They take his remote control for the TV and it somehow gets lost. They don't take care of patients, this place is awful. They don't care. No one, I mean no one ever calls you back or responds to emails. They have several complaints and will be investigated.

All of them helped to make my brother as comfortable as possible. At the end, they were professional, caring and supportive. The staff helped with physical and occupational therapy and swallowing therapy. The nurses and aids were always polite and friendly to the patients. You can not ask for a better place to send your loved ones for either rehab or comfort care. The free small bakery was a nice perk. I don't know what the problem was with the previous reviews but the ownership has changed and the staff was great.

10/2015 I am reviewing Lakeview while my family member is…read morestill stuck in this facility! I would like to take them out of this facility but I do not have much say in that decision. The ownership has changed I am not sure when because we we're never notified, even after 3 yrs of being involved with this nursing home. I have noticed ever since ownership have changed they fired many caring staff members, rather than hiring more staff. Instead they decided to do a makeover on the 2nd floor, you would think that was a good thing. However my loved one who lived on the 2nd floor for 2-3 years that they would keep them there. Since they were familiar with all the nurses, aids and their room! Not the case, my family member who has Alzheimer's was moved (basically kicked out) to the 3rd floor. Which by the way they never plan to fix, never mind about hiring more needed staff. Ever since my loved one moved to the 3rd floor it has been a nightmare. They ended up getting a very bad non healing bed sore, due to neglect. Even one of the staff members warned us quietly about the facility being neglectful (that staff member was probably fired later). It was a continuous nightmare from there. Fast forward 10 months later, as of 2 weeks ago my loved one now has a 2nd bedsore! When we called to fight with them about it they ignored our calls. I was calling the assistant of the director and she failed to call me back (I get that feeling the director forbids some of the staff members to be in contact with us). Instead she had a nurse call 2 days later! I did not want to speak to the nurse, I wanted to speak to someone in charged! But that never happened. Today my family member's child went to check on them 2x. Tonight they found their loved one drenched in sweat, hunched over the bed. Not one staff member noticed! The patient's child did say there was hardly any staffing tonight, so who's fault is that? I think there are a few staff members who genuinely cares about the patients, but majority of them I get the feeling just doesn't care. Why should they if they are understaffed, mistreated, perhaps under paid and it seems to me the director doesn't even give a damn? But that is no excuse for a patient to be left sick, hunched over their bed soak and wet in their own sweat, that is just unacceptable! My family member recently is very sick (again we believe due to neglect) and nobody really knows why. We have asked numerous of times is it pneumonia, a Uti, upper respiratory? Nobody seems to know. It's been a week and still nobody knows (even after supposedly test). Yesterday, the patient's child noticed the nebulizer was not working in order to treat their condition (whatever that condition may be). But the aid and nurse continued to give them the same treatment with the broken equipment. Until the patient's child spoke up. Their response "oh it must be broken" but they continued to give the treatment with the broken equipment and never bothered to get a functioning one. Not until the child complained on day 2. These are just minor ridiculous experiences with Lakeview but it's the same old story month after month. Tomorrow I will have to contact them yet again to argue with these people about my family member's condition and the fact that they are neglecting my loved one. The director is a rude cold son of a b. If I had power and money, I would sue him, for harassment, neglect & stress. He does not care about the patients, or the patient's family. The director does not know anything about the patients, I bet not even their names (a previous staff member would say hi to any patient that pass him by, name and all, he left). I never saw this current director say hi to not one patient. He had harassed me and my family while we we're having a peaceful visit with our loved one. I can't say why because I am afraid for the safety of my loved one. I am afraid the director made me very suspicious of his and Lakeview's intentions. This is why I can't say if the patient is a male or female. Or if the patient's child is a son or daughter. I will probably have to report this facility to the bbb, send out more reviews and pobably speak to a lawyer. We are not even those kind of people, we just want our loved one to die in peace and dignity! So I warn you to not put your loved one in this facility if you want them to be taken care of. Before Lakeview's ownership changed, the director and the staffing was actually more than ok, for a nursing home. However after our experience in the last year with the new director and ownership my impression is that Lakeview just does not care about the old or dying. They do not care to see these people die with dignity. My loved one is on their last days and this is how they will go, sick, drenched in piss and sweat hunched over their bed. Tomorrow I will speak to another facility to see about other possibilities, perhaps closer to where we live. I wish this on no one. Tomorrow I will fight.

I went to Leisure Physical Therapy for treatment after a distal radial wrist fracture. The first few sessions were competent at best, albeit as I discovered later, the evaluation process was less than scientific. Subsequent sessions were routine, amounting to basically heat application and hand strengthening exercises, which I could do myself. There were a few other activities that involved using the organization's equipment, again mostly done independently of a therapist's help. After 4-5 sessions I decided to seek treatment elsewhere and went to St. Charles Rehab, which actually accepted my insurance and as I discovered in the first session, did a comprehensive assessment of injured wrist/hand in terms of strength, range of motion, etc using a variety of tools, and carefully documented results, including progress notes and updates in each subsequent session. The occupational therapist who worked with me at St. Charles was extremely professional, conscientious and clearly invested in my progress to as close to being fully functional as possible. None of this was done at Leisure Physical Therapy, which operated more like an assisted fitness center for seniors ( one size fits all approach) than a professional medical facility. Really very disappointing experience and would not recommend. On a side note the OT at St Charles Rehab was exceptional in another respect. The therapist treated me like an individual, intelligent person, not some incapacitated, "senior citizen" who couldn't think or articulate their own thoughts clearly. Although I am only 64 years old and easily more articulate than people a third my age, my experience of "senior" medical care facilities or any medical facility's treatment of seniors is that office as well as professional staff are frequently condescending, short, dismissive, even rude. The people working at these practices have no business being in the business of patient care.

Not long after I submitted, I received communication from Shirley and spent the end of last week running around to get diagnostic testing redone, etc. At no point did Shirley or the Indeed posting indicate "the facility has been known to be slow on interviews it does take them a few days to review" it wasn't until I withdrew my application via email that this information came to light if this was not bad enough, she had the audacity to tell me in the same email response she states " I did not hear from them an interview confirmation so I did not say anything" makes the entire interaction even worse and reinforces my point about a lack of transparency, consideration, and respect, which reflects very badly on her character and, subsequently, Apex. Sending a quick update, such as "Hey so-and-so" or whatever greeting you choose, followed by "I've submitted all your information to the facility. Once I receive an update regarding an interview date, I will let you know," or something along those lines, shows you respect the applicant and their time. Choosing not to say anything, as she did, and making that admission (which I would have kept to myself) indicates a basic lack of respect for the prospective applicant. There is a stark difference between miscommunication and non-communication, and it is apparent that she does not know the difference. In this case, this was non-communication on her part, but she says, "it was miscommunication," and, more importantly, there was a lack of transparency from the very beginning. She deflected, blaming the facility and/or me for her lack of candor/professionalism, when it was her/Apex responsibility to be transparent, which would have allowed me and other applicants who value their time to make an informed decision, but Shirley prevented that. When a person hides important facts and prevents you from making a smart choice about a job, it is called job scamming, fraud by omission, or deceptive recruitment. That is what Shirley did by preventing me, and potentially other prospective applicants, from making an informed decision. I have chosen to post these interactions as a matter of public interest and to warn others about potential interactions they may have when dealing with Shirley Tao. Some may ask why I gave it 2 stars if the interaction was this bad. The reason for the 2 stars is that A. Mahamed from H.R., whom I spoke to on the phone about this matter, was very pleasant to deal with and showed compassion, understanding, and a legitimate desire to resolve this matter. But I can't stay silent and not make others aware of these interactions with Shirley, because then I am complicit and a hypocrite for discussing issues of transparency and then not being transparent myself.

Her business is spotless, our appointment was right on time and she is a very nice person and considerate with your needs. Natalie also understands that any kind of therapy can cost a lot and will work with you in anyway she can. Last but not least, she is an honest woman. The only complaint is that I was told my mom would be given her pain medication regularly as prescribed, but she was not. Unless she asked for it what she did didn't because she has dementia so she missed several doses and was in pain. Other than that quality care. The in-house hairdresser was amazing. Made my mom look 20 years younger. And lots of recreation.

I would not recommend this facility…read more When a facility is unable to have sufficient staffing levels, it is impossible to provide care in a timely manner. (On one weekend all the aides called out for the overnight shift, so an aide was pulled from another unit to help. With 40 beds in Mom's rehab area, it was tough.) Aides should not have to hunt for supplies when preparing to change a patient. Large sized briefs, barrier cream and garbage bags should not be in short supply in a nursing facility. 95% of the employees here are good souls. Their heart are in the right place, they care about their patients and attempt to do the impossible. The kitchen team is incredible. They offer so many options to patients, produce delicious food and are incredibly responsive to requests. As someone who stayed with their loved one - leaving only to shower and briefly nap - I have the following recommendation to caregivers. - Keep a careful eye on prescribed medications - Visit frequently - Check skin integrity frequently - Engage with the dietician and ensure personnel delivering/recovering food are aware of current meal plan protocols. Dietician does not reach out to patient/family to inform them how things are supposed to work. You will need to ask. - The PT program is not a rigid structured time setup. It is effectively first come first serve. Which may result in the therapist who is familiar case being unavailable when the patient is available. - lack of visibility into prescribed medication as well as report of medications actually dispensed, needs significant improvements. Key personnel administering medication have experienced significant unexplained turnover, which is somewhat in step with facilities personnel charges at all positions. When Mom was last released from a hospital, our family opted to bring her directly home instead of risking another experience like the one at Island Nursing.
