kindred hospital is the worst hospital in the state. my husband was there twice within month and…read moreboth times the AC was out except for the nurses area. the rooms were filthy and some days not cleaned for days. they had the meds laying on the filthy counters in the room. most of the nurses had a huge additude. they chatted more than helped patients. i asked the charge nurse to call me back when my husband was dying and the nurse said ' oops i guess i forgot to call u back.'. my husband left that place to hospice and died two days later. i truly believe it was from the filthy area he had to stay in. jb

These patients sing praises about Forte, from the doctors to the nurses to the efficient people at the front desk. From a visitor's point of view, Forte is great. They are very straightforward with their instructions, which I appreciate. Just tell me what to do and what to expect, and I'll do it and expect it. Everything went pretty much according to what they said. They have a smoothly running system here. Visitors can help themselves to the unlimited coffee and water at the coffee and water station. This is where the Little Free Library is located! The waiting room also has multiple seating arrangements, from high-top desks to comfy chairs to love seats. Or, if you need something to eat or drink, there's a cafe on the first floor. (See my separate review for La Verna Cafe.) The waiting room is airy and bright. One thing that's kinda weird is that a "cruise director" comes and talks about what to expect with your surgery ... right in the waiting room, among all those random people. That seems unusual, right? However, there are private rooms that you're taken to when the time comes to talk about your particular case with the doctor. As a visitor who is not a patient, Forte is great. But I've been told that it's also great being a patient here, so there's that.

I continue to be at home healing and building strength, though I'm largely moving toward becoming more independent and hopefully returning to work in the not so distant future. One aspect of life that will need to change is the need for a different wheelchair given my change in physical status. Thus, because I have commercial insurance quite a bit of red tape is required. This includes both a tech evaluation and a full mobility evaluation by a physical therapist. At the recommendation of my DME vendor, I scheduled an appointment with Community Health Network's Physical Therapy & Rehab - Neuro Specialty precisely because they were both close and could get me in fairly quickly. They are located in the Community Health Pavilion at Fort Ben and are almost directly across from both Fort Ben and Culver's. They stumbled into a bit of a mess today and handled it quite nicely. I received a call from the DME vendor about an hour before my appointment stating that it would need to be canceled and rescheduled for 1/30/20. Since this is such an important part of me getting back to work, the prospect of a two-week delay had me pretty agitated. I started making some calls and decided to show up for my appointment that had been canceled. I wasn't in a good mood when I arrived, but I was also determined to either get it all straightened out and maintain the appointment or get an appointment sooner than 1/30. The pavilion is wheelchair friendly - they have multiple accessible parking spots and a front-entrance canopy for safe drop-off. Both glass doorways at the entry have push-button power access. This particular office is directly on the right when you enter. The person working the desk appeared to have stepped away. The operator greeted me briefly, though seemed a bit confused as I was not in their system for having an appointment. I explained the situation. I knew that the DME vendor rep had an appointment at the same office immediately prior to mine. I'd hoped I'd caught him prior to my appointment but would learn he'd left. I was impressed with the front office staff here. I was frustrated, but they kept an even tone about themselves and simply did what they were able to do. They were firm about not being able to have an appointment without the referral, but also shifted focus when it became clear I was determined to get them the referral and maintain the appointment. After multiple phone calls, I had all the pieces in place and the DME vendor agreed to return. The evaluation occurred as scheduled - Cara, the physical therapist, was a delight. She was personable, thorough, professional, and remarkably upbeat. She switched gears and I appreciated her willingness to go through with it. I loved her philosophies - she even gave me an explanation of using the phrase "differently abled" that I really respected. She set a great tone and did a great job. She assured me that given the time sensitivity she would get the evaluation done as quickly as possible but with all the needed information. This could have been a difficult experience, but with a top notch front office staff and a wonderful therapist, this ended up being a really positive experience. Cara also gave me a flyer for an amputee support group - and was also mentoring a student. I really was impressed and would easily return.

These folks really know their stuff and are super friendly to boot! The ladies at the counter always greeted me warmly and with smiles and so did all the rest of the staff. I worked with 2 specialists and both were great at getting me back to moving around again, you can't go wrong with Ryan and Beth! But, really from what i experienced and saw all of the staff took great pride in their work and cared for their clients. And that makes all the difference, you can tell they care.

If you need Physical Therapy, look no further because Community PT Carmel is a great place to go…read more I have been to a lot of Physical Therapy practices over the years with my knees, shoulder, hip, and feet, as well as with my teens and mother-in-law. Of course the PTs are all competent and the facilities all have the equipment you need (wooden steps, bikes, tables, straps, etc.) but you have to find a PT with the personality that works with you and an environment that is comfortable. I have seen my share of mediocre experiences -- large chaotic rooms full of dozens of PTs & dozens of patients, or a room of PTs talking about their weekend more that focusing on their patients, or front desk personnel that give you a list of appts. only to find out later that each appt. is with a different PT (does NOT work well)! I or my relatives have been to PT at St. V., Methodist Sports Ctr., ATI, Body One, IU, Carmel Rehab & more. Community PT Carmel is my favorite for many reasons: it has a convenient location, easy parking, walk in building and it is right there (car to front desk = 1 min. max.), little wait, good & helpful front desk personnel, and just 3 PTs and therefore no more than 6 in the room so it is easy to focus and get attentive care. I have had sessions with two of their PTs for knee and feet issues: Amy Guillaume and Brett McDonald, and both are very good at what they do, as well as being just nice and pleasant people who communicate well and care. It is a good, calm, effective experience from the beginning to the end -- Community PT Carmel!

They responded to my request the next day and then offered to book me in for the following week. Amazing, considering how popular they are! Once my appointment was booked, I was emailed a form to fill out digitally before arrival. It helped me prep for the appointment and it was nice to get my insurance info squared away before visiting. They also sent reminder texts and emails, and sent me a COVID screening the morning of the appointment (mini online quiz to say if you'd been feeling sick or not). It was all very seamless! When I visited the office, they checked me in right away and I was brought back right at my appointment time. A PA named Kyler came in to assess my knees and she was super awesome and helpful. I was then brought back for knee x-rays and the x-ray women were SO nice too. As soon as I was brought out of the x-ray room, an attendant brought me back to my room and Dr. Shook came in to assess. He delivered good news (yay!) and was so kind and helpful. He communicated very clearly, seemed very patient, and was open to any questions. He printed off exercises for me to do at home and the attendant brought me to check out. All seven or so people that I interacted with were so friendly and sharp. The whole appointment was very efficient and Dr. Shook seemed to know everyone by name. I would highly recommend this place to anyone!

Elaine is a wonderful, friendly, knowledgable therapist who created a realistic plan from the first time we sat down together and has been very encouraging and motivating throughout the process. There is a clause that you must call 24/hrs in advance if you want to cancel. I came back from a cruise on a Sunday, totally sick from Influenza-A and I'm an asthmatic, so I was also wheezing.
