    My first visit was quick, but at my second I waited nearly an hour past my appointment time. I…read morereceived a standard skin check and had one wart frozen. No biopsy, no prescriptions, nothing else treated. Despite this, I was billed separately for a wart removal procedure and an office visit, totaling about one hundred seventy-four dollars. According to AMA CPT coding guidelines, the evaluation for a minor procedure already includes the basic exam and discussion. A separate office visit can only be billed if there is a "significant, separately identifiable" service documented beyond the procedure (AMA Modifier-25 guidance). CMS policy also states that the initial evaluation for minor surgical procedures is always included in the global package unless there is clear extra work (CMS NCCI Manual, Ch. 12). The AAPC further warns that billing both codes together without documentation is improper and considered "double billing." When I asked the billing office, they said they charge for an office visit every time and never code annual skin checks as preventive. That means patients are automatically billed for both, even when guidelines don't support it. Patients should be aware that this practice leads to higher out-of-pocket costs, especially on high-deductible plans.

    Over a month ago, I scheduled an appointment with Dermatology Specialist. I am a new patient here…read moreand decided to go with this doctor due to their availability. I really needed to get in sooner but this was the first available. While setting the appointment, the office set me up an account, got my insurance information and personal information. They told me a date and time and I confirmed with them the time specified. The day finally comes for me to come in and I noticed that I had not received a reminder text or call, which I thought was unusual as most doctors these days will send a reminder. I get to the office 25 minutes early to take care of new patient paperwork and I was told no appointment was set. They have my account information that was set up that day but they did not actually have an appointment for me. I had moved around meetings at work to make the time they had told me and driven nearly 45 minutes to get here. I expressed my concern as they looked for other available days nearby. They agreed to see me today but set it for hours later than my appointment time. I sat in the waiting room watching patient after patient come in to the waiting room and go back for treatment. Finally my name was called. The assistant put me in a room and asked me what I was there for. I told her a few concerns and said I would like an overall check up. The assistant left the room and returned with a paper gown. The doctor was very thorough, Listened to everything I had to say, and seemed generally interested In addressing my concerns. I'm pleased they were able to work me in but feel like it shouldn't have been an issue to begin with if the office had set the appointment line they told me. I will say however that those who had an appointment that did not get messed up were seen fairly quickly.,
