    Serious Concern Regarding Unprofessional, Unsafe Blood Draw, poor technique and poor hygiene on…read more8/20/2026 by Kalinda Smith MA On Thursday, August 20, I visited Dr. Thomas Avella with my son for our annual physicals. My initial encounter with Medical Assistant Bryan was excellent -- he was friendly, professional, and thorough while obtaining our vitals and medical history. However, the blood draw performed afterward by Kalinda Smith, MA, was deeply concerning and did not meet basic standards of hygiene, professionalism, or safe phlebotomy technique. Smith entered the room without introducing herself. When I asked whether she was a nurse or phlebotomist, she stated she was a medical assistant. Immediately afterward, she stepped into the cabinet area and tied her shoelaces. She did not wash her hands before preparing the blood draw equipment and after she tied her shoelaces. As a medical professional myself, I found this extremely unhygienic and unacceptable. During the blood draw, she inserted the needle into my left arm but did not obtain blood. She repeatedly moved and manipulated the needle inside my vein, causing significant pain. When I told her she was hurting me, she argued and insisted I "did not drink enough water," despite the fact that I told her I did. Eventually the blood began flowing normally. Additionally, she left the tourniquet on for the entire duration of the blood draw, which increases the risk of hemolysis and is not appropriate technique. Her communication throughout the encounter was dismissive and unprofessional. When I asked her to confirm whether Dr. Avella had sent my prescription, she responded in an annoyed tone and rushed me to provide a urine sample. I only learned her name from the printed visit summary. After leaving the practice, my arm became swollen, painful, and later developed bruising, which I have documented in photos. In my entire life, I have never experienced swelling or bruising after a blood draw in any hospital or clinic. This was the worst blood‑draw experience I have ever had, caused by Kalinda Smith's poor technique and poor hygiene. She didn't even deserve one star for her unprofessional behavior. Crystal Run has always maintained high standards, which is why this experience was so disappointing. If Smith is not yet proficient in blood draws, patients should not be used as test subjects. She requires additional training in phlebotomy technique, hygiene, and patient communication to ensure safety and maintain the quality of care your practice is known for. For other patients: if you encounter Kalinda Smith, please be careful. Based on my experience, I strongly recommend not letting her draw your blood. I trust Crystal Run will take this matter seriously and address it promptly.

In conversations before the bris, I knew that my friend was nervous about the process, the ceremony, and the actual circumcision. I can say with confidence that Dr. Rumbak put the family's mind at ease both with her expertise and her gentle approach. As a "spectator" at the ceremony, Dr. Rumbak passed around sheets so we can all follow along and feel part of the ceremony, and explained what different blessings meant. She also added some Jewish text in a modern and relevant way, which I appreciated, and my family continued to discuss post ceremony. The sweet baby did super well and was calm throughout the entire process. Truthfully, I didn't even know when the actual circumcision actually happened because she was so gentle and discrete. At the end of the ceremony, many people went up to Dr. Rumbak to compliment her on one of the best brit mila's they ever went to, and many asked for her card (including my mom for our temple!). She has a great business partner in Shachar, who socialized with the guests of the ceremony, kept everything organized, and was just an all around stand up guy. Oh, and how great that she is a mohel AND a doctor. Talk about putting your mind at ease..that will definitely do it. Highly recommend.

Dr. Rumbak deserves credit and thanks for her discretion in not circumcising our son the day of his bris due to a urological condition she noticed in her ore-ceremony examination. We are thankful for that. She performed what essentially worked out to a baby naming ceremony for us. Obviously the day was emotional to begin with and the news she couldn't circumcise was extremely disappointing and heartbreaking since this was a day we looked forward to for so long. In the days after the bris we were able to absorb the events better and realized exactly what happened. To begin with, my wife asked for a program to keep as they were being collected (yes, collected), and the response was, "I collect them." She said the same to my mom (baby's grandma). We consider this ridiculous - even if the baby's grandparents couldn't have one, at least the parents should. When I emailed Dr. Rumbak about the issue I'm about to describe, I also mentioned the program and that part of my email was left unaddressed. Here's the major problem, though. We paid $800 for a circumcision plus $150 for travel from Manhattan to Nassau. We were happy to do this because we really liked Dr. Rumbak on the phone. However, after the bris, we had a bit of a realization that really irked us. Why were we never offered any kind of refund for the circumcision not being performed? It really should have been offered that day, on the spot, but it wasn't. Days and weeks turned to months and we still were never offered anything. We didn't even get an email or phone call to check on our son given the medical issue Dr. Rumbak identified. This was surprising given the emotional nature of the day but also because part of Dr. Rumbak's service (advertised on her web site and vouched for in her reviews) is her frequent follow up with the family. Finally after a few months, I emailed her explaining we feel we are owed some kind of refund due to the most critical part of the ceremony, the circumcision, not being performed. It is that expertise for which we paid. Dr. Rumbak issued a $200 refund, saying that the rate for a baby naming is $600. It's unfortunate such a beautiful day was soured by the experience we had. As new parents we feel we were, in a way, taken advantage of. Dr. Rumbak did ultimately give some money back, but we feel we shouldn't have had to ask. Experiences like this are what make people cynical about organized religion. The right thing to do would have been to give us a refund without us having to ask. We would not recommend her.

The most you can leave messages with the representatives. On 9/4 I was told by my insurance company I do not owe any co pays after receiving my bill. Since then to date i have been calling every week. I kept asking to speak to a supervisor or mgr. I was told there is no one to connect me to. Last Wednesday I called again and spoke to Ruth. She said, she will escalate the matter again to Brian. She said he will call me back. To date, Brian has not call me back. I called this morning n spoke with Fran she said she will leave another message. This is all these people are telling you. No one in management to speak with u or the billing dept. I need my money back. That's all I want. It belongs to me. If I do not get a call from Brian or anyone in billing I will escalate this matter to the business bureau. This is what these medical billing office do. Once your money taken, it is hell to get your refund. This is robbery. Scamming patients of the $$$. I will never pay any medical office ever a copay. I will request a bill. I hope other people see this review and do the same. STOP paying copay at any medical office. It is a scam going on amongst them. This is not the only medical billing dept I am going thr this with. They are all the same. It is so sad what it comes down to. No one you can trust even in the medical billing dept for your money back once it is taken.
