I've been waiting for my scheduled appt for 2 weeks now. I get a call today from Evelyn who states she has to cancel my appt bc they don't take my insurance. I checked my insurance and Dr. Slavin is in network so I call Evelyn back. She realizes my PCP sent her the wrong info so she sets me up again for tomorrow. A few minutes later she calls and I can tell she doesn't know how to tell me that she has to cx my appt because "we cannot see you according to my manager", and she is very apologetic. She then says that her manager will be calling me, so I ask the name of the person who will be calling me, and she says "Mrs. Slavin". I tell her "I'm sorry you will be hearing me but I know what's going on. She won't let Dr. Slavin see me bc I'm an ex-employee. Mari (Mrs. Slavin) is petty, unethical, and unprofessional. You can tell her I said that. And again, I'm sorry you got caught up in this". I worked for Dr. Slavin for 6 years. I never stole from him nor did I do anything illegal. I took care of his patients and went above and beyond to take care of his patients. And now his petty, unethical, unprofessional wife refuses me services. I need his services as a patient, I'm not there looking for work. What happened to the Hippocratic oath Dr. Slavin took? What happened to "I will treat the sick to the best of my ability", or "I will remember that I do not treat a fever, chart, or cancerous growth (or an ex-employee), I treat a human being", and last but not least, what about "may I long experience the joy of healing those who SEEK MY HELP". I need his help but apparenlty, Dr. Slavin's wife, Mari, threw his Hippocratic oath out the window.

I always arrive at least 15 minutes before my appointment so that I am on time. I ended up being there 4 hours! Unacceptable! The paperwork was tedious and I couldn't understand why I was asked questions that did not concern my reason for the visit. I did have an exam that was very extensive and found that he discovered several things wrong that I did not know. Including an answer as to why my legs ached. I was given paperwork to make appointments for labs and xrays. When I called to make one of the appointments and stated that it was from Dr. Leonard the office started laughing. Well that did it and now I am searching another physician to assist me.

We arrived and went inside. The place looked so old and not that nice. I went up to the window and said I had we had an appointment. They gave me a bunch of papers for my husband to fill out. All I noticed in back of the secretary were a bunch of files just stacked all over the place. All over the floor and just all over the place. He finished filing out the papers fast just so they could tell us the doctor was not their but at a call at the hospital and would be about 30 min. My husband hates waiting and said if he didn't arrive in 15 min we would leave. We ended up waiting the 30 min and he went in and saw the dr. They gave us referrals to get X-rays and blood work done and that they had sent a prescription sent to our local walmart and wanted my husband to take the medication before the next appointment. Once we left he looks at me and said it was the worst doctors office he had ever been too. Everything was so unorganized and dirty. They had to go through 3 rooms to find a clean one they could use. The prescription never made it to our local pharmacy. We called them asking if they had sent it. They would always say they would get back to me, or that they would send it. They never did. We went back after the lab work. Just got charged a couple of hundred dollars just so my husband would get prescribed another medication similar to aleeve and his back pain is still there. Never again!

Please don't visit this practice because it is no longer open. It has a different name now and I don't know what they do. He was sending his patients to his father who would provide the medication and that was a big issue and also seeing patients and overcharging the insurance so he had to close his practice.
