    Absolutely not elderly bedridden bed sores not attended to medically.Horrible wound care as well Just an awful, awful place.

The facility is beautiful in a Tuscan theme. The staff is amazing, they are the nicest empathetic group of people. The food is a good variety. The only qualm I have is my dad isn't able to feed himself and has been going without food. He won't ask for help because he doesn't want to bother anyone. I would recommend this facility to anyone and everyone. Thank you for taking the best care of my dad.

    My 92-yearl-old dad stayed and rehabilitated at AHC (Sunset and Jones location) for a couple of…read moremonths. He had suffered stroke-like paralysis on one side for both his arm and leg. We thought his recovery was an extreme longshot at best. However, thanks to the truly superb staff, he was able to slowly but successfully regain his ability to walk as well as a lot of his abilities to be somewhat independent. This would not have been possible without the amazing medical assistants, nurses, physical and occupational therapists, wound care specialists, case managers and even their maintenance man. When dad first arrived, and for most of his stay, he was completely dependent on AHC staff for bathing, toileting, getting food, and all. The staff was so attentive, patient and understanding (he did get grumpy and stubborn at times as he was dealing with a lot of medical issues;-) They never missed a beat and motivated him to keep working hard and improving. Without hesitation, I highly recommend this AHC for rehab. A few names I must recognize: Himani, Jordan (therapists), Mary, Janet and Chanel (the CNA's), Luz the head nurse, her daughter the woundcare specialist (forgot her name:-) and Arnelle the maintenance man. Arnelle didn't even have anything to do with patient care, but he had such a positive, smiling and motivational presence and got to know our dad and kept him smiling and laughing on a daily basis which is hard to do when someone has just lost their independence and is in pain. A huge thank you to the entire team; they are really like a family.

In general I don't post negative reviews and…read moretry to focus on the positive, but after 2 weeks here we were given 24 hours to move our 94 year old immobile mother out of the facility or pay $600/day. I'm not complaining about that so much as the lack of notice, the lack of explanation on what our options were and the utter lack of concern about the stress surrounding all of it. They should know more about how the process works than I do - and they either don't, or don't care enough about the family the patient or the process to explain any of it. I know more about how Medicare, medigap and supplemental insurance than the management of this facility after going through this process one time. The care here is good enough. Some wonderful nurses and some impatient ones. But it's a tough job and I appreciate anyone doing this for a living. But understand the process and be ready to be thrown out with no safety net and no help figuring it all out with literally no notice. Once insurance because an issue they were relentless over a 24 hour period about getting her out. OK - I get it, nothing is free. But they know the process, we don't and they really offered no help in what to do next since she couldn't go home. PS : our mom passed within a week of getting kicked out of here and honestly the stress of it all was a factor.

My beautiful lady, Mary Lou was here as a guest during her transition from being at home. Unfortunately, she's at the moment now where she needs assisted living and that extra care. It's been a long and stressful time. Rough couple of months. Finally, while this was not permanent the staff and facilities here truly were a "god send!" This is a place where your loved ones are in great hands. The rooms primarily are not private but accommodate two individuals. They are co- ed. You never know how long your neighbor in the next bed will be here. The telephone and television are on both sides of the bed, but the loudness can interfere with the patients comfort level. The doctor and nurses here are so overwhelming great the way that they handle the patients care and pain levels. The patient, even Mary Lou, my angel, does not want to be here. She wants to go home . There is no more home for her. It's assisted living. They make this transition easier for the patient. The visiting hours are longer than normal and all an individual needs to do is sign it. You're allowed to bring outside food and drinks for patients . You can decorate and adorn their space with flowers and other trinkets to make them feel a little piece of home. Love is the focus and makes the world turn. Sometimes we cannot decide where we will be. When it comes to the end we will go in and out of who and where we are ?! My Miss Marylou, my angel you are loved and will be continued to be loved as long as you are here. We will go beyond to make you happy.

My mom has…read morebeen here for several weeks and was transferred from an acute rehab for continued nursing and physical therapy. It seems as though the care provided has been no different than had we put her in hospice... mind you, we are nowhere near that point in her life. Her weakness has only gotten worse, physical therapy has not been done as we were advised it would be, and she continues to lose weight, her muscle strength, and has been laid up in her bed for more than three weeks now with very little movement. She has become stiff and rigid in the position that she's in and is very painful to even attempt to move her into a sitting position. Communication from the staff and her case manager has been extremely poor. I was notified last week on Wednesday that Medicare was only providing one more week of treatment for her and have yet to be provided a discharge date. I have left several messages for the staff with no response and still don't know when I need to set up transfer for her to depart that facility. Based on the timeframe provided, it would appear that I have 24 hours or so from now, yet no information has been provided to our family on when she will be discharged. Her time at this facility has been a complete waste of time and only further set back any progress she had made prior to admission. While Mom says "most of the people are nice," she also said that they very rarely even check on her and never move her. Please do your research to find the best fit for your loved one. We thought we had, and while we understand that skilled nursing is a lower level of care, we feel that there is zero excuse for the lack of treatment she has been provided. It is also important to note that many times when we have been there visiting, there have been other patients crying out for help from their rooms that has gone on for more than 10 minutes before anyone responds to their request or urgency. This is uncalled for! The staff is there to care for these patients and they hardly acknowledge the calls for assistance... definitely not in a timely manner. We are extremely disappointed with our experience and how our loved one has been cared for, or lack there of.

Prior to TLC my father was in at least three "memory care units" where lots of promises were made regarding the patient-centered care that would be provided and the expertise of handling any difficult behavioral problems that come with the progression of Alzheimer's. As a nurse practitioner, I thought I had done my due diligence with researching the facilities. I was wrong. With the covid shut down in March 2020, many of the facilities housing elderly took advantage of the fact that families could no longer check on loved ones physically and a lot of negligence occurred. My father was moved three times during this time as I tried to find the best care. What did Tender Loving Care provide that these other facilities did not? 1. As with any patient with dementia, moving them to a new facility can be very difficult. The transition can cause them to progress in dementia and not recover. It can increase behavioral disturbances during the adjustment period. My father was already having issues with agitation when needing to have his brief changed/bathing so the staff already had their hands full when he came over. Dr. McGivney and Kerri were amazing and worked very closely with me to make changes to his medication regimen to reduce the behavioral disturbances to allow the staff to care for my father. Once he was stable, the doctor would reassess with the goal of reducing medications as needed depending on dad's response. There is not one facility out there that I know that would spend this individualized attention with a patient. The staff is watching the resident daily and reporting back to Kerri and Dr. McGivney on medication response. 2. The staff truly got to know my father and what he liked. When they asked for the list of things he liked they actually used that information to learn how to get him to engage with the community. I would come in to visit and he would be in his "office" writing for work. He would be at the desk reading the newspaper while the staff was also sitting with him engaged with activities. Staff found out he liked to organize and clean, they would have him helping them and encouraging him, meeting him at his level . These were the biggest differences between TLC and other institutions. My father felt safe, happy, and content here. 3. The amazing backyard/courtyard for the residents. Lots of natural light inside the house. Previous facilities promised he could freely walk outside in the locked outdoor courtyard yet at each facility I would find out from my father they were all locked and a loud alarm would go off thus my Dad not wanting to go out. He loved being outside and this was the reason I had chosen those facilities !! At TLC, it was truly open and unlocked so residents could freely go outside in the backyard. This is so critical especially with the COVID shutdown that residents can get fresh air and sunshine. 4. Maintain all the COVID precautions. 5. I could go on and on with all the little things the staff did to let me know how much they cared for my father but there isn't enough space here. Thank you TLC for everything

My…read moremother was in relatively good health, of sound mind, but very frail, needing 24-hour care due to her advanced age. TLC provided this care with exceptional attention to detail, with much affection & respect shown to both my mother and our family. The TLC Staff made a potentially difficult situation less painful for us. We consider our Family blessed to have had the McGivney's & their staff in our lives. Thank you TLC!

I was recently fired and I know I may sound like just another disgruntled employee but this is not the case. The management team are definitely not in the business for the residents. They are very unprofessional in the way organizing. I have never seen so many falls as far as the residents go. They can not keep employees. It is literally a swinging door. I loved working with my fellow employees except for a few, which I know that happens everywhere. This place is in need of new managers if nothing else. They are very nice to you for a little while, and then the true colors really start showing. I was never late, and the reason I got fired was because I got put in the hospital for pneumonia and gallstones. and then was summonsed to court in Utah to help my daughter fight for custody for my grandson. They showed nothing but sympathy until a week later after I got back from Utah and they had hired four more people while I was gone. I understand I was still in my 90 day grace period, but the way they went about letting me go was nothing short of under handiness . I would not recommend working for this company unless you enjoy working around fake people.

    My wife went to Pam from the hospital and started in a regular room. In a couple of days, she was…read moreisolated and I was asked to wear a gown when I see her. No info just do it. It wasn't long and she was getting sicker by the hour. They talked about pneumonia and some other possibilities, but ended up on a ventilator. She was bad when she got there, but got worse in a hurry. I reached out to her doctor 3 times and didn't get a response. The last day there she was on a ventilator and being cleaned in the morning. Nurse told me they always clean them. I see her everyday First thing in the morning and at the end of the day. Never saw her cleaned. He also took a couple of meds off the IV saying that was routine. That afternoon I got a call that they had to send her to Centennial Hills ICU. When I got there 30 minutes later, they had started 3 support lines in her thigh. Main line for drugs, for blood pressure device and for kidney dialysis. In 30 minutes they got that info and started to treat. She was at PAM for 3 weeks to get this bad. I never got day to day updates as I don't believe they knew what was happening. The nurse was cleaning her that morning with no indication of how sick she was. On the ventilator you're sedated. Evidentially they removed the Propofol that morning and by the afternoon she hadn't responded yet. Usually you wake pretty quick from propofol. She arrived that day at the Centennial Hills ICU with 800 glucose-Sodium thru the roof very high, she needed blood as she had developed an internal bleed they hadn't found but she needed a transfusion. This was Tuesday late afternoon. She never regained consciousness. We decided to remove the life support and she passed peacefully Friday afternoon in the ICU. Never did hear from her Doctor who was also the doctor on the her board in her room at Centennial Hills. I Saw every specialist but the guy who was in charge. Nurse at PAM lied to me about what he was doing that morning. They had to be called in the middle of the night when she first got there to get assistance in her room. She Had breathing issues but could not get the nurse for over 2 hours. She called me and I called and got the head of nursing and got someone over there. She said they were rude to her. I found a cold reaction from most staff. They don't answer anything and always seem to be on guard. Her Death certificate states a medical term for undiagnosed infections as part of the cause. I don't recommend this place for anyone who needs real care. She never had therapy in 3 weeks so I can't rate that. There's a telltale thing I noticed right away. Staff at Centennial Hills were very accommodating. Going out of their way to be courteous and just plain nice. I was there all night the last couple of days and the nurse made sure I had privacy, food, drinks. They all were the same. My adult children came from out of town to see their mom and the nurses did the same for them. Pam Care you needed to call over and over. Sometimes go to the nurses station to get them. Night and day. Very disappointed with our choice to use PAM
