Why is it so difficult for you, Bill, to comply with State and DOT regulations to provide service as a SAP DOT? While you may meet some NCAC1 requirements (hard to confirm). You are NOT in compliance with Washington State DOH regulations. Your CL60157386 does NOT authorize you to conduct any kind of drug and alcohol assessment, and you lack the credentials needed to diagnose or treat mental health per state law. We do have the right to ask. To provide SAP services in Washington, you must adhere to specific licensing requirements. Your retired/active SUDP status prohibits you from charging for services and for emergencies. To clarify how you obtain permission for SAP work, this information was conveyed to the courts. Moreover, stating that NCAC1 qualifies as an SAP is INCORRECT. Validation licensing is mandatory to serve the public under both state and federal guidelines, including 49 CFR Part 40. According to the DOT, you are not permitted to directly observe any drug and alcohol test as part of their assessment, and you're NOT a certified collector. Only authorized and trained collectors are authorized. For a direct observation of a collection, there are specific stipulations and restrictions. A SAP is to conduct evaluations, provide recommendations, provide treatment, and determine when an employee is ready for a return-to-duty test. You can look it up in the 49 CFR Part 40 guidelines for DOT and in Washington State law. 49 CFR Part 40: the SAP's original requirements are the only ones that are authorized to make adjustments or other recommendations. You do NOT show any documents stating that your evaluations expire. By giving the same tests and almost the same recommendations, and the same answers since May 2025. It is considered fraudulent to charge for the same services. Asking for a full refund.

Selecting the Best endodontist is important for everyone, since we all want to be able to save our teeth and have them for the rest of our lives if at all possible. What I truly respected about Dr. Bolles is that he explained up- front that my tooth could be cracked and the only way he would know for sure is to go inside and look at the tooth canal with his operating microscope. Selecting the right endodontist I feel is so important.
