However, their required documentation consisted of 2 sections. The first seemed a fairly straight forward medical consent. The second was a credit card agreement consisting of several pages (3-4) of legalese which would require an attorney review to safely sign. I have to wonder what they've been caught doing that they need this kind of an agreement. I left without signing anything. Can't say much about their actual service since I have no other experience so gave them 3 stars. Will have to contact Dr to see what other options there are.
