This facility is horrible! The staff is rude, the head nurses will not be accountable for the CNA's and nurses behavior. Response to patients needs is inadequate and not timely. She's supposed to be there to get better, but she was declining until I took it up with the case manager (who was the only responsive person,) Do not send a family member there unless you can be there daily or you have someone there as a patient advocate. read more

This state needs to intervene. This place is not safe. It is a place where your loved one will most likely be neglected or fall due to negligent caregivers who are unqualified. I don't know how this facility is able to operate and needs state intervention immediately. This is not a rehab center. This is a place getting insurance and not doing their jobs. The food is low level. The level of care is incompetent. My friend fell here and received no gainful rehab. The nurses come by and slide their food down, laughing and talking to each other and don't even help the patient open their juice, etc. My friend ended up falling and had to be moved to another facility immediately. Do not take your loved one to this facility. If you have taken your loved one to this facility and did not receive competent care, report them to the state agency who can take action. read more

I was the primary care giver for one of my Loved Ones. I can not say enough about the incredible level of care that they provided for her. We've been to and through other facilities in the Tallahassee area, and there is no comparison. This facility and staff is beyond top notch. The quality of the staff, thoroughness and compassion is second to none. They went over and above to help me/us out when we were in a sticky situation with care and I can't give enough Thank Yous and high grades. The facility and rooms were EXTREMELY clean, and of high quality, especially compared to some other facilities in the area. Specifically, Nurse Pete was astonishing, and you are very lucky to have him on your staff. He was consistently over and above every second of every day. I'd give a 4 and not a 5 with the only challenge being the billing department. Needs some improvement there. DO NOT take your loved one to Pruitt Health in Southwood Tallahassee, unexplained severe bruises and welts and definite neglect. I wouldn't wish this place on my worst enemy. Stop don't go! My experiences and visits to this "rehabilitation facility ", leave me concerned for the welfare of the patients and residents there. This place is woefully understaffed. Most of the staff I encountered were talking with each other and complaining and not engaged in their job. When I called the facility directly, rarely was the call answered. When I could connect with an administrator, they were somewhat helpful. But, the absence of leadership in this facility and this organization is evident by the disengaged staff and the despair of the patients. I felt as if each patient I observed was simply waiting to die... slowly, painfully waiting to die while the staff denied their presence. There is a buzzer to get in or out of the facility, but often there is no one there to allow entry or exit... so you're unable to get inside or you're locked inside! My loved one, with lack of fine or gross motor skills was not given assistance in eating, not given a straw to drink from, and when the drinks were spilled, left in wet clothing. He reported being left to sit in feces and urine for hours on multiple occasions. He reported being pushed to a room and left for hours with his wheelchair shoved up against a pole unable to see around him or move. I witnessed no activity or engagement on any visit, just people in wheelchairs at tables with empty styrofoam cups. My loved one called almost every day screaming about the horrific treatment. He shared that one staff member daily degraded him, removed his food before he finished, slapped food off his lap, cursed at him and screamed that he made a "f--king mess." I lost nights of sleep concerned for my loved one's welfare. I called the Pruitt Complaint line, it never picked up. I called the regional administrator, he didn't return my call. On my last visit before my loved one was moved, I saw an old woman who had been sitting in a huge wheelchair, in the same place, for hours. I sat and talked with her. A staff member was sitting behind her, on his phone, completely ignoring her the other patient next to her. I was drawn to her and stopped to speak. She cried as we spoke. I said goodbye to her and left crying. Hurt that I couldn't change this situation, crying that people who are said to be caregivers are so negligent and dismissive and cruel.
