No attention to an obvious diabetic patient's food needs, no dietitian on site most days, nursing staff could do nothing. Food delivered was mostly starch and sugar- not good for diabetics. I agree the facility was on the dirty side, especial the "bathroom" which was "a toilet in a closet". Because it was an older facility the sink was OUTSIDE of the bathroom. There were also 4 people sharing this one bathroom, including the room on the other side. At one point there were men in the adjoining room. Since it was a "Jack and Jill" bathroom people had to lock and unlock the door after use, so it would be accessible for everyone. That meant everyone was touching each doorknob with unwashed hands each time! Certainly it wasn't hygienic to say the least. The bathroom often had feces left on the toilet and on the floor on one occasion. When they did clean after we complained it was never a good cleaning, only the largest most obvious "situations" were cleaned. The kitchen was really messed up. There were supposedly two choices at each meal but the patients could never choose or know what they were. I asked for a menu but only received it for one day and even when I wrote down my choices I did not get them. Every meal was just sent to people and if you didn't like it or couldn't eat it you'd have to either call the kitchen who rarely answered the phone, or tell a nurse or aide. The quality of the food itself was low. The only reason I gave this facility any stars was because of the using staff. They have some very caring, knowledgeable, wonderful staff, who don5heir best to accommodate your needs. I would not recommend this facility because it was not clean or hygienic and the food was undependable and just not good. I am not a picky person when it comes to food and usually do not mind hospital food. read more

Do Not send a loved one to this facility. They are understaffed, the food is not healthy and does not meet any dietary needs for an individual with diabetes. My father went for rehabilitation and is now on his death bed because of the neglect. His wounds were not treated properly, which got infected, he face planted onto the cement floor after being there for a brain bleed and 2 broken ribs and they never called 911. per their protocol, they call some online doctor over a zoom call, who doesn't physically examine the patient to give medical advice. Theres a virus that spread like wildfire throughout the entire facility without any containment and residents are getting taken out one by one. This facility needs to be investigated for a lot of shady happenings that are being covered up. There's 2 LNA's to 40 residents and 1 nurse. impossible to give the best care under those circumstances. Please, do not send a loved one here. read more

If there could be negative stars that's where I'd be. The staff were great, the conditions they work in and the patients live in are deplorable. The food was terrible the bathroom facilities work only some of the time, the place was filthy. DO NOT send anyone you care about recovering here. read more

My wife was send there after knee surgery. The place smelled, it was dirty, There were four people using the same bathroom and some attendants also used it. First night there my wife went to use the bathroom, no shower, and there was feces on the floor and seat. Attendant just covered over it with paper and said it was OK. There were IV bangs on the floor from a previous patient, no phone or TV. Broken wall sockets that we cracked and pushed in. (Safety hazard) Base board heating painted over the rust Attendants talking loudly outside here room up and down the hall. There was a cart outside here room where they would gather and eat their snacks and have their drinks. She checked herself out with Dr's approval and apologized. read more

My mother was at Oceanside for a little over a month before her passing. She was 97. She spent one week in rehab and the remainder in long term care. The Rehab center helped to build back her skills and stamina. The long term care unit afforded her round the clock care and was especially helpful to manage her dementia. The nursing staff was very caring towards my mother and her needs. The facility is large enough to allow for movement and mobility and there are daily activities for the clients. We found the overall facility to be clean and uncluttered, but it does reflect an environment typical of a long term care facility. The administrative and admissions staff were very helpful. My mother absolutely loved the hair dresser. read more

This place is the worst facility I have ever seen or experienced. Our l mioved one is going backwards fast in this place. It smells aweful, the food is disgusting. The nurses do not care and the biggest problem is the people caring..pure laziness! If you don't have great insurance this is the type of place you end up in. Infection is the current battle above everything else. They don't change the clothes, shower once a week on Friday's. We are affraid when we leave after a visit...its a horrible feeling! If that's what your looking for in a facility then this is the best choice...our loved one will be out of this place as soon a possible before something tragic happens. I won't give this place 1 star...I will say they do have a couple professional people out of the entire group that seem attentive, other than them the rest are a complete JOKE! Let's hope and pray nothing happens before we get our loved one to a professional rehab facility! It made me pick a star to post...- 1 star! read more
