Although I made a following appointment with her last year for this current year, her office asked me whether or not I had made an annual appointment for a follow up and I had to tell them, that yes, I did. Dr. Clotfelter has literally changed my life, and is one of the main reasons I trust Doctors of Osteopathy infinitely more than an average PA or MD. They did not even give priority to fill them in. Once they had it done, it was not sealed so I asked for that otherwise I had to gi back again. They really do not understand the importance of these documents and how this can affect people lives.
