My grandfather died in this facility. If you love your family member you will not put them in this facility. Most of the workers are not actual nurses, they are CNAs who have no idea what they are doing. Most were very rude or dismissive. My grandfather's leg was bleeding because he was on blood thinners and they gave him a tiny bandaid... another time he fell out of his chair and called the front from his cell phone multiple times and pressed the button. No one came. When she finally did she said she was in another wing, it was over 30 mins... There were also multiple patients there who ask for help and no one comes. I had to help a gentleman on our wing with his oxygen because it was twisted in his wheelchair. We also had asked for towels so we could bathe my grandfather and the CNA gave us full access to a room that holds the medicine and supplies, but no towels. I pray this place closes. read more

Never let a loved one get admitted into this rehab place. A few years ago, my mom had a fall and after a hospital stay, needed rehab. She was placed a Madison on Marsh .. what a poorly run and disgusting place. The staff does not care, the food was disgusting, and the level of neglect which occurs there is off the charts. A simple walk up and down the hallway will conjure screams of "please help me!!" from patients desperately seeking assistance. Ringing the call button does no good there. When we were able to get my mom discharged from this facility, we vowed to never return. It really should be under investigation by HHS. Stay away! read more

Madison on the Marsh is a clean, well kept facility. The nursing staff, doctors, social worker, and OT/PT staff are wonderful, caring people who do the best they can to offer quality care. The downside is that there are just not enough staff to meet the demand. Consequently, the patients have to wait longer than they should have to for someone to come and give them the attention needed. The quality of the leadership needs a definite upgrade. They provide misleading information. Be advised that if you are told anything concerning the financial/insurance coverage side of the stay, have them put it in writing. We found out the hard way that we should have done so. read more

Please do not allow your loved ones to be sent to this facility for "rehab". Members of the staff are given to abuse, the purpose of the facility is rehab, and they are highly remiss in this regard. Reaching someone and receiving return calls, or emails, are hit and miss. Take no chance on allowing them to assist someone you love. Members of the staff will lie to cya. I want to clarify my review from Jun 30 2025 to note that the best part of my experience with this facility is the exercise regimen. My only complaint with it is that it's only 5 days a week. I have my own weights to do leg lift and arm exercises on weekends, but no access to the stationary bicycle. read more

I'm very frustrated with this place due to the fact that no one answers the phone there. I try to get a hold of my father and have been put on hold for up to half an hour and I end up hanging up and never getting through. It's very frustrating because my father doesn't know how to use his cellular phone and there is no other way to get a hold of him. read more

This is not a place that you want to send a loved one to. We went on a Friday, and there needed to be an introduction, instruction, communication, and a process. There was none of that. They put my husband in a room, and that was it. Some weekday staff members are professional, but nights and weekends are filled with uncaring and lazy people. My husband was unable to do things for himself when he first arrived. Their response was to put him on a mattress on the floor "for his safety." They acted as if helping him use the bedside toilet was an inconvenience to them. His physical therapy was great; however, the speech therapy was lacking. In defense of the rehabilitation team, the director is very responsive and a God-sent because she is considerate, thoughtful, and willing to assist. She is the best part of the facility. Ice machines are not automatic; they use a scoop to get ice, which is not sanitary. The food is dry and mostly scaps - who pairs a chicken wrap with two soggy onion rings? But I must say, the young lady that usually brings dinner and gives showers is AWESOME. She is around there walking fast and getting this done with a very pleasant disposition. I signed him out one evening, and no one noticed or called about him returning until 4 am the next day, stating that he was supposed to return by midnight while another person told me there is a 2-hour LOA limit - which is it and don't you think you should have said it as he was being checked out? When he was discharged, the nurse said she would call me to pick up a prescription; however, a week later, I had not heard from them. Once my husband reached the point where he could walk better and was less confused, they displayed more tolerance - seriously? Yes, I guess they don't like sick patients. My saving grace is that we lived close and had a supportive family, meaning we could check-in and/or stay with him a lot instead of heavily relying on the facility 24 hours a day. I would stay far away from this facility. read more

I was transferred from Baylor Scott & White Plano on Weds Nov 27 and my meds that the hospital doctor found for me that helped me in every way. Weren't there yet. Thursday still not there in the Mean time some nurse name Mary came in room and said she needed to give me a shot... when I asked what was in it and asked 4 times I was told each time it was a welcome shot! Then later that night around 3:00am i asked why my night meds where not given to me, they had to wake up this nurse Mary and she comes to my room and ask me which pills did i want ... i was like what do you mean which pills? I need my night time pills... she comes back with a pill cup with pills in it and as i am trying to see what pills are there she says i am doing bad business and should just trust the nurse... then Thursday am my nurse Kenneth looked up my meds and said he could see they are ordered but not there yet ... he was hopeful they would be there Friday.... So this morning again he looked them up and as they were still not there and he was trying to find out why... or they could be on the next delivery. So tonight when i asked my meds... after 3 times nurses would come in my room with the wrong name about to give me someone elses meds.... I decided i had it and was leaving.... NOW that I am leaving all of sudden they are going to get my meds tonight ... I said to late.... They should have been here yesterday.... Now I have a huge migraine and I am going home to my old meds to help my head. They kept trying everything to get me to come back inside... they went as far as calling the director of Nursing and she wanted to talk to me... I said to late I am going home I do NOT recommend this place to anyone.... The staff Lies, the food is gross, and they just want you there so your bed counts as patients and they get money for it...... THIS PLACE NEEDS to be REVIEWED and then SHUTDOWN.... Its not even a place i would board my dog!! Do NOT believe all These 5 stars on here.... They have to be relative's!! read more

Some of staff does not wear badges. Per the state law all employees should wear badges. When you ask them to wear one they think you are wrong of asking them . I told them want to see the badge. Anyone can walk into your room and you will not know who it is. Then your loves ones is in dangerous. STAY AWAY FROM THIS PLACE and the staff think they are in control of your health (missing giving some of medication for 4 days and checking my glucose levels ) . The Executive Director had a worker call my daughter for financial responsibility on 11/14/2023. My has never been up there nor signed any paperworks then they tried to get me to signed the paperwork on 11/15/2023 to correct their mistak. I refused to sign then Executive Director become RUDE and stated "you want me to call the State of TX. I told her I know how to call the State of TX and then told I was discharging myself. By the way, They miss taking residents vital signs. DO NOT TAKE YOUR LOVERS THERE if you loves them. I was there from 11/9/2023 to 11/15/2023 and I discharged myself. Beware my grandfather received some of the worst care here which I think attributed to his passing. Instead of the families request to take him to the hospital, the instead gave him another IV and hours later he quickly declined. In addition soon afterward the state did an investigation and penalized them for another case. Rumor has it they have had 10 deaths at this rehab facility in the first 6 months of this year and fired employees who raised concern over care of patients. read more

I would give zero stars if it were an option. Bruises and infections, lies and ultimately death. If that's what you want go there. Only if you sit in the room 24/7 does your loved one get even close to decent care and you are giving most of it. Waiting hours in wet clothes, deep tissue bruises, unexplained cuts and bruises, infection after infection.....they killed my mom. read more

Do not put your loved one here. The Administrative staff is horrible! Because we live on the other side of the country, a social worker was assigned to her when she needed to go to the hospital. They recommended Madison and told us they would be able to follow her can since we are out of state. We never heard from the Social Worker again. You can never speak to anyone when you call at Madison because they are always "in a meeting." We had our relative there for over a year and were advised by Doctors not to move her closer to us out of state because of her mental/medical condition. We called several times a week, when they would answer the phone. When she passed, they claimed they left a message for us to call, but after looking through all phone messages on cell and house, we never received the call. They never followed up and we learned of our loved ones death 2 days later from the hospital she was transferred to. The Administrative staff never once call to offer their condolences or to ask if we needed anything. 2 1/2 weeks later after we buried her, I had to call to speak to the Director who gave me lots of excuses. There has got to be a better place to place your loved one! read more

My advice, filled with deep sorrow and regret, is to not allow anyone you love near Madison. My mother was having terrible abdominal pains, and a doctor at the hospital my sister took her to, sent her to Madison for rehab. After asking for weeks if my mom was taking all her medications she took at home, and being told yes by the nurses, I discovered she was not... no heart medication, no blood pressure meds, blood thinners, etc., nothing but her insulin. After meeting with the inept doctor, we were told she would finally be getting her regularly prescribed meds, but we don't know if that was really true, or not. And, the stupid therapist there told my sister that my mom was lazy and didn't want to work out, and that the bathroom was her "safe space." Well my mom had diarrhea and gas daily, and was losing weight rapidly, wouldn't eat, and was always in severe pain. They gave her tums and aspirin. I argued constantly with my sister to remove her from the facility and send her to a real hospital, but that didn't happen quick enough. After over a month, she had to get two UTI's and they started an IV antibiotic. When she started having heart issues, a nurse called and asked if I wanted to wait and see her regular cardiologist after they released her in 3, 4, or 5 more few days, or transport her to the hospital that day. I said send her to the hospital immediately (and not the first hospital who sent her to Madison on Marsh). They quickly found out what was wrong with my mom. Evidently, my mom lived in the bathroom not because it was her "safe space" but because she had cancer of the colon, and was in agony with a folded colon, with a mass of cancer. She had surgery to remove the mass, and 12 inches of her intestine. The following day, she had cardiac arrest, but they resuscitated her after 12 minutes. However, it was too much for her, and she was dying now. After 12 minutes of death people change, and she was afraid, not knowing where she was, or who we were, and then we had to just watch her die. It was too late, a month and two weeks too late. I wish I had never allowed her into Madison. She might be alive today, and I could call her and tell her how much I love her. I don't scream obscenities when I drive by Madison anymore, but I know they killed my mom with their ineptitude, and I will forever tell any and everyone - DO NOT SEND SOMEONE YOU LOVE TO THAT HELL HOLE. read more

The Madison on Marsh is the best place to drop off you beloved family, if you want them to be ignored and mistreated. Majority of the staff are just there for a paycheck and confirm the stereotypes of senior facilities. The only good thing I can say about this place is that if it wasn't for one of the receptionist (Michelle I think) I would have removed my father sooner. Her charismatic personality and friendly smile helped me and my family get thur the roughest times of our lives. She took time out of her day just to build personal connections with all of us. She truly is a god sent.People like her dont belong in places like that. read more

First of all, I work at Madison on Marsh. We do the best we can to give the best care possible to our residents. Seeing negative comments about the people I work with and especially to the people we care about is really pathetic. We give the best care possible. DoN, Charge Nurses, Housekeeping, Dietary, CNAs' and the Director, has and will aleays continue to do everything possible to help. And believe it or not, we love our residents and family members, Biased? Heck ya! read more

This facility is wonderful. I am a volunteer here and every time I come, I am welcomed by warm staff in a beautiful environment. The patients receive good care and the staff is very responsive to both family and patient needs. They have a great rehab program and I hear nothing but good things about the facility from the patients. This place truly deserves its five star rating! read more

my parent went to the madison on marsh for therapy after surgery and what a nightmare! the staff is completely inept to giving anyone care. they left my parent in their soiled clothes for more than 8 hours and never once admitted that they knew anything was wrong. you have to scream and throw fits just to get someone to come in to their room to see what is wrong. they have no clue how to do skilled nursing! this is the worst place you could put your loved one in!!!!!!!!! i would rather die than stay at this facility. the healthboard is constantly visiting them with reports from residences and their families. stay far away from this horrific place!!!!! read more
