Rule #1 - Don't pee before you come to your appointment. They always request a urine sample and I've been "empty" a few times. It's embarrassing having to go to another patient in the waiting room and begging them to "top-off" my empty sample cup.

I am writing to document the timeline and details surrounding my medical care that began on…read moreNovember 28, 2023. During an initial consultation, Dr. Philip Bosch diagnosed me with Interstitial Cystitis, a diagnosis made without conducting any diagnostic tests. Following this, I was advised to undergo bladder instillation treatments twice weekly throughout December. In January, I advocated for further investigation and requested a bladder cystoscopy. The procedure subsequently revealed no abnormalities within my bladder. Despite this finding, Dr. Bosch suggested that cystoscopy was not the definitive method for diagnosing Interstitial Cystitis. Throughout this period, I consistently reported persistent pain during my instillation appointments, yet I was repeatedly assured of my eventual recovery and healing. As my symptoms continued without improvement, I eventually suggested that the source of my discomfort might be ovarian rather than bladder-related and requested a referral to a gynecologist. Consequently, I was referred to Dr. Karen Kohatsu. Regrettably, Dr. Kohatsu also proceeded without conducting comprehensive diagnostic tests to ascertain the underlying cause of my symptoms. My bladder instillation treatment continued for approximately ten months under this course of care. My situation significantly shifted when I sought a consultation with Dr. Daniel Burgess, a surgeon based in Encinitas. Dr. Burgess demonstrated a thorough understanding of my symptoms and, despite not being a gynecologist, promptly recognized the possibility of Endometriosis, facilitating an immediate referral to a specialist in that field. Following extensive diagnostic testing by the referred gynecologist, I received a confirmed diagnosis of Stage 4 Endometriosis. This advanced stage of the disease necessitated a complete hysterectomy, oophorectomy, and cervix removal. Post-operative findings tragically revealed that the Endometriosis had also spread to my rectum. It is my profound concern that an earlier and accurate diagnosis could have potentially prevented the extensive progression of the disease to my rectum and necessitated such invasive surgery. The prolonged period of misdiagnosis and the diagnostic approach taken during the initial phases of my care have led to significant distress and a far more severe and irreversible treatment outcome than might otherwise have been necessary.

He is kind, attentive, and extremely good at what he does. I trust him with all my treatment. When he performed my vasectomy he was meticulous and thorough. I followed his instructions and had almost no pain. Two days later I went for a 1.5 mile hike. Based on my experience I would highly recommend North Coast Urology and especially Dr Frasier.

These were out of pocket procedures but told they would prepare my surgery. Dr Shapiro with another Dr performed what I was told was a successful surgery ( 5 hours w partial hysterectomy). After two months I noticed by body still had the prolapse but in addition I was using the bathroom more at night, sex was painful, and I'd lost a fair amount of strength as not able to lift any weights after my surgery. Bottom line is I was better off not doing anything. Was hopeful and the Drs were friendly but the bottom line was I was worse off than before I went. Dr Shapiro suggested I return to using a pessary which I was before- thx to a nurse at tru-care who suggested I find a gehalte Dr at Scripps. Sadly on Medicare I went to tri-city which is where these drs work from. What could improve? Vote to improve hospital, better communication with drs. as I was never told there was a high percentage or any percentage of not working. Just told not using mesh.
