistant does the treatment which can really help when you are in pain because they know how to make adjustments. All depends on your comfort level on who is doing your TMS for you. Privacy: Staff are friendly which some people like. But I did end up overhearing people's personal details so if you are an introvert/want some level of anonymity speak up and set a boundary. Pro: doctor is always on site which is required in case you have a seizure during TMS or high blood pressure during spravato which they are supposed to test before, during, and after treatment. Key takeaway: do your research and advocate for yourself. Don't let the fatigue of finding the right treatment and place stop you from finding the right environemnt for you because it can really make a difference to your treatment. read more

I wish I could review Dr. Mekles specifically, rather than the whole practice, because I used to see Dr. Sager and he was a fantastic psychiatrist! But when he went on sabbatical, he transferred me over to Dr. Mekles, who has been rude, inattentive, dismissive, and can't bother to listen to me during our sessions. He always shows up to our video calls 15-20 minutes late, rushes through the appointment, and responds with "sounds good!" regardless of what I actually say. On multiple occasions he has prescribed me the wrong form and wrong dosage of my medication, and since it's schedule 1, I'm left rationing my meds because I can't get a refill for another month. But the worst part is that when I left negative reviews on some sites mentioning my concerns, he called and texted me over and over until I picked up! He and his COO tried to get me to take down the negative reviews, and when I stood by them since I was just telling the honest truth, he tried blaming his incompetence on the insurance company, on "bad luck", and said that I just "slipped through the cracks". He couldn't even take responsibility! If you're booking so many patients that you're late to appointments, can't pay attention to any of them, and prescribing the wrong dosages, it's your fault for overbooking yourself! This is not a field where its acceptable for patients to slip through the cracks. NYC Psychiatric Associates has been amazing to me, and Dr. Mekles especially. Prior to treatment, I had been staying in the New York City Homeless shelters which were horrific. I had gone through severe trauma in my life and grew up without a family or any form of loving support, so chasing dreams and goals in my life was daunting, if not insurmountable at the time. My depression and anxiety were so severe that I was walking 15-20 miles a day to physically punish myself and keep my mind preoccupied. Despite not having any insurance and being in such a difficult place, Dr. Mekles saw me without cost until I was on my feet. He was kind, patient, and understanding. He was able to prescribe me Lexapro and Trazodone for my insomnia and depression. The medication absolutely changed my life. Not only was I now able to hold down a job (because I wasn't sobbing or freaking out in private every day), I was able to focus and take up hobbies, build supportive relationships and friendships, and get a place of my own. Once I was on my feet and had insurance, he further assisted me and has always been there when I need my medication refilled. I find his virtual appointments convenient as I work from home and can't commute sometimes. NYC Psychiatric Associates have been a real blessing, and I encourage others to use their amazing services. In addition, Dr. Mekles is very down to earth and helpful and has always been an encouraging soul. There are a few things I have to remind others of: psychiatrists are not therapists. They are not there to listen to your story and help you. While they will listen, they aren't the same as a therapist or psychologist. Psychiatrists are known to be quick, abrupt, and to the point. This isn't a bad thing. They're there for medication - not therapy. Just remember that if you want a therapist, you don't see a psychiatrist for that. Always make sure you order your refills in advance. I've sucked at this, lately, but don't wait until the last minute. Pharmacies take their sweet time. Again, the ability to be seen and to get the medication I needed to attack my issues has been life-changing. I had my first appointment with Dr. Todd Mekles, and it was an absolute nightmare. Being new to addressing my mental health through psychiatry, this experience with him was beyond dreadful, bordering on traumatizing. From the moment I joined our consultation, it was evident that he had no regard for my concerns or well-being. There was a shocking lack of empathy during the rushed appointment, as if he couldn't be bothered to spend more than a few minutes with me. He seemed completely uninterested in understanding the depth of my experiences, leaving me feeling isolated and unheard. His abrasive nature and complete insensitivity toward my struggles only exacerbated an already distressing situation. It's unnerving to think that someone with such a lack of compassion is entrusted with the mental well-being of others. To make matters worse, Dr. Mekles prescribed medication without properly assessing my needs. He quickly gave me a diagnosis after hearing about my life's experiences for 5 minutes and was absolutely confident that what he prescribed will cure me within one week. Shockingly, the prescribed medication not only failed to address my issues but also exacerbated them. It's alarming that a supposed professional in the field of psychiatry could be so careless with someone's mental health. I have met this doctor once, for less than an hour and I really doubt he knows anything about me so needless to say, his recommendations now are irrelevant and I am left questioning the ethical foundations of his practice. I was really hoping this appointment would have been the first step to recovery but this review is an honest critical review of services received. Despite our brief interaction, it became painfully clear that the negative sentiments expressed in other online reviews were a red flag that I regrettably chose to overlook as I really wanted this to work out. It's been months since this appointment and I have met with other professionals who have not only helped me but enlightened me to standards of care and respect that patients deserve--standards that were blatantly disregarded by Dr. Mekles. I had a mixed experience seeing Dr. Fan. Initially i was really hopeful to talk with a psychiatrist that came from the same culture with me. Our first two initial video session was really nice, he was warm and friendly and we had a great conversation. What I liked about him was that he asked me to list the 3 things that I wanted to improve (for me it was my attention, mood and carelessness). The medication regimen he put me on has drastically improved my symptoms and has helped me immensely. I'm very lucky and grateful for this (for some patients medication doesn't work or it takes years to get the adjustment right). There were some uncomfortable side effects, but that has diminished after a month. Then comes the bad experience. I complained of drowsiness on the ssri i was on and we switched to an atypical antidepressant. Right away i was extremely anxious and irritable, I hated it and i felt my mood worsen. I texted him how I was feeling and asked to be put back on the inital ssri. I reported my mood was very "irritable" looking back maybe I should've phrased it differently. He then proceeded to text me back at 3 am in the morning that I might be bipolar. That triggered my ptsd and drove my anxiety out the window. I texted him to explain more. No answer. I texted him again for another explanation (I've never experienced mania before). No answer. I texted the office to ask Dr. Fan to reply to my messages. No answer. When I finally saw him in person, I told how disappointed and mad I was by the lackluster response. If a doctor walked in and said you had cancer then walked away without any explanation or treatment, how would you feel? I didn't get much of an apology or explanation. I don't think I feel any of the criterias or bipolar disorder, but I did lose sleep and was filled with dread and anxiety the days leading up to the follow up call. As a healthcare professional, if something as a serious mental illness is to be diagnosed. It should be in person after a through history, matching symptoms to criteras before negligently texting your patient (who already has ANXIETY) at 3 am without a further explanation. read more

His approach is thoughtful, thorough and up-to-date. We've worked together over the years and I know he really cares about my wellbeing, and has from the beginning. Sometimes it's really tough to be open about mental health challenges, but not with him - he's really approachable and easy to talk to, and he takes his time to consider what you're sharing so you never feel rushed. I've seen many doctors and he's the best I've seen, hands down. It's thanks to him that I'm making a recovery. As part of my treatment I was able to also get approved for TMS, a different approach to treating certain conditions, which I was able to receive in the newly redone and beautiful office. The head of TMS is Ara, who is just a one in a million provider, too. He's patient, detail-oriented, flexible, and empathetic, and took the time to calibrate my treatment every time, as many times as it took, to make sure it was working exactly right. I met with Dr. David Willey today. In NYC it is insanely difficult to get an appointment. I had low expectations going in. Oh my god how wrong I was. I have had multiple therapists, psychiatrists and other types of doctors. Some good some bad. I have never felt so comfortable or listened to in my life by a doctor. It's like he knew me for years. He treated me with kindness, understanding empathy and respect and I am thrilled I found him and look forward to future appointments.
