We just checked our brother out of this facility after a two month stay. Not only did he rave about the staff and his care, but upon checkout the physical therapy staff voluntarily assisted to load him into the camper van so that we could transport him to be nearer to his children, which was no easy feat considering his inability to use his legs or arms or even sit up for any length of time in a wheelchair chair. They truly went above and beyond! read more

I thought long and hard before writing this review. In general I don't post negative reviews and try to focus on the positive, but after 2 weeks here we were given 24 hours to move our 94 year old immobile mother out of the facility or pay $600/day. [[HIGHLIGHT]]I'm not complaining about that so much as the lack of notice, the lack of explanation on what our options were and the utter lack of concern about the stress surrounding all of it.[[ENDHIGHLIGHT]] [[HIGHLIGHT]]They should know more about how the process works than I do - and they either don't, or don't care enough about the family the patient or the process to explain any of it.[[ENDHIGHLIGHT]] I know more about how Medicare, medigap and supplemental insurance than the management of this facility after going through this process one time. The care here is good enough. Some wonderful nurses and some impatient ones. But it's a tough job and I appreciate anyone doing this for a living. But understand the process and be ready to be thrown out with no safety net and no help figuring it all out with literally no notice. Once insurance because an issue they were relentless over a 24 hour period about getting her out. OK - I get it, nothing is free. But they know the process, we don't and [[HIGHLIGHT]]they really offered no help in what to do next since she couldn't go home.[[ENDHIGHLIGHT]] PS : our mom passed within a week of getting kicked out of here and honestly the stress of it all was a factor. read more

zero stars if I could!!! If you love your loved one at all, do NOT subject them to this hellhole. They start off ok, but the longer you are there, the more and more neglectful they become. My dad was sent to this Pitt of Hell after 3 weeks in the hospital with a wound to his arm after a fall. By the end of the third week, he was begging to be taken from there. [[HIGHLIGHT]]The nurses were mostly lazy and rude.[[ENDHIGHLIGHT]] [[HIGHLIGHT]]The case worker Jonna was awful![[ENDHIGHLIGHT]] [[HIGHLIGHT]]The director never came to talk to us, even after we expressed our displeasure.[[ENDHIGHLIGHT]] [[HIGHLIGHT]]Just a general feeling of apathy and laziness on the part of most of the staff.[[ENDHIGHLIGHT]] My sister and I would go everyday after work, so we would get there around 4:00. Walking down the hallways, you would see every patient in bed or in chairs in their rooms. There were hardly any patients up and walking around. Basically, other than their prescribed hour of PT, patients were stuck in their beds for pretty much 20 or more hours a day. They never got my dad up and walking around. My sister and I would make him get up and walk around when we got there. This is supposed to be a rehab place!! Instead of encouraging my dad to get up and use the restroom, they put him in a diaper and let him go all day before changing him. His room was often a mess and they would even leave spilled food on the floor. The food was awful. My dad lost 20 pounds while there. It was literally a hell hole and old people dying factory. [[HIGHLIGHT]]I especially had issues with a nurse named Florida. She must have gotten her nursing license from one of those fly by night nursing schools. She was lazy and rude.[[ENDHIGHLIGHT]] I would catch her lounging at the nurses station chatting it up instead of taking care of her patients. Useless! [[HIGHLIGHT]]This place doesn't communicate with family. Trying to talk to someone about your loved one is very hard.[[ENDHIGHLIGHT]] And when you do, you are labeled as difficult and they try to eject you. They lie on forms to Medicare and say things that are absurdly untrue. They told insurance that they had trained the family to take care of my dad at home. That NEVER happened. Finally, we got my dad out of this stink hole and into Assisted Living at Avamere, a beautiful place with kind people. Unfortunately, he only lasted another week before he passed. We believe that it was his neglectful care at Canyon Vista that led to his death. I am going to make it my life goal to get this place closed down. They are harmful and hurtful and shouldn't be in the care business. read more

My wife was in Canyon Vista rehab for just under a month following a referral after a hospital stay. According to Canyon Vista case management, she was discharged on the last day that insurance would pay. We were assured that her entire stay was covered by her insurance. Approximately two weeks later, we received a bill from Canyon Vista for over $1,700. Less than a week after the date on the bill, they sent a letter stating that the account was "seriously delinquent" and threatening collection action. Her insurance company said that Canyon Vista has been paid over $6,000, and that our maximum payment should be $150 (the amount of the deductible). The insurance company is still working on the matter. read more

If I could give a 0 stars I would. It's long but worth reading please. My 90 yr old dad was transferred to this facility from the hospital on 01/26/26 after having what I was told was a small stroke. When he left the hospital, my dad was sitting up talking, using his cell phone, responding to questions, walking around his hospital room. Once transferred to Canyon Vista his health declined rapidly, he was in and out of sleep but still speaking and answering questions. I came to Canyon vista the day he was transferred and we went over everything that evening. He was supposed to be having physical therapy, occupational therapy and speech therapy while there. However, when I came in the next day his sleep was worse. He was also having a harder time using his cell phone and communicating. I kept telling the nurses this is not how he was at the hospital; something is not right. This was the second day he was there so 01/27/26. While I was there the charge nurse came in along with his regular nurse (side note they are very lacking in nurse-to-patient ratio) they informed me that the doctor who had been in suggested using Narcan because of his sleeping and not waking up or waking up to answer a question but going right back to sleep. I said absolutely not, he hasn't had any pain meds and what would this do to a 90 year, no way. There is something else wrong. The next day I was in and he was worse again, on oxygen and was told by the nurses that he had a breathing treatment over night. I asked why is he on oxygen and having breathing treatments, they just said to make him more comfortable. [[HIGHLIGHT]]To make this long story as short but as truthful as possible, the doctor that saw my dad on 01/27/26 stated in the medical record notes, diagnosed my dad with Acute Respiratory Failure, but never told me or mentioned this.[[ENDHIGHLIGHT]] [[HIGHLIGHT]]All this time I was constantly calling the charge nurse, his nurse, to Susan the Admission, Rae, his case manager, anyone and everyone that I could talk to. Dr. Nasiri tried to get in touch with me once and I called him right back, never heard back from him until the day before my dad passed there at Canyon Vista[[ENDHIGHLIGHT]]. This place is a joke, my dad should have been sent back to the hospital, which is what I wanted and made known, not only to them but to my dad's primary doctor, and also his Dialysis team at Davita Dialysis (I was always in contact with them as well and they would have sent him back to the hospital seeing him struggling with breathing at all). This Canyon Vista was still trying to do rehab with my dad while he wasn't even able to stay awake. They lost his $1500.00 hearing aides, and I had to contact everyone again to get them to find his hearing aides. They never had his call button next to him anytime I went in; the wall had exposed wires where the call button was at the wall, any time I called the main number it was a 50/50 chance that someone would answer and at this point my dad couldn't use his cell phone. [[HIGHLIGHT]]Not being able to be there with him all day and night left me calling them to get updates and even when I was there the nurses would say they had tried to get in touch with the doctor but hadn't heard from him.[[ENDHIGHLIGHT]] This place is terrible and then I found out they were sending bills to his insurance with dates of service after his passing. Yes, I filled a fraud claim with his insurance. My dad passed on 01/30/26 while there, they had to call 911 because of my dad's breathing. I had previously called that same day spoke with the charge nurse again and asked her to go in and check my dad because I had a family friend go see him and he was struggling with his breathing, she said well I was just in there, I said I don't care go in again. I would not ever recommend this place and if you have a loved one there please check on them regularly and if you can get them out of there, I would suggest it. read more

STAY AWAY. No hot water in over a week. Second night waited 23 min to get help to bathroom, last night 45 min. Left with UTI infection. No rehab given. Had me lift weights with torn rotator. Room cleaned 1 time and bedsheets changed 1 time all in one a week. Ice locked up in closet in a cooler. Wound care incompetent. Stitches were dissolvable and said doc needed to take out. They wanted to take bandages off back after 4 days. I said no and get out. This was 2 surgeries to replace discs with plates put in the back and second surgery was to attach screws to plates and into bones. Severe surgery. I would not even give them 1 star. read more

I was taken to this facility after having a knee revision. I was moved there on a Saturday evening. After getting settled in, I was told I would be moved to another room the next day. No problem. I was then moved to another room down the hall. In the room next door, a man held continually about everything (more pain meds) and how he wanted out of there. This went on for a couple days; morning, noon and night). He finally left. Then another patient was put in place who was worse. Although he could not talk, he made screeching noises day and night. The staff would usually leave the doors open, so the sound was horrible. My family members would close his door and my door, but the sounds still could be heard. Then, another patient was put in a room across the hall. She yelled continuously in a very high, shrill voice. I my Between the two, the sounds were terrible. Nursing care was extremely bad, with the exception of a few CNAs. Namely, Jannett, Azeb, and Elizabeth were fantastic when I occasionally had them. The rest of the nursing staff were quite lousy. I was in the 400 wing, so when I'd press the call button, there generally would be no response. It turned out the nurses didn't use the 400 nursing station, but sat at the 200 station. So, when I called, no one saw the alert. Because of the layout of the rooms, the alert light above my door could not be seen from down the hall. The only bright thing was the Occupational Therapy and Physical Therapy staff. They really went above and beyond to provide care. Outstanding personnel are: Sierra, Nerissa, Davis, Nico, Stacy and Eileen. They are superstars. [[HIGHLIGHT]]Administration was horrible. When we asked what needed to be done to be released, we were passed from office to office. They'd say go see the Business Manager; the person was not available or in their office. This went on with all the management.[[ENDHIGHLIGHT]] Finally, the noises from other rooms was more than I could stand and the substandard care became unbearable. I have been diagnosed with PTSD from my military combat experiences, and I told them the noises were causing relapses. I needed to be moved to another room. They said they couldn't do that. I finally threatened to get a lawyer who specializes in elderly abuse. That started the the ball rolling for discharge. I finally was discharged after about 4 weeks. read more

Someone on staff stole something from my husband and after my first review ( which I believe they may have deleted) I got a call from Shawn/Sean who was in their marketing dept. My brother had a sub dural hematoma and recovery is very difficult. Canyon Vista gave him the needed PT physical therapy to perhaps go home and enjoy his retirement. After many years of working in the hospital in Las Vegas and helping people. I hoped he would be able to enjoy his retirement. But this illness cause him many episodes in the hospital and rehab centers in Las Vegas. Really No stars at all! Too long to write! The level of incompetence of informing family is terrifying!!!! My blood is boiling! I prayed to God the reviews were wrong but they're spot on! The 5 stars have to be fake! I'm not blasting I'm telling from even my experience of her being there just for the last 48 hours!!! They call me not to give info... but to just tell me she fell again. As I hear another nurse screaming at a patient while I'm on the phone then I disconnect and no call back! I have to get there!!!! I'm afraid they're going to kill her before I get there with there lack of Drs around and nurses not even able to read charts!!! I wrote this back in july and never posted it. My mother was then admitted to the hospital where she was severely dehydrated during her. Stay at Canyon Vista post acute. Unfortunately during her stay. She also had a bad sore open up. Nothing ever got better because she wasn't able to get the nutrition she needed. Eventually I lost my mom on August 22. I will never be the same without my mom. She was my best friend. I had her out of this place within two days, but it wasn't soon enough. If you're going to go to any place like this. You have to be there every minute to advocate, my deepest regret was trusting this facility as part of my Hope for my mom's recovery. read more

[[HIGHLIGHT]]Even if you eventually reach Canyon Vista, NO voice mail is ever returned.[[ENDHIGHLIGHT]] [[HIGHLIGHT]]My insurance also tried to contact them but gave up after a while. Nobody answered for them either[[ENDHIGHLIGHT]]. Al my bills were mixed up between what they tried to charge Medicare or my insurance. To add insult to injury, they sent me some additional bills that did not even mention their name but the name of a third party. Please do NOT send anybody there. read more

My husband fractured his left hip and was hospitalized for 3 1/2 weeks not standing. When he arrived at Canyon Vista he couldn't move his legs in bed. After 2 months of physical therapy and occupational therapy with the wonderful staff he is able to walk with a walker, stand up from sitting and get himself out of bed! This place is horrible. They almost killed my brother by not giving him his medications and letting him lay in his feces. His back got fully infected after his back surgery due to their negligence. He had to be taken back to the hospital and have 3 more surgeries due to infection and parasites in his back. We were told another week and he would have died. I would never recommend this place for care as they don't know what they are doing. We even have information from Dr's of his lack of care and by his lack of care it extended his recovery by 8 months if not more. Don't ever go to this place if you want to live and have a normal life. His current occupational therapist had to pull her dad out due to their lack of care as well. read more

This place provides subpar care, including terrible food, unreliable service, and less than adequate care. I don't know if it is because they are understaffed or if the staff is under qualified but patients have unreasonably been dropped, have bedsores, have gone from no insulin use to daily insulin use and have seen plateau or regression under their care. read more

my roommate was there to receive physical therapy. they wouldn't let her out of bed for 10 days then made her sit in a wheelchair for 10 hours a day without getting her out. their definition of physical therapy was 5 minutes a day and the rest sitting in a wheelchair alone. it was like a prison. she left worse than when she went in. don't let them fool you with a fancy lobby or promises of physical rehabilitation. it's all about the money they can bilk Medicare out of. read more

One hand doesn't know what the others doing! These guys put the three stooges to shame. Starting with the food - We're dealing with it "dietitian" who doesn't know the difference between Vegan and vegetarian -if that's not bad enough I had to explain it and break it down to her over the phone. I had to explain no fish no dairy no eggs no meat blah blah blah. Tofu, And/or any other meat substitute was acceptable as long as it's plant-based. Question How can you be a dietitian in charge of the kitchen and not know how to make a balanced meal? Aren't you supposed to have a degree or something. At least understand nutrition. HOW IS NOODLES AND CARROTS or SPINACH AND POTATOES AND BALANCED MEAL?!?? So now I'm spending my own money at approximately 20 bucks a meal (In a place that is probably charging $400-$500) per day I cannot take the swill they send to my room!!! The place is severely understaffed. They also have quite a lack of knowledge of people with Parkinson's. But there are some good CNA's and nurses. Type of care, care, quality, staff was horrible, during my four weeks there I think I had four physical therapy session and that was it. It told me to stand up and sit down for my exercise. It was a joke and they left me on a Porta potty for two hours. There's so much I could say about this place. It is horrible. Never take a loved one there. read more

This place is the absolute worst! My father was in this facility for almost two months before I decided to take him home to Pass away. They stated they were taking him to therapy but they were not and then my dad contracted Covid and they moved him to another section of the wing where they only checked on him once in a while and when his meals were delivered they just dropped the food on the tray knowing he needed help unwrapping the clear plastic over the tray and no one helped so my father hardly ate and ended up losing more weight. When I took him out of there is was only 100 pounds or so. The nurses and CNA's couldn't care less about the patients there. This Place needs to be given a negative review. If you love your family member don't place them in this facility for sure. read more

I wish I could talk more about this place but unfortunately things didn't go as smoothly as I He planned for my fiancée, it didn't turn out to be a great end. My fiancé said at the time of service .no one came around for hours & was left alone. Then I arrived and no one greeted me or talk to me. Finally 10 hours later A night shift crew came in . They knew nothing it was so quiet and possibly I could say spooky . When I finally left it was 11 PM at night and my fiancé was in the room all alone . At this point he was exhausted & extremely sleepy , hungry and he didn't feel comfortable . Lack of communication and organization . Got a phone call early morning and things didn't work out as planned. He immediately went back to the hospital so he stayed less than 24 hours that's why I said there's not much I can say about this place. Lack of communication with staff. read more
