I have seen and visited other facilities in the past, and am thankful we were able to have both my mom and grandma at Pacific Post Acute. Having a loved one in a facility is never ideal, and while seeing several sick and frail patients can be jarring, I feel as thought Pacific Post Acute does the best in comparison to all facilities I've seen before. The fact that they are small is also a huge plus. Most of the issues we raised were addressed and resolved in a timely manner. The majority of the staff was great with my mom and grandma. Some of the nurses and CNAs became like family after several months of having my mom and grandma placed there. Kevin and Garret were the best nurses, and the CNAs (Salima stands out the most, with her goofy jokes and caring ways) were always concerned with their well-being. It's not an easy job, but you can tell which folks are there because they care about the patients. I also loved that they had several people arranging activities for the residents. Activities I also participated while visiting, and felt they were entertaining and not as forced as other places. Additionally, the patio area was a great amenity. It was nice to be able to roll them out to a tranquil space with fresh air. I know they are busy and have their work cut out for them. While I can't say they the experience was flawless, I do believe it was the best available and would recommend them over any of the other facilities we saw in the area. read more

I normally don't write review's, and however I am writing this one to say to family members who potentially may take your loved ones here don't believe the hype. DO NOT be fooled by the five stars they have received on their page. My Loved one was discharged from St. Johns Hospital and was admitted here for physical therapy for broken ribs. On his first night in this facility, I gave the CNA who came to the car to pick him up because NO ONE is allowed inside due to COVID no visitors. So I gave them his discharge orders regarding medicine, PT, and any other orders from the doctor at St. Johns. They gave him meds that put him completely out and he was still feeling drugged the next morning when I spoke to him. I decided to give him a call on the same night to see how he settled in after his admit to this raunchy facility, he never answered his phone so I decided because they have five stars call the front desk to speak with a Nurse or receptionist, the phone rang 20 times on my first call no answer and 20 times on my second call still no answer. I was feeling very scared and distressed. So the very next morning I give the admissions coordinator a call explain the night before to her and she's very reassuring on the phone she will speak to everyone and get to the bottom of this. Well lo and behold the very next day the same incident happens except this time, my love starting requesting his pain medication a little after 5p.m. and finally received his medication around 9:45p.m. after I called three times to the front desk and the phone rang twenty times each call, then someone finally answered the phone and implied that my Love was was making this up, he never pushed the call light or spoke to anyone regarding his medicine she was very nasty and unconcerned regarding the patient care. PLEASE PLEASE do not take your loved ones here, they even charge for the rails on the side of the bed. They have NO empathy for the patients or their families. read more

My family and I have had a wonderful experience . The staff, the rehab, and the facility are all first rate. It is not fancy and a little dated but is always clean and has a nice courtyard. My mother was here for almost 3 weeks following a broken pelvis and the staff were outstanding. The PT and OT are done almost every day and my mother made great progress. They were outstanding. The director Janes Mackey was terrific - working with us to get the right set up for my mother. The setting is not fancy and residents mixed so not necessarily a comfortable setting for folks used to a more upscale environment but the care made it worth it. For few nights we hired a private caregiver to be sure my mother had constant attention and the staff were more than willing to work with us for that which was very appreciated. UCLA geriatricians also have privileges there and are there a few times a week which is also very helpful. So great for post-injury rehab for elderly! read more

I would give Pacific Convalescent Center an excellent rating because of it's warm, friendly and competent staff. The place is always clean and there are a lot of activities on a day to day basis for it's patients. Kudos to you all! read more

As the husband of one of your patients who has been in this facility for six full weeks (April to May 2018) 12-14 hours a day, I have many good things to say. My wife got here without the ability to sit on her bed by herself. It was even impossible for her to stand up even for a few seconds. With the expert care of the group of therapists led by Therapy Director Daniel Perischetti, my wife is now able to walk with a walker and almost ready to be discharged. Hats off to the other excellent therapists, namely: PTs Lisa and Carmen, OTs Tom and Liz, and even OT-trainee Sam. We all saw an improvement from day to day, and we cannot thank these therapists enough for preparing my wife to get herself reintegrated into the outside world. The nursing staff led by "Ate Mirla" were superb and dispatched their duties with a lot of loving care, respect and compassion. From my several hundred hours of observation, special mention belongs to the following: RNAs Salima (who also brings in a lot of fun and joy each time) and Rachna, LVNs Eva, Ruth, Kevin, Cherry, and Garret, CNA's Eulalia, Ashley, Martha, Rosie, Mira, Francesca, Gloria, Isabel and other staff: Malou (Business Office), Lupita (Housekeeping-a special thank you!), Jojo (Human Resources), Rosemary (Kitchen), Cindy (Social Services) and Marcela (Activities Director). Kudos to James McKay (Administrator) for his management by example. BTW, for many years I was Vice President And Director for Human Resources and Executive Assessment Consultant, so I know what I'm talking about. I highly recommend this facility. read more

My mother passed away at Pacific Post-Acute last week after being there for 6 days. My mom had a chest tube so the options for a nursing home after she was discharged from UCLA Santa Monica were limited to 4 nursing homes and I chose Pacific Post Acute because it had the best Yelp and Medicare ratings of all the options. My mom was moved late at night to the facility and she called me the next morning hysterical and said "get me out of here. Please get me out of here right away." She said that she overheard someone say that she would be "leaving in a hearse." It wasn't in my mom's nature to be hysterical and so I tried to calm her down and mentioned that Pacific Post Acute was the best of the options. I wanted to speak with the manager about the "leaving in a hearse" comment but my mom wasn't sure who made the comment. My mother's condition deteriorated very quickly after she went to Pacific Post Acute. My mom wasn't eating much because she was being served food that she couldn't chew (healthy food like broccoli and brown rice, but she couldn't eat it because she had her dentures out and was very weak). She needed to be spoon fed so I brought her yogurt and fed her myself because it didn't appear that anyone else took the time to make sure she was eating enough. Her room was also very hot and she was very uncomfortable. When I visited her in the afternoon the day before she passed she looked terrible and couldn't speak. The head nurse on that Monday was not available when I was visiting so I requested that she call me because I was very concerned about my mom. After not hearing back for several hours I called and spoke to the head nurse and she seemed inconvenienced by my call because she had some paperwork to do and said she would look into it and let me know. She never called back and I found out the next morning that my mom passed. As I write this review, I'm trying to stay as objective as possible. I wasn't with my mom the whole time she was there but I know she was miserable at Pacific Post Acute. I also find it completely unacceptable that she wasn't given a diet that she could eat on her own and that she wasn't being fed enough. Additionally, the head nurse not calling me back the night before my mom passed when she was in so much pain is unacceptable. read more

This facility and majority of its employees are very professional.....starting ftom the DON Camille!! I worked at Pacific Post Acute And Rehab last year for several months. Its time for my certification renewal and I've called the facility spoke with secretary Esperanza several times informing her what my call was about. The DSD never returned my call instead gave my phone number to a cna (Salima) for her to inquire on my initial call. Salima was very rude and unprofessional! I contacted Corporate and Jackie from HR was clueless as to what and why I escalated my issue to HR. I reached out to HR because the facility is denying me of my inservice hours that I'm entitled to and required for renewal. If a DSD is not conducting Inservice Education classes at Pacific that's not my problem. I'll inform CDPH and Sacramento about Pacific. Not having an administrator plus a DSD who choose to ignore and deny former cnas of their rightful inservice hours. This is very unprofessional and against State and Health Department guidelines! I hope no other cna ever experience what I'm currently experiencing. Btw still haven't heard back from the DSD. I NEED MY INSERVICE HOURS!! read more

My dad was admitted this past fall due to a foot injury. He is 81 and mentally with it! He appreciated everyone's assistance at the facility. Because his injury left him having to learn how to walk with a walker we appreciated the fact that the facility was all on street level. It was easy to get him in and out for doctor appointments. The staff were very helpful and were genuinely interested in making sure he had a good experience. They would even check on whether he liked a meal! They definitely tried!! He also had a very good experience with the PT team. He ended up leaving early because he was able to walk faster than expected. This was in part due to the excellent physical therapy regiment. I would recommend this facility. read more

My mother was released on June 27th 2016 after spending 2 months under the care of a professional and caring staff, She was sad to leave and wishes she could live there permanently. Unfortunately she had to move to her new assistant living home. I am extremely happy with the care and attention my mother received at Pacific Post Acute center. My father-in law is currently staying there and the yelp pictures and reviews are very misleading. Though, I do believe they are professionals. The place seems extremely run down. There was hardly any staff around to provide a greeting etc. The patient's food is barely edible and the facility exits and all the elements around the hospital seem below standards. Entry ways, seem to be a hazard. The pictures are completely misleading and they are clearly advertisement pictures. Though, I have seen some improvements in his health, I would not fully credit the staff there for it. In fact, staff is barely visible when we have been there to visit. These prior reviews seem false to me! I am against unethical reviews. The health board needs to walk through this facility These reviews seem placed and they make it sound like club med of facilities. It is far from that and do not believe all of these reviews. read more

This is an amazing facility--from wound care (pressure ulcers) to physical and occupational therapy. The staff, especially the Director of Nursing (Liza Salcedio) and the physician in charge, Dr. Garcia, as well as the entire PT staff and nurses, have helped my elderly friend regain her physical and emotional strength. They care so deeply for all of their patients. The degree of caring and compassion is so impressive. We have used a number of rehab facilities in the area, and this one is, by far, the absolute best. read more

After a total knee replacement surgery, I needed to move to a facility with a first rate rehab. team. Pacific Post-Acute met the requirement and provided excellent care. The physical and occupational therapy staff (Daniel, Tom and Carmen) were outstanding. I am sure that my rapid recovery is due to their excellent work. I don't usually post, but I felt compelled to do so as this is a very important subject when it comes to our loved ones. My mother lived with me for 10 years until she got to a point that she needed more medical care than I could provide her at home. I never wanted her to go to a nursing home but I did not have a choice. I am a big researcher on any subject, and when it came to my mother, my best friend, I spent endless hours driving all over the city going to visit different places. I must have gone to at least 15 facilities, each one was worse than the other. I then went to Pacific Post-Acute and I was so impressed with how clean and nice it was. I couldn't beleive it. Their staff was so nice and friendly. Their director, Tanya Papazian, made the entire process so nice, she was very informative, and she helped me with everything. She works very hard to make sure all her residents have what they need and her staff clearly loves her. The rehab helped my mother. I went to visit my mother 7 days a week for an entire year, until she passed. If anyone can testify to the hospitable, cleanliness, rehabilitation qualities of this place, is me. I recommend this place to anyone who needs to be in this type of facility for any reason, you won't find better. read more
