After I was…read moreable to leave the hospital I ended up staying at Santé for years (off and on) & it's the only physical rehabilitation facility that I will go to. I have had the displeasure of staying at 2 different PT rehabs before I ended up at Santé & you would not believe the horror stories I have from those places, just unbelievably awful (Both stank of urine & feces the moment you get passed the front desk.) & Sante was like a Godsend for me. Let me start with the facility 1st. The place is very nice. They have a Starbucks Cafe, a full gym staffed with friendly & knowledgeable physical therapists that will work with you everyday, a full restaurant, and most importantly, HUGE SPACIOUS INDIVIDUAL ROOMS equipped with a flat screen tv, a mini fridge, a kitchenette, a microwave, and a nice big full size bathroom for my wheelchair. Next let me tell you about the staff. Everyone there is so friendly & polite. I stayed there for years & most of the staff stayed there and are still there since I first went to Santé in 2019 (That should tell you something. Most nursing facilities are like a revolving door when it comes to the staff). I'd like to mention some names of people who really became friends of mine while I was there. Marlin, Brian, Aiden, Catalina, Sharon, & Kimberly from the CNA team are all excellent at their jobs & true professionals. These guys went over & beyond their duties for me & I thank God for their help, they truly are the best (You need to trust me on this, at other places I've had criminal's & people who just don't care & I never got that at Santé. They were always good to me). From the nursing staff id like to mention Betty, Jose, Fennela,& Elizabeth. These nurses are top notch. They actually care about the patients & they don't treat you like a person that they have to deal with, they treat you like a person & that is so rare in healthcare especially post covid. If I had to complain about anything it would be the food. When I first went to Sante in 2019 they had an extensive menu with lots of options & the food was actually good. I left for a year and when I came back the kitchen had really gone down hill. You know it's not good when the employees don't eat from there. I talked to the main chef Daniel & he told me they cut his budget drastically & the quality of the food suffered as a result so it's not his fault, corporate is always looking to save a buck & they shouldn't do that with the food, it's kind of important for those who have to stay there for long periods of time. I was able to eat there everyday if I wanted to but I ordered delivery most days because the food was nasty. There are so many others I am forgetting to include but the staff for the most part is amazing. If you are looking for a nice place for yourself or a loved one look no further. It doesn't get any better than Sante of Mesa. Other places will put on a show for you & trick you into staying there thinking it's a nice place & then when management goes home the night crew will come in and they are terrible, sometimes even abusive. This doesn't happen at Santé, they even have couches in the rooms with pull out beds so you can have a loved one spend the night with you, they welcome it because they are fully staffed at nighttime and don't leave you with a skeleton crew like everywhere else. At least that's been my experience. Last but not least I'd like to thank Kristina the maintenance manager! Kristina is an angel walking around on earth. She totally gets me & we were friends (she is another 1 who has been there a long time.) She will make sure everything in your room works & she keeps the place nice. Management is also great! If you do choose Santé' make sure you don't get Irene as a nurse. She is a nurse at Santé who doesn't belong. She is the only one I ever had a problem with & it was like she liked to pick on me. I honestly never did anything to this lady & for some reason she was always rude & unprofessional. Whenever I would ask for her she literally took an hour or longer to get to me if she ever came at all. I would ask her to change my folly catheter in the morning time and she would blow me off all day & try to pass the work on to the night nurses. The worst part about working with her is when I would ask for her help I could actually hear her just talking & laughing with the staff instead of attending to my needs. She made some of my days very bad & I always loathed the days I had her for a nurse but she is the exception, all the other nurses there are great, especially the ones I named. If you have any questions feel free to ask me, I will answer any questions truthfully because I know how important it is to put your loved ones into a good nursing facility & God willing I never need to go back but if I ever need to stay in a nursing facility again Sante is where I would go.     Went in March for a hysterectomy. Outpatient doors were closed said to go through the ER. Go to…read morethe ER they make you walk all the way to regular administration who's doors are open with a closer parking lot then the ER. I have a port because I have Multiple Sclerosis and receive my medication monthly by infusion, they refused to use it without an Xray. They also made me pee in a cup to prove I wasn't pregnant...it takes 3 appointments, waiting 30 days and a biopsy that is the most painful thing I've ever experienced in order to get a hysterectomy, who would get pregnant in that small window of time where I took 2 other pregnancy tests AND had an ultrasound. Plus since it was surgery I hadn't drank anything. Super annoying. When I came to the nurse who took care of me was great, very easy going, was prememptive with care like had a heater thing ready for me and water, explained everything, his energy was very good and calming, perfect for what was happening. I chose to stay the night in order to have pain management because left to my own devices I let the pain go too long and it causes problems. My first nurse was Coraline. I had to pee as soon as I got there, I asked her and she asked me to wait 5 minutes. Luckily for me the nurse who does the blood pressure, I think her name was Cheyenne helped me and was super sweet the entire time. Caroline did eventually come back but not in the 5 minutes and didn't remember, she did not seem very experienced. Then later in the evening when she came in I asked to have my water filled and she again said later and that I had gatorade, my mom had gone and bought snacks because Caroline couldn't find a phone for me to order food and she did place the order herself but obviously I'm not going to page the nurse everytime I need something. A very nice man, the one who transported me, saw my mom with the bag found us a phone and apologized that we bought everything he seemed pretty irritated that this wasn't taken care of by the nurse. I didn't get my water refilled until the night shift when the blood pressure guy Chris came in, he was also great. I don't remember my nurses name that night but I really liked her, she was very friendly and competent perfect care after a very long tiring day. Checkout was easy. I just really wish they had used my port because I would've been far more comfortable and that's why it's there. Coraline could also use a little help in making sure patients are comfortable, she's the only one who didn't seem to recognize what was going on.

They did bloodwork, a CT and an ultrasound. I was waiting to hear the results. A discharge nurse came in, with paperwork to discharge me. The doctor had not even come in to tell me what was wrong, and give me treatment options. I had to demand to talk to the doctor. He told me I had gallstones, but it didn't require immediate surgery. Granted, I already had surgery scheduled for 7/6. The doctor said it could wait, prescribed me pain and nausea meds and told me to follow a low fat diet. His tone, when telling me about the diet, made me feel like he was criticizing my intelligence regarding this. But t, I'd already been doing this one year now, for weight loss. Friday, 7/3, I got in touch with my surgeon. She got me on the schedule that day. The next morning, she said my gallbladder was inflamed. It needed to come out. A doctor should communicate with their patients, giving them all information necessary, so the patient can exercise their right to choose the treatment they want. I wasn't given this, without having to advocate for myself.

    Our mother was admitted to Well Springs SNF following a hospital stay and was supposed to receive…read morephysical and occupational therapy to help her regain strength and independence. The evening she arrived, she was still able to walk. Unfortunately, the therapy she was supposed to receive--at minimum 3-4 times per week--rarely happened. Sessions were short, inconsistent, and often skipped entirely. Instead of helping her improve, staff frequently left her lying in bed or sitting in a wheelchair for most of the day. Over time, her muscles deteriorated significantly, and walking became nearly impossible. Eventually, she could no longer take steps without the assistance of a walking device. Basic care was severely neglected. She was not properly bathed or given adequate peri-care. Staff tried forcing her into Depends, despite the fact that she had never needed them in her life. As time went on, it became obvious this was simply because staff did not want to assist residents to the restroom. There were many times she would call for help and wait 30-45 minutes before anyone responded, often resulting in humiliating accidents. Her medications were mixed up on several occasions. We also witnessed feces- and urine-soaked bedding and clothing being left in her room and even thrown onto the floor. Our family met with management, including Heath, multiple times, but despite plenty of promises, nothing ever changed. We ultimately removed her from the facility, and within 12 hours she had to be hospitalized with a severe blood infection caused by unsanitary conditions and neglect along with bed sores on her back side. The night staff were by far the worst. My mother began recording the amount of time it took for someone to answer her call button because the delays were so excessive. She also suffered falls there--despite never having fallen before in her life. On multiple occasions, night staff left her lying on the floor with only her own pillow placed under her head while waiting for another employee to help lift her, sometimes for 20 minutes or longer. There were also several times our family arrived and found all the doors locked, forcing us to wait extended periods before anyone answered. Each visit, our mother appeared more neglected and distressed because of the poor care she was receiving. Late at night, children--presumably employees' kids--would run through the halls loudly, disturbing residents. Her water containers were frequently left empty and dried out. This place was an absolute nightmare. We personally witnessed verbal abuse from staff toward other patients in common areas. Based on our experience, I strongly urge others to stay far away from this facility. Their negligence nearly cost our mother her life.

    I am a CNA of 20+ yrs that picked up a PRN shift Christmas Day 2024. The nurse I worked with was so…read moreawful & unprofessional I returned to Wellsprings the following morning waiting for them to open to report this nurse to HR. The nurse I worked with was VERY unprofessional in his remarks, attitude, & tone toward me. This nurse did not help me with fulfilling patient needs when I specifically ask him. This nurse made a point to tell me 2 times that "ya know, I am a good nurse". Never asked him if he was, this came as responses to asking him for help by grabbing more bleach wipes when I was in full PPE in a room assisting a resident. He never helped me once. This nurse created issues between a patient & myself that did not exist. He would not let me speak. He believed his conclusion. The resident corrected the nurse in his wrongful accusations toward me. Another incident while making rounds, I knocked on a residents door to check on her again. This nurse was angry with me because he was in the room I "disrupted him while talking to the resident". This nurse was very egotistical. I didn't understand why he was so upset with me. I had not done anything wrong. This nurse scolded me again & told me to stay out of this particular patients room that the issue was with. That I'm never to go back in there. I didn't understand why or what happened. A couple minutes later this nurse completely flips out & starts talking VERY loud in the common area where all the residents & their families could hear & see him and loudly said that "he doesn't trust me with any of these residents!" Then this nurse, not a manager, not HR, but this LPN nurse tells me to just leave now & go home that he doesn't trust me, to stay away from all the patients!! Of course it's nearing the end of the 12 hour shift &another CNA had showed up. But everything was ok until he knew he had to cover his lies & get rid of me so the truth wldnt be exposed. He wldnt let me speak or listen to me. He would talk over me or cut me off. I was absolutely shell shocked. This was my1st time working at this facility & I did a stellar job, especially having the limitations of it being a busy holiday shift & many families were visiting. I did my job & I ALWAYS assisted & cared for the residents as if they were my own family. The issue was a nurse that needed to cover himself & the mess he alone created & wasn't going to hear about anything else. He could have simply said he was wrong about what he thought had happened & I would have said it's ok, no big deal, water under the bridge. Now, I am happy to write this truth on the staff & the lies they will create to shift the blame &create scenarios that never happened. The Nursa app has evolved & now we can see the reviews left on our performances. Wellsprings of Gilbert has even left me a negative review about my quality of work. I was very angry at how I was talked to & treated by this nurse during my shift & I expressed that to Wellspring HR. I told them to watch their cameras! The resident did correct the nurse of what he thought happened or said but he didn't want me to speak to that resident again. That's when he flipped out. I am absolutely shocked that their management believe lies over truth. There were so many other incidents that happened with this nurse during my shift I would just shake my head in disbelief & continue doing my job. One that I will note is one resident was new to Wellsprings from the hospital. This resident had a couple incidents that pointed toward having C-Diff. I made the nurse aware & suggested to put a contact precaution on the patient's door so if in-fact she was positive for C-Diff, anyone who entered would know to take the proper precaution & prevent spread as it is highly contagious. I ask the nurse 2 times about doing this. He was angry at my suggestion &said he didn't like me telling him what to do. I told HR about this also. I explained to the nurse and HR I'm only reporting from previous experiences I have had with patients & hoping for protection by taking an extra precaution. This nurse never took a test to send off to see if the resident was positive nor did he put up a contact precaution sign at least until a test could be done to be determined. The resident did have several episodes & symptoms that pointed to C-Diff. I just report what I see. I was met with being scolded with egotistical remarks. Shame on you Wellsprings for turning on me. I brought this to your attention! I am a good, honest, hard working CNA. Yes, I was very angry & said colorful words as adjectives of behaviors I experienced, never direct name calling. I shared my experience with other healthcare management & they all agreed that I should have reported Wellsprings & that nurse to the AZ Board Of Nursing. I regret not doing that. I'm not perfect but I know God saw ALL of it and I can stand before Him blameless in your accusations.

My brother is autistic and requires clear verbal guidance and assistance with paperwork. He went to this facility for an outpatient therapy program but was mistakenly processed into inpatient care. He clearly told the front desk he was there for outpatient therapy and even asked them to confirm. Despite this, staff instructed him to continue filling it out without clarification. What's even more concerning is that his therapist, Eddie who has seen him before continued the intake process without recognizing or correcting the mistake. Meanwhile, staff called my mother saying my brother never showed up for outpatient therapy, causing panic when he wasn't answering his phone. An hour later, they called back to say he had been admitted as an inpatient. My mother asked for an explanation, Eddie eventually said that if my brother is checked out, he would call the police and that he could be placed somewhere worse where we wouldn't be able to find him. That kind of response is completely inappropriate and unacceptable, especially given my brother's condition. We went to the facility in person and asked for a supervisor. He was the only staff member who showed any compassion, but ultimately said there was little he could do beyond reporting the situation. When my mother spoke with the director, she saw nothing wrong with what had happened. There was no accountability, no resolution, and no corrective action. This experience was deeply upsetting and handled with a complete lack of care, communication, and professionalism.
